2 de Souza Institute Mission: To inspire and empower health care professionals to provide the best cancer and palliative care through education and mentorship. About de Souza Institute: In 2008, the Ontario government funded de Souza Institute to support nurses that care for cancer patients and their families. In 2013, de Souza Institute expanded its mandate to include other health care professionals. Through de Souza Institute, health care professionals obtain the specialized skills to guide and help cancer patients throughout their journey (prevention, screening, diagnosis, treatment, survivorship and palliation). de Souza Institute courses are offered primarily online with the occasional course through webcast or in-person. de Souza Institute is pleased to partner with the Ministry of Health and Long-Term Care, Cancer Care Ontario and University Health Network to offer the best continuing education in cancer and palliative care. 4 de Souza Designations What is a de Souza Designation? A de Souza Designation is awarded to a healthcare professional who has completed our comprehensive learning pathway. de Souza Designates are well-regarded health care professionals possessing strong clinical and leadership skills in oncology or palliative care. They have the up to date knowledge to deliver complex treatments, alleviate pain and other symptom related suffering and address psychosocial needs of patients and families.. Current designations are open to nurses and we are developing a system for designation for other health care professions. Once a designation is achieved, it must be maintained through ongoing professional education and practice. Below are details on each designation: de Souza Intern For nursing students with an interest in oncology and/or hospice palliative care Earning a de Souza Intern Designation requires: Obtaining a total of 2.0 credits; by completing at least one course from each of the four domains of practice Passing the Foundations in Oncology Nursing Practice course Completing a clinical placement in an area of practice that provides cancer or hospice palliative care Successful completion of a nursing program, and have a valid licence issued by the provincial/territorial regulatory body de Souza Nurse Associate For RNs or RPNs, who are not specialized in oncology or hospice palliative care, but are caring for patients with or at risk of developing cancer, or providing palliative care Earning a de Souza Nurse Associate Designation requires: Obtaining a total of 4.0 credits; by completing at least one course from each of the four domains of practice Passing the Foundations in Oncology Nursing Practice course and/or the Provincial Standardized Chemotherapy and Biotherapy course At least one year (1950 hours) of continuous clinical experience Having current registration and being in good standing with the College of Nurses in a Canadian province or territory de Souza Nurse For RNs who are certified in oncology, hospice palliative care or pediatric oncology Earning a de Souza Nurse Designation requires: Obtaining a total of 4.0 credits; by completing at least one course from each of the four domains of practice Passing the Foundations in Oncology Nursing Practice course and/or the Provincial Standardized Chemotherapy and Biotherapy course Completing a 75 hour clinical fellowship Certification in oncology or hospice palliative care by CNA, CPHON or ONCC Having current registration and being in good standing with the College of Nurses in a Canadian province or territory de Souza APN For Master s prepared RNs, Nurse Practitioners, Clinical Nurse Specialists, Educators and Managers who are certified in oncology, hospice palliative care or pediatric oncology Earning a de Souza APN Designation requires: Obtaining a total of 4.0 credits; by completing at least one course from each of the four domains of practice Passing the Foundations in Oncology Nursing Practice course and/or the Provincial Standardized Chemotherapy and Biotherapy course Completing a 150 hour Clinical Fellowship and a research project Certification in oncology or hospice palliative care by CNA, CPHON or ONCC Having current registration and being in good standing with the College of Nurses in a Canadian province or territory de Souza Scholar For PhD Nurses in an oncology or hospice palliative care related field; nurse executives, managers, professors or scientists Earning a de Souza Scholar Designation requires: Completing a de Souza research mentorship/fellowship Publication in a peer-reviewed journal within the four domains of practice Receiving grant funding in research, education or leadership in Oncology or Hospice Palliative Care Having current registration and being in good standing with the College of Nurses in a Canadian province or territory 3

