NAC is currently seeking a Homefinder (Social Worker) for the agency s Foster Care and Adoption Program. Department: Foster Care and Adoption Position Type: Full-time Key Responsibilities: To assess and prepare prospective foster/adoptive families who will provide homes for children in need. Facilitates the matching process of children referred for placement with the most appropriate family. Collaborates with agency Recruiter to identify prospective families through community outreach, recruitment fairs, and orientation programs. Assesses foster parent applicants for their potential suitability as a foster/adoptive parent through in home and office interviews to all household members. Co-trains prospective foster families in approved pre-certification training curriculum and assists Foster Parent Trainer in ongoing recertification foster parent training for licensed foster parents as necessary. Completes all ACS and NAC paperwork in a timely manner and ensures that documentation in foster parent records conforms to agency, City and State regulatory requirements. Maintains caseload through CONNECTIONS, including bed availability, training, and annual recertification of assigned families. Maintains hard copy of foster parent records. Accepts referrals for placement and facilitates appropriate placements into certified homes. Participates in Family Team Conferences and Case Reviews as needed. Responsible for 24-hour on-call duty coverage on a rotating basis. May provide individual therapy and/or group work services as assigned. May be asked to assist other units on an as needed basis. Minimum Requirements: MSW from an accredited school of social work; LMSW preferred. Child Welfare experience; prior work in foster care preferred. Prior experience working with diverse cultural backgrounds. Spanish language skills a plus.

3 JOB TITLE: Care Manager New Alternatives for Children, Inc. (NAC) is a child welfare agency in Midtown Manhattan that serves children with medical complexities, significant disabilities or severe emotional or behavioral conditions and their families through a range of clinical programs, social services and onsite medical and mental health clinics. NAC is participating in the Children s Health Home program and is seeking a qualified individual to apply for this position. DEPARTMENT: Children s Health Home Program REPORTS TO: Care Manager Supervisor PROGRAM DESCRIPTION The Children s Health Home Program is a NYS initiative that serves children on Medicaid (up to age 21) with complex medical and/or behavioral conditions, and provides coordinated care management services to improve their care and reduce costs. This is done primarily through the work of a CARE MANAGER who is responsible for ensuring the overall provision and coordination of services to children in an assigned caseload, working in collaboration with an interdisciplinary team. The CARE MANAGER guides program enrollees and their caregivers (legal guardians) through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. MAJOR DUTIES & RESPONSIBILITIES: Works closely with the child and their caregiver(s) as well as the child s care team at NAC and in the community. Ensures required program enrollment consents are obtained from the caregiver/consenter. Completes initial and ongoing needs assessments (Child and Adolescent Needs and Strengths; CANS-NY) to determine the child s most appropriate level of care management. Enters CANS-NY data into NYS online portal. Responsible for the overall creation and management of the child s Individualized Plan of Care (POC), by working collaboratively with the child and family and the child s care team. Based on the POC, the Care Manager is able to: A. Coordinate the enrollee s provision of services, as per their acuity level. B. Support adherence to treatment recommendations. C. In consultation with clinical and health care staff, monitor and evaluate a child s needs, including prevention and wellness care, conditions affecting the child s medical and mental health, care transitions, and social and community services where appropriate. Meets client contact requirements, including face-to-face visits in the child s home on a consistent schedule as per the acuity level determined for each child (high, medium, low).

4 Meets Care Management documentation requirements in a timely and accurate manner by effectively utilizing designated Care Management Portal (Medicaid Analytics Performance Portal; MAPP) and Electronic Health Records (EHRs) as needed Functions as an advocate for clients within the agency and with external service providers. Promotes prevention and wellness care linking enrollees with resources and services based on their individual needs and preferences. Works with health care staff at the agency and in the community to educate the child/caregiver(s) on care of chronic conditions, importance of immunizations, screening tests and other preventive interventions. Helps clients obtain health care services covered by Medicaid or their Managed Care Plan. Effectively communicates and shares information with the child and family/caregiver(s) with appropriate consideration for language, literacy and cultural preferences. Coordinates and conducts care planning meetings/conferences and serves as an interdisciplinary team member to effectively provide/coordinate comprehensive and holistic care. Identifies available community-based resources and actively manages appropriate referrals, access, engagement, follow-up and coordination of services. In the event of hospital admissions, participates actively in the discharge planning process to ensure that the transition home is supported by the provision of necessary services and treatments, all in place at the time of discharge. Attends and participates in ongoing staff development trainings to enhance skills needed to effectively meet the demands of the Care Manager position. Ensures child has periodic evaluations and follow up treatment for dental, vision and hearing care, following Medicaid EPSDT guidelines. All other duties, as required by agency. QUALIFICATIONS & COMPETENCIES: A Master s Degree in Social Work, Health Education, Community Health or related field. A Bachelors of Art or Science Degree with demonstrated experience in care coordination, case management, health education, health advocacy or related area. Work experience in a Bridges to Health, Care at Home, Targeted Case Management programs a plus. The ideal candidate will possess the following experience: Experience serving children and families in child welfare, developmental disabilities, mental health, health care and/or other systems. Experience providing service coordination and information, linkages, and referrals for community-based social and/or health care services. In addition, the candidate must demonstrate the following competencies: Excellent communication and organizational skills. Able to work collaboratively with an interdisciplinary care team. Demonstrated initiative and interpersonal ability to communicate with a variety of providers and negotiate/advocate on behalf of clients. A passion for serving children and families with special needs. Ability to travel to community-based agency offices, participant s communities, and homes within assigned catchment area. Proven self-management abilities, including meeting deadlines, ensuring compliance with agency policy and procedures, and overseeing complete and timely maintenance of agency records, in accordance with contractual requirements.

