1 MSCS ADN Admissions Packet Rev Page 1 Associate Degree Nursing Program Admission Process Welcome to Minnesota State College Southeast (MSCS). We are pleased that you have indicated an interest in the Associate Degree Nursing Program. Admission to the MSC Southeast Nursing Program is a three-step process. The following checklist will assist you in this process. All items must be completed in order to be considered an applicant for admission to the nursing program. STEP ONE Admission to Minnesota State College Southeast o Students not enrolled at MSCS must apply online and submit the $20.00 application fee. o In order to be considered for the Nursing Program, you must be accepted to MSCS. o Request college transcripts from all institutions the applicant has attended (except any Minnesota State Colleges & Universities institutions attended after 1990). Official transcript(s) from any other colleges must be sent directly from the previous institution(s) to the Registrar s Office. STEP TWO - Complete the Accuplacer placement test at MSC Southeast o If the Accuplacer was taken at another location, submit a copy of the results. Test scores cannot be over 2 years (math) or 3 years (reading) to fulfill this requirement. o Ask for Branching profile 03 Health to incorporate the Computer Skills Proficiency (CSP) test when taking the Accuplacer. STEP THREE Apply to the Associate Degree Nursing Program o The deadline to be considered for admission to the nursing program: Fall Semester PRIORITY ACCEPTANCE no later than NOON on May 1 o We will continue to accept applications until June 15 or the program fills. Spring Semester PRIORITY ACCEPTANCE no later than NOON on October 1 o We will continue to accept applications until November 1 or the program fills. o Submit all of the following to the nursing department Complete this Associate Degree Nursing Applicant Documentation Form (p. 1 7) and provide all supporting documentation. It is the applicant s responsibility to ensure the accuracy of the application. Incomplete applications will NOT be considered for the program. Review and Sign the Nursing Program Essential Abilities Form (p. 4 7) Please Note: Applications will be reviewed as they are received. Applicants whose application forms are complete will receive a letter by USPS mail with the dates for the entrance exam. Applicants will take the HESI Admission Assessment (A2) Entrance Exam, a multiple choice assessment of basic academic knowledge in reading comprehension, math, vocabulary & general knowledge, grammar, biology, anatomy & physiology, and chemistry. The Elsevier HESI A2 Entrance Exam Review book is available in the College bookstore. The test fee is $ The Nursing Program Acceptance Committee will select the top applicants for the program. Applicants will be notified of their acceptance status by USPS mail approximately one week after the HESI A2 exam. It is the student s responsibility to have an accurate mailing address on file with the college.

Student Name Student Tech ID Date received in nursing department The college offers nursing programs at its campuses in Winona and Red Wing, MN. During their clinical rotations, nursing students may be required to travel up to a 100-mile radius from their home campus. Prerequisite Checklist This checklist must be complete and all documentation received in the nursing office by the DEADLINES noted on page ONE at NOON to be considered for admission. Math Score Test Date MATH Course RC Score Test Date ENGL Course Math Prerequisite Accuplacer COLLEGE LEVEL MATH score of 50 or higher. (Test scores cannot be over 2 years to use as a prerequisite) OR ACT math score of 22 or higher; OR SAT math score of 530 or higher; OR MCA math score of 1148 or higher (Test scores cannot be over 5 years to use as a prerequisite) OR Completion of MATH1025 Algebra OR MATH1020 Special Topics in Math OR MATH0990 STATWAY Statistics 1 with a grade of C or higher OR Requirement waived with an official transcript documenting a transfer course of equal or higher level. Please note: A math course is also required for the program. Higher Accuplacer scores may be required as a prerequisite to certain General Education courses. Please check course prerequisites when planning your educational path. Communications/English Prerequisite Accuplacer READING COMPREHENSION score of 78 or higher. (Test scores cannot be over 3 years to use as a prerequisite) OR ACT English score of 18 or higher; OR SAT English score of 480 or higher; OR MCA score of 1047 or higher (Test scores cannot be over 5 years to use as a prerequisite) OR Completion of ENGL0528 Reading & Writing 3 with a grade of C or higher OR Requirement waived with an official transcript documenting a transfer course of equal or higher level.