6 Interprofessional Psychosocial Oncology Distance Education (IPODE) Course Series Interprofessional Psychosocial Oncology Introduction to Theory & Practice (IPODE) Psychosocial Oncology is a specialty area of cancer care that is concerned with the myriad of psychological, emotional, social and spiritual responses of persons affected by cancer. This 13 week web-based course provides an opportunity for practicing health professionals to explore common psychosocial issues that individuals and families experience when an adult family member is diagnosed with cancer. Primary emphases in the course include understanding the illness experience and assessing distress and coping from a family centered perspective. The course draws on a case-based, narrative format and provides opportunities to learn in small groups of interprofessional learners. Sexual Health in Cancer Part 1 & Sex Counselling in Cancer Part 2 (IPODE) The purpose of these two 7 week courses is to assist health professionals in cancer care to integrate sexual health care in practice. Sexual concerns are one of the most common problems encountered in cancer care settings; yet they are the least likely to be discussed unless asked. The framework for Part 1 encourages course participants to: 1) Address their own and society s, attitudes and biases towards sexuality and sexual health. 2) Increase knowledge regarding functional and dysfunctional aspects of human sexuality; and 3) Develop knowledge and skill with regard to assessment and educational interventions for common sexual health issues in cancer care. Attention to issues of diversity will be integrated throughout. Part 2 develops more advanced knowledge and skill in: 1) Evidence based interventions for common cancer related sexual issues. 2) Analysis of case studies to develop treatment plans; and 3) Interprofessional knowledge and skill. Relational Practices & Families in Oncology & Palliative Care (IPODE) Emphasis of this 13 week web-based course will be on exploring the interprofessional care of families experiencing cancer along the illness trajectory from diagnosis through to bereavement or long term survivorship. Using case based learning in small interprofessional groups; participants will explore a variety of key learning themes relevant to the interprofessional care of families. Themes that will be addressed include: family theory, models of family & couple counseling/ therapy, family assessment, therapeutic conversations and interventions. Case examples will be drawn from the experience of families across the cancer illness trajectory, from diagnosis through to death and dying, bereavement and long term survivorship. Small group work will allow participants to develop a rich understanding of the cancer experience from the perspective of families and to build competency in family assessment, intervention, interprofessional collaboration, and cultural safety. Attention to diversity will be integrated throughout the course. How will these courses be delivered? The main mechanisms to facilitate learning in these courses will entail elearning, independent study of the course materials, and, participants will meet in a real-time online seminar for 60 to 90 minutes every week. A range of innovative learning resources have been developed specifically for these courses, including case study narratives, video-taped interviews, and dramatizations of individual, family and interprofessional situations. 7 Treatment & Delivery of Evidence Based Care Courses by Domain Advanced Pain Assessment and Management Examines the pathophysiology of cancer pain; cancer pain syndromes, pharmacological and non-pharmacological intervention for intractable pain in palliation. Participants will explore methods on how to conduct educational and advocacy work. Credits: 0.5 Learning Hours: 24 Format: Online & Webcast Foundations in Oncology Nursing Practice Introduces the art and science of oncology practice. Fundamental concepts of cancer care will be presented for those who work with cancer patients. Insight into the four most common types of cancer: breast, lung, prostate and colorectal will be offered. The content is optimized for nurses and is open to other regulated health care professionals. 