In addition to the full range of general services there are also a number of established subspecialty activities in the fields of Fetal Medicine, Minimal Access Surgery, Gynaecological Endocrinology and Gynaecological Oncology. The Directorate works closely with other local Departments, particularly that at Guys/St. Thomas and King s College Hospital. Currently, the only provision for termination of pregnancy are for pregnancies with fetal abnormalities. The Department is fully committed to women s choice and autonomy, and midwifery management of normal pregnancy. There is currently a particularly skilled group of midwifery managers leading the process of change in midwifery practice. The obstetric practice is however generally high risk due to the nature of the local population, requiring significant medical involvement in a substantial proportion of pregnancies. The Gynaecological service is well supported and there is ready access to laparoscopic camera systems in all theatres. Page 6 of 13
7 Within the Directorate there is a policy of inter-firm referral in areas of special interest in order to offer the best possible standards of care to our patients, fostered by a high degree of collaboration between the existing Consultants. We also maintain close relationships with other Directorates, particularly Children s Services whose administrative centre is shared with Women s Health as one Division. The fetal medicine unit at UHL is a well-established tertiary referral centre. The unit offers a full range of prenatal diagnostic procedures (including CVS and fetal blood sampling). There is a formal research link with the Harris Birthright Centre at King s which is planned to continue for the foreseeable future. 8 Sexual and Reproductive Health Department The Department of Sexual and Reproductive Health is consultant-led. It provides contraception as well as sexual and reproductive health services from 4 sites in Lewisham, on an open access basis, and outreach work to schools and colleges. The department offers contraception to level 3 of the National Sexual Health Strategy, and STI work to level 2, as well as cervical cytology and psychosexual counselling. We hope to add GU work to level 3 and an early medical abortion service. In 2010/11 there were 60,358 clinic attendances by 30,443 people, 19% of them men. 49% of the attendances were by people aged under 25. The department was one of the first to be computerised, using an EMIS electronic patient record Consultants Dr Elenor Draeger Consultant in Genitourinary Medicine HIV Department Since 1992, HIV services at UHL have been provided through the HIV/Haemophilia service and it is from this that the current service has evolved. The service became full-time in 2002 and patients are seen in the Alexis Clinic, a dedicated HIV Unit within the hospital. The service recently relocated to a purpose built facility within the main hospital. Lewisham is an area of high HIV prevalence with a rate of 6:1000. This ranks as the sixth highest HIV prevalence rate within the UK. Late presentation is a feature of the newly diagnosed patients and a significant proportion of patients are female and of child bearing age. Patient numbers continue to rise and there are now over 600 patients under regular follow-up. Numbers are expected to increase further with the anticipated increase in HIV testing from within primary to tertiary care. Outpatient care is delivered by a consultant led multidisciplinary team. Care for HIV inpatients is led by the HIV consultant with support from the medical teams at UHL. Out of hours and on-call cover is jointly provided with HIV consultants from Kings College Hospital as part of a clinical network. This network also facilitates shared care for complex patients, clinical governance, continuing professional development and also provides opportunities for collaborative research. The service falls within the wider SE Sector HIV clinical Network. The HIV also works closely with various disciplines within the hospital. In particular care is often shared with, haematology, rheumatology and chest medicine. Antenatal HIV care is provided jointly with the Obstetric service in Women s and Sexual Health. Paediatric HIV care falls under a dedicated team within paediatrics and the services are closely linked. The department has recently merged with Sexual and Reproductive Health. It is hoped that this will facilitate a comprehensive and integrated service for sexual Health and HIV for Lewisham residents. Consultant Dr C. Mazhude, Consultant HIV Dr. M. Medical Education & Teaching Participate and contribute to departmental meetings Provide training where required lectures for KCH medical students on a scheduled rota c. Clinical Audit and Clinical Governance Conduct and present regular audits d. Continuing Professional Development 1. Participate in continuing professional development at local and national level 2. Implement audit and research findings as appropriate in clinical practice 3. Prepare and participate in an annual appraisal in accordance with Trust policy 4. General Information