Higher Accuplacer scores may be required as a prerequisite to certain General Education courses. Please check course prerequisites when planning your educational path. Computer Proficiency Prerequisite Accuplacer CSP minimum score of 50% and must come from the identified test areas. Questions answered outside of those areas will not be included in your final score. (Test scores cannot be over 3 years to use as a prerequisite and may be taken only once) OR Completion of any 1 credit COMP course with a grade of C or higher OR Requirement waived with an official transcript documenting a transfer course of equal or higher level. Associate Degree Nursing (Generic Option) Requirement Nursing Assistant: 1. Certificate of hours from an approved course (Minimum of 75 hours) or an official transcript AND 2. Copy of current Registry card. OR Equivalent: 1. Proof of 2080 hours of CNA work experience in the last three (3) years by submitting a letter from employer verifying employment hours. Letter must be on facility letterhead and signed by Human Resource Officer or Director of Nursing AND 2. Copy of current Registry card. Associate Degree Nursing (Advanced Placement for Licensed Practical Nurses Option) (Note: This option is not available Spring Semester 2018) Advanced Placement LPN LPN License Work Hours Evidence of licensure as a Practical Nurse (LPN) AND Evidence of 1000 hours worked as an LPN within the last two years Course Requirement Note: Effective FALL Semester 2018 BIOL2515 Course If applying for admission in Fall 2018 or after: Completion of BIOL2515 Anatomy & Physiology I with a grade of C or higher.

4 MSCS ADN Admissions Packet Rev Page 4 Criminal Background Study NOTE: A Minnesota and national criminal background study does not need to be completed for the admission checklist, but will be required if you are admitted to the nursing program. If you have any concerns about potential problems with the background study, please contact the nursing department prior to the application process. Minnesota law requires that any person who provides services that involve direct contact with patients and residents at a healthcare facility licensed by the Minnesota Department of Health have a background study. An individual who is disqualified from having direct patient contact as a result of the background study and whose disqualification is not set aside by the Commissioner of Health, will not be permitted to participate in a clinical placement in a Minnesota licensed health care facility, and therefore would not be able to successfully complete the nursing program. Drug Testing Policy Be aware that if you are accepted to the nursing program, prior to or during participation in the classroom or clinical experience, you may be required to undergo drug/alcohol testing of blood or urine. You may also be subject to testing at any time throughout the program based on reasonable suspicion that you are using or are under the influence of drugs or alcohol. The MSCS Nursing Program Drug Testing policy is located on the College website for review. Communication/English and Math Requirements Applicants may have met the minimum Accuplacer cut scores for Math and Communication/English as a part of the acceptance process; however, nursing students are still required to take the Math and Communication/English courses that are identified on the Associate Degree Nursing program plan. Nursing Career Grade Requirements Courses taken at MSCS must be a grade of C or higher. Transfer grades in General Education must be a grade of C or higher. Minnesota State College Southeast Nursing Program Essential Abilities The Nursing Program Essential Abilities listing is to make students aware of the physical, cognitive, and mental activities, functions, or tasks one may encounter once employed in a nursing position. This list is adapted from the National Council of State Boards of Nursing, Inc. (NCSBN). The following standards and functional abilities are expectations of the nursing major not requirements for admission into the nursing program. Individuals applying for admission to the nursing program should review these functional standards to better understand how they may address any disability or limitation through reasonable accommodation. If you have any questions regarding these Essential Abilities, please contact the Nursing Program Dean or the Admissions Advisors. Gross Motor Skills Move within confined spaces (eg. Around patient bed) Provide standing support to patient Manipulate equipment above shoulders (eg. IVs)

Transfer client) Move light objects up to 10 pounds Move heavy objects weighing from 10 to 45 pounds Defend self against combative client Carry equipment and supplies Use upper body strength for CPR, restrain a client Squeeze (eg. Charting, reports, papers) Use computer In the Clinical Setting Students may be subjected to electrical, radiant, and chemical hazards Persons in the nursing sciences have been identified as having the likelihood of occupational exposure to blood or other potentially infectious materials and, therefore, are included in the OSHA Exposure Control Plan with its specifications to prevent contact with the above materials.