6 JOB TITLE: Per Diem Waiver Service Provider CITYWIDE OPENINGS New Alternatives for Children, Inc. (NAC) is a private, non-profit health care and social services agency in NYC that provides services to children with special medical needs and their families. The agency s Bridges to Health Program provides family and community support services to children in foster care and their caregivers so that these children can remain in the community and avoid placement in institutional settings. These services complement existing foster care services for children who are medically fragile, developmentally disabled, or suffering from serious emotional disturbances, and can involve multiple family networks and follow the participant after the foster care case has closed. We are currently seeking Per Diem Waiver Service Providers in all five boroughs (Manhattan, Brooklyn, Queens, the Bronx, Staten Island) for the Bridges to Health Program. DEPARTMENT: Bridges to Health SUPERVISOR: Supervisor of Waiver Services RESPONSIBILITY: Under the general direction and supervision of the assigned Waiver Service Supervisor, the Waiver Service Provider interacts with clients and families in a variety of settings including biological, preadoptive and foster homes, schools, neighborhoods, community and group care settings, and will deliver the waiver services that are outlined below. DUTIES: Demonstrates skills that promote a safe physical and emotional space for children and families within the context of their living environment. Provides a variety of waiver services to youth and family, including Family/Caregiver Supports and Services, Skill Building, Special Needs Advocacy, Crisis Avoidance and Response Services, Intensive In-home Supports, Planned Respite, Prevocational Support and Supportive Employment. Services assigned will depend on education, experience, and availability. Develops Detailed Service Plan goals, and if providing higher level services, are responsible for full completion of these plans every 6 months. Reports on progress towards these at service plan reviews and team meetings. Assists in crisis avoidance as necessary, with elevated levels of responsibility depending on the service being provided and clinical experience. Offers availability to provide services at least one weekend day and after-school/evenings (some services are also available during weekday daytime hours.) Maintains a consistent and regular schedule with clients. Communicates regularly with NAC staff and responds to and phone calls within 24 hours. Completes all Medicaid documents within prescribed 24 hour timeframe. All finalized hard copy documents are due within 5 business days of service but must be approved via first.

7 QUALIFICATIONS: Educational experience may vary, however, services and cases assigned, as well as financial compensation, are commensurate with experience. Bachelor s degree required with a major in Education, Psychology, Sociology, Social Work, Human Services or other related field. Master s degree preferred with a major in Education, Psychology, Sociology, Social Work, Human Services or other related field. Master s degree required to provide Crisis and Special Needs Advocacy Services. One year of experience working with children and families, preferably children with special needs. Two or more years preferred. Must be willing to work in participants' homes and communities. Our Kinship Care Program is designed to deliver a range of clinical, case management and supportive services to Kinship care providers and the children that they are caring for. Participants in NAC s Children s Health Home Program are drawn from the agency s other programs as well as from community referrals. The Children s Health Home Program is a NYS initiative that serves children on Medicaid (up to age 21) with complex medical and/or behavioral conditions, and provides coordinated care management services to improve their care and reduce costs. The CARE MANAGER SUPERVISOR provides administrative and clinical supervision to 5 8 Care Managers in the delivery of healthcare service coordination to a caseload of medically and behaviorally complex children. DEPARTMENT: CHILDREN S HEALTH HOME PROGRAM REPORTS TO: DIRECTOR, CHILDREN S HEALTH HOME PROGRAM MAJOR DUTIES & RESPONSIBILITIES: Provides direct clinical and administrative supervision to 5 8 Care Managers who oversee a total caseload of approximately 100 medically and/or behaviorally complex children (includes weekly individual supervision). Close supervision of Care Managers in engaging children and families with appropriate consideration of health literacy and cultural preferences, identifying resources and services based on their individual needs and preferences, promoting appointment compliance and adherence to treatment recommendations, and advocating on child s behalf with service providers in the community. Works closely with providers involved in child s care in the community and within the agency (e.g., staff in Foster Care, Preventive Services and Aftercare programs, nursing, the medical and mental health clinics) to ensure interdisciplinary input in care coordination functions, and promote collaboration and shared planning between Care Managers and providers. Oversees Care Managers administration of the CANS-NY (Child and Adolescent Needs and Strengths) to ensure reliability; provides ongoing staff training to maintain high quality assessments. Supervises and monitors performance of Care Managers in carrying out wide range of functions that include:

13 o o o o o Collecting assessment information from providers to determine client enrollment eligibility; Accessing online IT systems to enroll clients in the Health Home program, enter CANS- NY data, care management activities, progress notes, billing data, etc. Coordinating meetings with child s family and care team members to develop the client's Individualized Plan of Care (POC); Identifying available community-based resources based on the POC and managing service coordination and utilization, including service referrals, access, engagement and follow-up, and ensuring communication among providers; Meeting client contact requirements, including face-to-face visits in child's home on a consistent schedule. Monitors Care Managers in completing required documentation in a timely and accurate manner and entering data into the Care Management online platform, the NYS Medicaid Analytics Provider Portal (MAPP), or the agency s E H R patient record, as appropriate. In the event of a client s hospitalization, oversees Care Manager in coordinating discharge planning process to ensure transition to home is supported by necessary services and treatments prior to discharge. Leads and coordinates ongoing staff development trainings to enhance skills needed to effectively meet the demands of the Care Manager position. Participates in ongoing trainings to maintain high quality program standards. All other duties, as required by agency. QUALIFICATIONS & COMPETENCIES: Master s Degree in Social Work or related field with at least two years of supervisory experience.