about the Trust Trust Profile Lewisham Healthcare NHS Trust was established in August 2010, following the integration of The Lewisham Hospital NHS Trust and the Community Health Services of Lewisham Primary Care Trust. This will allow us to further improve the care we provide to residents of Lewisham, Bromley, Greenwich and surrounding areas. Lewisham Hospital itself is in the center of the London Borough of Lewisham and provides a wide range of elective and emergency healthcare to an urban residential population of around 265,000 including people from a broad sweep of socio-economic and ethnic backgrounds. The Trust also provides some emergency and tertiary elective services to residents of neighbouring Clinical Commissioning groups, particularly Greenwich, Bexley and Bromley. The hospital site, University Hospital Lewisham, is a campus for the Guy's, King's and St Thomas' School of Medicine. As a busy Acute General Hospital, our Trust gained University status in 1997 in recognition of the continuing role it plays in undergraduate education and research. Our health professionals also provide care to adults and children across a range of health centers, community clinics, and in patients own homes. Adult services include community matrons, district nurses, the diabetes team, the Home Enteral Nutrition (HEN) Team who provide care to people who need to be fed by a feeding tube, and the sexual and reproductive health team. Services for children and young people include health visiting, occupational therapy, physiotherapy and speech and language services. Lewisham Healthcare NHS Trust employs some 3,000 staff on both the hospital and community sites, which makes us one of the biggest employers in South East London. Management Structure The Trust Board is responsible for policy and the Trust Management Executive is responsible for operational management. The Trust Chief Executive is Tim Higginson and the Trust Board Chair is Elizabeth Butler. The Interim Medical Director is Dr Elizabeth Aitken, Consultant Care of the Elderly Women s and Sexual Health Children and Young People Specialist Medicine Acute and Elderly Medicine Surgical Services This structure helps us to draw upon the knowledge of experienced healthcare professionals in the community and hospital, who work closely together. Our focus is on providing the best services where they are needed be it a hospital, community health clinic or in a patient s own home. Page 10 of 13
11 Links with other Health Care Providers Lewisham Healthcare NHS Trust brings together University Hospital Lewisham (UHL), a district general hospital, and a range of community services in the borough. Although Lewisham Hospital NHS Trust formally separated from the Guy s and St Thomas NHS Foundation Trust in 1993, strong clinical and academic links have always been maintained. These have been further enhanced by the Trust s close involvement with Kings Health Partners (KHP), which is one of London s three Academic Health Sciences Centres. KHP was formed in 2009 by Kings College London (KCL), Guys and St Thomas NHS Foundation Trust (GSTT), Kings College Hospital NHS Foundation Trust (KCH) and South London and Maudsley NHS Foundation Trust (SLAM). SLAM provides local Mental Health services for adults and older adults on the Lewisham Hospital site (Ladywell Unit) and in the community. KCH has located its rehabilitation service and some inpatient services on the hospital site; it is also linked to Lewisham through the Stroke and Trauma Networks. Some specialist adult and children s services are shared with either KCH or GSTT. The Trust continues to maintain and build effective clinical and academic partnerships with the members of Kings Health partners. We also participate in the newly-formed South London Health Innovation and Education Cluster (HIEC). In addition to the recent integration with Lewisham Community Provider services, the Trust has also successfully tendered for the local Primary Care Musculoskeletal Service; we intend to build on this to provide other specialist services, for example local polyclinics. Medical Services Lewisham Hospital provides the full range of acute general hospital services: medical, surgical and emergency services for the local community. We also provide a range of specialist surgical services: ear, nose & throat, vascular and paediatric day care surgery, and services for patients with cystic fibrosis. Lewisham Hospital has 500 beds including a well-equipped 14-bed Critical Care Unit providing level 3 and level 2 care, a 5-bed Cardiac Care Unit and a 46-bed Medical Admissions Unit with 8 monitored beds providing level 1 care. The hospital has a highly rated Stroke Unit linked to the Hyper-acute Stroke Service at Kings College Hospital. The Vascular Surgery Unit at Lewisham Hospital specialises in carotid artery surgery. Lewisham Hospital is one of the few hospitals in London to have a separate children's Emergency Department - a service that is rated 'Excellent' by the Care Quality Commission as well as by parents and carers. The hospital has 11 operating theatres of which 2 are dedicated paediatric theatres and 1 is a dedicated obstetric theatre. In November 2006, the Trust opened the Riverside Wing, a 6- storey state of the art medical and surgical ward block. This block also contains the Riverside Treatment Centre with 4 of the Trust s operating theatres which enables the physical separation of elective and emergency surgery. The Treatment Centre also accommodates the new endoscopy unit, which houses the regional bowel cancer screening centre. The Endoscopy Unit is supported by three consultant gastroenterologists and three consultant respiratory physicians. Page 11 of 13