11 Meet the Faculty de Souza Institute Faculty Dr. Mary Jane Esplen, PhD, RN Dr. Mary Jane Esplen is the Director of de Souza Institute, Clinician-Scientist at the University Health Network and Professor in the Department of Psychiatry, Faculty of Medicine, University of Toronto. Dr. Esplen continues to have a practice in psychosocial-oncology and conducts research to develop new measures for the field of psycho-oncology or interventions, including group therapy interventions or counseling approaches for women and men undergoing genetic testing or who are dealing with a current cancer and its impacts. She has an interest and expertise also in approaches to managing grief and loss, conducting workshops for health professionals. Donalda MacDonald, RN, CON(C) Donalda has been an oncology nurse for 40 years, working in both inpatient and outpatient units. For the last 25 years she was a primary nurse in lung and GI oncology as well as working in the chemotherapy suite. For the last 10 years of her clinical career, she has been the solo instructor for the Sinai C.A.R.E. Cancer Care education series that she and her chemo colleagues developed targeting nurses new to oncology. In 2008, Donalda was nominated by her oncology peers and was one of 100 nurses from across Canada to receive the CNA Centennial Award honoring her work in education of novice oncology nurses. In 2014, she was again honoured by receiving the Lifetime Achievement Award from CANO. Komal Patel, RN, BScN (Honours), MN, CON(C), CHPCN(C), CVAA(C) Komal began her nursing career at William Osler Health System in Brampton, Ontario in 2006 on an in-patient oncology unit. Komal holds a BScN (Honours) from the University of Windsor and an MN with a teaching focus from Athabasca University. She obtained her Oncology CNA certification in 2010, Hospice Palliative Care CNA certification in 2012, her CVAA certification in 2014 and her de Souza APN Designation in Dr. Jiahui Wong, MD, PhD Dr. Jiahui Wong has over 15 years of experience in research and program evaluation in pharmaceutical, educational, and health care contexts. Jiahui has an MSc in Epidemiology from Laval University in Quebec and a PhD from the Institute of Medical Science, University of Toronto. She completed her postdoctoral training in Randomized Clinical Trials at the University of Toronto. Her recent research activities include instrument development, phase 3 clinical trials as well as program evaluation in nursing. External Faculty Aronela Benea, RN, BScN, MScN Aronela Benea is an Advanced Practice Nurse at Women s College Hospital in the After Cancer Treatment Transition (ACTT) Clinic. She completed her RN training in Romania in Aronela obtained her BScN degree (Internationally Educated Nurses Program, 2009) and her MScN degree (2012) from York University. She has worked in oncology clinical care and research since In her current role she provides cancer survivorship care to patients with various cancers. Patricia Bieronski, BScN, RN CON(C) Patricia has been involved in oncology nursing for over 35 years in both the inpatient and outpatient setting. Patricia graduated from the University of Western Ontario with a BScN and has held national certification in Oncology from the Canadian Nurses Association since She has worked in different roles including: staff nurse, resource nurse, charge nurse, palliative care coordinator, nurse manager, Education Practice Lead and oncology clinic nurse. Her current position is a medical oncology clinic nurse at the Grand River Regional Cancer Centre in Kitchener. In this role Patricia works in partnership with patients diagnosed with solid tumors and their families. She helps them navigate the system from consult through treatment and initiates transition to survivorship while addressing information and education needs, symptom management, distress, and achievement of cancer care related goals. Susan Blacker, MSW, RSW Susan is the Director of Cancer Services Planning and Performance at St. Michael s Hospital in Toronto, Ontario, and a Psychosocial Oncology lead for the Toronto Regional Cancer Program. Susan is an Adjunct Professor at the Factor-Inwentash Faculty of Social Work, and the Quality Co-Lead, Division of Palliative Care, Department of Family & Community Medicine at the University of Toronto. She is a recognized leader and educator in palliative care and has been involved in many palliative care initiatives in Canada and the United States. She is a founding board member of the Social Work in Hospice and Palliative Care Network. Dr Paul Daeninck, MD, MSc, FRCPC Dr. Paul Daeninck is a medical oncologist and palliative medicine consultant with CancerCare Manitoba and the Winnipeg Regional Palliative Care Program. He is Chair of the Symptom Management and Palliative Care Disease Site with CancerCare Manitoba, and an Assistant Professor and Leader for the Palliative Care Longitudinal Theme at the Max Rady Medical School of the University of Manitoba. Dr. Daeninck works with several national organizations, including the Canadian Society of Palliative Care Physicians, the Canadian Partnership Against Cancer and the Canadian Consortium for the Investigation of Cannabinoids (CCIC). Dr. Daeninck is the author of several research papers as well as textbook chapters and serves as a reviewer for palliative medicine journals. He has an