1 Health Home White Pages As a follow-up to the recommendation made at the December 12, 2012 meeting, the Bronx Health and Housing Consortium has compiled the Health Home White Pages, a directory of contact information for Health Homes in the Bronx. The White Pages are organized in two tiers: (1) the executive management/policy level contacts and (2) the care management/program-to-program level of communication that is client/patient specific in most instances.
