8 American Telemedicine Association Defines Telemedicine Telemedicine is the use of medical information exchanged from one site to another via electronic communications to improve a patient s clinical health status. Telemedicine includes a growing variety of applications and services using two way video, , smart phones, wireless tools and other forms of telecommunications technology. Telehealth is often used to encompass a broader definition of remote healthcare that does not always involve clinical services. Videoconferencing, transmission of still images, e health including patient portals, remote monitoring of vital signs, continuing medical education and nursing call centers are all considered part of telemedicine and telehealth. Key Definitions Synchronous Telemedicine Real time live video interaction between a patient and their provider. 8

9 Key Definitions Asynchronous Telemedicine The store and forward transmission of medical images and/or clinical data from one site to another. The data transfer takes place over a period of time. The transmission typically does not take place simultaneously. This is the opposite of synchronous or real time interaction. Key Definitions Tele presenter The presenter is an individual, located at the patient site that provides support to the patient and the telemedicine consulting provider, in completing the physical examination and/or telemedicine activity. 9

10 Key Definitions Originating Sites An originating site is the location of an eligible Medicare beneficiary at the time the service furnished via a telecommunication systems occurs. Medicare beneficiaries are eligible for telehealth services only if they are presented from an originating site located in: A rural Health Professional Shortage Area (HPSA) located either outside of a Metropolitan Statistical Area (MSA) or in a rural census tract; or A county outside of a MSA. As a condition of payment, you must use an interactive audio and video telecommunications system that permits real time communication between the provider, at the distant site, and the beneficiary, at the originating site. Asynchronous store and forward technology is permitted only in Federal telemedicine demonstration programs in Alaska and Hawaii. 11 Key Definitions Distant Site Practitioners Practitioners at the distant site who may furnish and receive payment for covered telehealth services (subject to State law) are: Physicians Nurse Practitioners (NPs) Physician Assistants (PAs) Nurse midwives Clinical Nurse Specialists (CNSs) Certified Registered Nurse Anesthetists Clinical Psychologists (CPs) and Clinical Social Workers (CSWs). CPs and CSWs cannot bill for psychiatric diagnostic interview examinations with medical services or medical evaluation and management services under Medicare. 12 Billing and Payment Numerous CPT (Current Procedural Terminology) and HCPCS (Healthcare Common Procedure Coding System) Codes are approved for CMS billing Codes must include a GT modifier via interactive audio and video telecommunication systems Originating Site Facility Fee Additional HCPCS Code Q3014 is a separately billable Part B payment Wisconsin Reimbursement for Telemedicine Reimbursable Telemedicine Services The following individual providers are reimbursed for selected telemedicine based services: Physicians and Physician Clinics RHCs FQHCs Physician Assistants (PAs) Nurse Practitioners (NPs) Nurse midwives Psychiatrists in Private Practice Ph.D. Psychologists in Private Practice These providers may be reimbursed, as appropriate, for the following services provided through telemedicine: Office or other outpatient services (CPT procedure codes , ) Office or other outpatient consultations (CPT codes ) Initial inpatient consultations (CPT codes ) Outpatient mental health services (CPT codes , 90862, 90875, 90876, and 90887) Health and behavior assessment/intervention (CPT codes , ) ESRD related services (CPT codes , , ) Outpatient substance abuse services (HCPCS codes H0022, H0047, T10066) Reimbursement for these services is subject to the same restrictions as face to face contacts (e.g., POS allowable providers, multiple service limitations, PA). 12

13 Telemedicine Parity Laws Licensure Current state law requires that the provider be licensed in the state where the patient is located. The Interstate Collaboration in Healthcare Stakeholder Group is aggressively pursuing reciprocity between states. 13

14 Credentialing By Proxy Telemedicine Rules Adopted for Hospitals/CAH s: New and amended rules effective July 5, 2011 permit hospitals and CAH s to provide telemedicine services to their patients through written agreements with a distant hospital or distantsite telemedicine entity. Streamlined Credentialing & Privileging for Telemedicine Physicians & Practitioners. Hospitals and CAH s may rely, when granting telemedicine privileges, upon the privileging decisions of the distant site hospital or telemedicine entity with which they have a written agreement that meets Medicare requirements *Center for Medicaid, CHIP, and Survey & Certification/Survey & Certification Group Contracting Contracts are established with independent hospitals and clinics It is best to have agreements in place when providing services outside your own system 14