ongoing research interest in cannabinoid use in patients with cancer. Valrie Hursefield, RN, BA, MHA, CHPCN(C), CVAA(C) Valrie Hursefield, a clinical nurse educator at the William Osler Health System, has specialized in oncology and palliative care since Val completed her nursing education in Bedfordshire, England, and holds a BA in Health Care and a Master s Degree in Health Administration. Val completed the Oncology Nursing Distance Education Course (ONDEC) certificate course in 2011, attained the CNA Hospice Palliative Care (CHPCNc) certification in 2012, and the Canadian vascular access certification (CVAAC) in She is also the third recipient in Ontario of the de Souza APN designation; having completed a clinical fellowship research project on advance care planning. Outside her clinical role, Val is dedicated to supporting academic and career progression of nurses and other health care professionals, in her role as a course lecturer with Humber College. Cathy Kiteley, APN, RN, MScN, CON(C), CHPCN(C Catherine Kiteley is a clinical nurse specialist in the area of oncology and palliative care. She has both her Oncology and Palliative certification from the Canadian Nurses Association. Over her career, Cathy has engaged extensively in research, professional practice and education. She holds a master s degree in nursing from U of T and has been involved in teaching activities at de Souza Institute, LEAP and the University of Toronto. Prior to retiring from her full time role at Trillium Health Partners, she was an active clinician and regularly mentored nursing, inter-professional and medical students in oncology and palliative care. She was part of the development team for several CCO symptom management guides. She taught and evaluated workshops in emotional care, led an innovation in triaging and navigating palliative care referrals at Trillium, and she also facilitates numerous programs at Wellspring cancer wellness centre. Janet Lovegrove, RN, BScN, MSc, CPMHN Janet Lovegrove, RN, BScN, MSc, CPMHN, has taught the introductory Fundamentals Hospice Palliative Care course while teaching undergraduate nursing students at McMaster University. As an Associated Medical Services (AMS) Phoenix Call to Caring Fellow at McMaster University s School of Nursing, she led an interdisciplinary team advocating for the self-care needs of caring professionals (their AMS Phoenix Fellowship project titled, Bringing Transformational Change to Health Care Education, Practice, and Community Settings continues to help interested caring professionals in various educational, clinical, and community settings to navigate the world of growing technological care with care and compassion for self). In January and February of 2015, together with Viola Fodor her AMS Phoenix mentor, Janet co-facilitated twelve Self-Care for Caring Professionals oneday workshops for critical care nurses at University Health Network. 10

12 Meet the Faculty - Continued Dr. Madeline Li, MD, PhD Dr. Li is an assistant professor in the Department of Psychiatry, University of Toronto and a clinician scientist in the Department of Supportive Care, Princess Margaret Cancer Centre. In addition to clinical practice in cancer psychiatry, she conducts research in psychoneuroimmunology and psychosocial oncology. She has authored several publications, guidelines and interprofessional education resources on the management of anxiety and depression in cancer and is the developer and physician lead of the Distress Assessment and Response Tool (DART) program. Charmaine Lynden, MN, RN (EC), CON(c) Charmaine holds a MN and works as a Nurse Practitioner in Radiation Oncology at Mississauga Halton Central West Regional Cancer Centre at Trillium Health Partners, Credit Valley Hospital site. She graduated from the University of Toronto in 2008 and has worked in both Radiation and Medical Oncology. Her clinical work involves running a NP-led symptom management clinic for patients receiving combined modality treatment for thoracic and pelvic malignancies. Her other areas include primary CNS tumors, and Stereotactic Body Radiotherapy (SBRT). Judy Simpson, RN, BN, MEd, CHPCN(C) Judy Simpson s nursing career spans more than forty years and consists of diverse roles, including staff nurse, educator, administrator, mentor, consultant and volunteer - most of which were devoted to the advancement of palliative care nursing in Canada. Since retiring in 2009, she has been a consultant in Palliative and End-of-life and has been the primary lead for the EPEC-O Canada project. She has a strong background in interprofessional education, developing, leading and evaluating projects and has co-authored standards, competencies and practice guidelines. Judy s accomplishments have been recognized through a number of honours