12 The Radiology Department offers a wide range of imaging modalities including MRI, CT, Interventional Radiology and Nuclear Medicine. A new CT scanner was installed in April Comprehensive Pathology services are also available on site with consultants in each specialty. The Maternity Unit is responsible for about 4000 deliveries per year and in May 2010 we opened a Midwife Led Birthing Unit (MLBU). Plans for an Urgent Care Centre (UCC) are at an advanced stage and will be opened in the near future. As well as hospital services, Lewisham Healthcare NHS Trust has over 700 staff members in the community, providing and supporting a range of adults and children s services. Academic Activities and Research The Trust has a strong record in medical education and we play a key role in the education of tomorrow's doctors and nurses. Our hospital is one of the leading providers of clinical placements for medical students, and pre-registration nursing and midwifery students. We have over 200 commissioned students passing through our wards and departments at different times of the year. The Trust has a strong record in medical education. Undergraduates from the KCL School of Medicine are assigned to Lewisham in years 3, 4 and 5 of their training. The Trust also trains a large number of postgraduate medical trainees from Foundation to completion of their specialist training in all disciplines. Both trainees and students value their attachment at Lewisham highly. Medical education has been a trust priority for some time and this has recently been reaffirmed by the appointment of a Director of Clinical & Academic Strategy to the Trust board in Furthermore, there are plans to augment the existing education faculty by appointing an Education Lead for Simulation who would oversee a multi-disciplinary simulation training facility. From August 2010, the hospital and the community services will be uniquely integrated in one Trust. The simulation programme will therefore provide an excellent medium of education for the entire healthcare system of the Lewisham Borough. We aim to co-ordinate all existing educational activity based on simulation in a structured programme in a central purpose-built simulation facility. This novel method of learning will further enhance the trust s commitment to excellence in healthcare education. The Trust has made a major commitment to the development of Research and Education with dedicated centres comprising lecture theatres, seminar rooms, a multi-disciplinary library, a Computer Assisted Learning Room and Medical Illustration/Graphics capacity. The Trust encourages its staff to undertake research supported by the Research & Development Unit jointly led by the Trust Research & Development Director ( Prof Gabrielle Kingsley) and the Director of Knowledge, Governance and Communications (Mrs Joy Ellery). The Trust participates in a range of research projects linked to the local Comprehensive Research Network (CLRN), including studies in Cancer, Stroke, Medicines for Children, Rheumatology, Neurology and Critical Care. An annual Research and Clinical Effectiveness day and Academic Report further promote the academic activities of the Trust. 13 Location, Housing and Transport There is a wide variety of housing available in the North Kent area and many of the consultant staff live in nearby Blackheath, Bromley, Beckenham or Greenwich where there are good state and public schools as well as a variety of shopping centres. Most of the hospital wards overlook the Ravensbourne River and Ladywell Park with lawn tennis courts and a recreation area. The hospital is located on Lewisham High Street and is very well served by public transport: bus, rail and DLR. Ladywell station is only a five minute walk from the hospital through Ladywell Park and provides a convenient quarter hourly train service. Ladywell Station is on the railway line between Hayes in Kent and London Bridge or Charing Cross in Central London. Lewisham Station is 10 minute walking distance or a short bus ride away from Lewisham Hospital. Trains from Victoria, Cannon Street and London Bridge Stations run through Lewisham Station to all areas of Kent. The DLR terminates at Lewisham Station. Bus routes to Lewisham High Street are as follows: 208-Orpington via Bromley, 54-West Croydon, P4-Brixton, 199-Canada Water, 47- Deptford, 284-Grove Park Cemetry, 75-Croydon via Sydenham, 122-Crystal Palace/Plumstead, 136-Peckham/Grove Park, 185-Victoria, 484-Camberwell.
