9 Implementation Anyone that can afford to pay for services should Some services have been collecting a contribution for many years Analyse you current contributions are you anywhere near 15% of your CHSP grant How can you achieve that? Ask staff/clients for ideas Current examples: flat fee for all services, gold coin donation, monthly subscription, combining services and offering a package deal, Start slowly and communicate, communicate, communicate like any other change process
10 HOME CARE PACKAGES
11 CHSP & HCP Manual In defined circumstances, however, a Home Care Package client may access Commonwealth Home Support Programme services in addition to the services they are receiving from their Home Care Package budget (that is, the additional Commonwealth Home Support Programme services will not be charged to the client s individualised budget). These circumstances include: o Where the Home Care Package client s budget is already fully allocated, a Level 1 or 2 Home Care Package client can access additional, short-term or episodic Allied Health and Therapy services or Nursing services from the Commonwealth Home Support Programme; o Where the Home Care Package client s budget is already fully allocated, and a carer requires it, a Home Care Package client can access additional planned respite services under the Commonwealth Home Support Programme; o In an emergency (such as when a carer is not able to maintain their caring role), where a Home Care Package client s budget is already fully allocated, additional services under the broader Commonwealth Home Support Programme can be obtained on an emergency or short term basis. o These instances should be time limited, monitored and reviewed 11

13 Flexibility Provisions 80/20 sub programme output flexibility what does it mean where a service provider receives a large volume of referrals from My Aged Care for clients requiring social support, but less than the level of referrals expected for personal care, it may need to utilise a small portion of the funding it receives for personal care to deliver social support In essence the flexibility provisions are there to help providers manage their services flexibly when demand varies The general rule is use the flexibility to meet local need and if it exceeds the allowable level of flexibility talk to the Dept about a change in grant agreement MAC referrals and DEX data over time will show what at the real demand is Contradictions referrals from MAC what about existing clients?
14 Examples Can a provider allocate up to 20% of the Personal Care (PC) funding to Transport even though the PC service hasn t yet reached 80% (or can the flexibility only be applied once a service is delivering 80% of its funded outputs)? Response don t need to reach 80 per cent of the PC first. However only 20% can be allocated elsewhere so if it looks like you won t hit the 80%, particularly if not getting adequate referrals, you should tell the Department early as it might mean they need to reconfigure the agreement 2. Given the significant demand for Transport, could Transport outputs be taken above 120% (adding 20% of the PC funding to Transport would allow the Transport service to operate at 143% of funded outputs, due to the different funding amounts for each service). Response - a shift that big requires a conversation with Department to reshape the funding agreement.
15 Data Exchange (DEX) Nov New way of reporting program activities (first full reporting period Jan- Jun2016) No interface with MAC so need to set up your outlets Client Management systems need update to DEX compatibility No CRM - HADs in NSW a major issue and ACS working with ADHC & DSS for a solution Long term the DEX and MAC will allow greater levels of data for planning