and awards. Dr. Mary Vachon, RN, PhD Dr. Mary Vachon is a nurse, clinical sociologist, psychotherapist, educator, and cancer survivor. She is currently a Psychotherapist in Private Practice, Adjunct Professor in the Department of Psychiatry and Dalla Lana School of Public Health at the University of Toronto and Clinical Consultant at Wellspring. Dr. Vachon is the author of Occupational Stress in the Care of the Critically Ill, Dying and Bereaved. She has published over 170 scientific articles and book chapters and delivered over 1,600 lectures around the world on topics related to bereavement, occupational stress, lifethreatening illness, palliative care, survivorship and spirituality. IPODE Course External Faculty Marianne Arab, MSW, RSW Marianne currently works at Cancer Care Nova Scotia, the provincial cancer agency in Halifax, Nova Scotia as the Manager of Supportive Care and Psychosocial Oncology. Her career experience includes working as an inpatient clinical social work in the areas of Family Medicine, Oncology and Palliative Care. She is co-lead of a federally funded initiative training paramedics to provide palliative care at home for patients in crises requiring palliative support who do not wish to be transported to their local emergency department. As well she has been instrumental in the adoption of screening for and management of distress in oncology clinics across Nova Scotia. She has been an on-line facilitator for the IPODE continuing education course since Karma Guindon, MSW, RSW, PhD (candidate) Karma is a registered social worker and registered marriage & family therapist. She uses a systems-based and integrative approach to practice that is largely informed by postmodern principles. She presently works in private practice providing couples and family therapy, and individual therapy to children, adolescents and adults. Many of her clients seek her services to address the psychosocial impacts of chronic and acute illness and loss. Karma practiced for 15 years at a child & family therapy program at an Ontario teaching hospital. Her clients included children receiving cancer care, and their families. Karma chaired the hospital s education committee, which was dedicated to improving front-line professionals knowledge of psychosocial issues. Karma has also practiced in the areas of childhood sexual abuse, adolescent eating disorders, and adult mental health. She has provided supervision to students and therapy trainees, and consultation to health professionals. Karma is a PhD candidate at the Faculty of Social Work, Wilfrid Laurier University. Her research is exploring contemporary professional ethics for social work therapy practice. She has her Bachelor of Science degree and Master of Social Work degree, both from the University of Toronto. Karma has extensive teaching experience at the college, university, and professional development level. She has been an online facilitator with IPODE since She is also an instructor with the Social Work Professional Development program at Wilfrid Laurier University. She regularly presents at national and international conferences. Karma co-authored a therapy workbook for teens, which was published in Lisa Roelfsema, MSW, RSW Lisa Roelfsema is a clinical social worker with over 16 years experience at Trillium Health Partners, the last 10 specializing in working with patients and families affected by Cancer. Part of her role at the Mississauga Halton/ Central West Regional Cancer Centre, involves facilitating the Women s Sexual Health Group Program for women who have had any type of cancer. Lisa is also a facilitator of the on-line Gynegals group for women with gynecological cancer experiencing sexual side effects. Lisa also has a long standing interest and additional training in the area of the impact of illness on sexuality and sexual function. Lisa has a private practice in Streetsville for individuals, couples and families struggling with medical illness and related spirituality, sexuality, relationship issues, grief and loss concerns. Sandra Rotholc, MSW, RSW, RMFT, RST Sandra Rotholc has been practicing for over 18 years as an Individual, Couples, Family and Sex Therapist. She has a special interest in coaching people to have healthier relationships. She is a Registered Psychotherapist, Registered Social Worker, Registered Marriage and Family Therapist and Registered Sex Therapist. Sandra now lives in Toronto, where she has her private practice continuing to focus on Individual and Couples Psychosexual Therapy. Sandra worked at the BC Cancer Agency in Vancouver. She offers sex education talks on Sex and Cancer through ELLICR, Wellspring and Gilda s Place. Marianne has her Bachelor of Social Work from McGill University and her Masters in Social Work from Dalhousie University. 11
