During Out of Hours please telephone 111 for urgent care service. HOW TO REGISTER To register as a new patient with the practice you must be able to produce documented evidence of your address. Family Doctor Service Registration forms must be completed to enable us to obtain your medical records from your current GP. We also like all new patients to have a health check with the health care assistant. This helps to ensure that we understand your health needs and can provide the right care for you. You can, of course, see the doctor if you have an urgent problem. If you live outside the practice boundary we will not be able to offer home visits. HOW TO CONTACT THE DOCTOR Online Appointments If you have not yet registered: Online appointments can be booked quickly and easily wherever you are, 24 hours a day, 365 days a year. To access the appointment system via SystmOnline, you will need to fill out a Registration Request form which can be obtained from our Reception desk. Please bring with you an approved form of identification (Passport, Photocard Driving License or Birth Certificate). Our receptionists will then be able to register you and give you your login details. TO REGISTER, YOU MUST ATTEND IN PERSON AND BE AGED 16 OR OVER. Pharmacy First Many minor ailments can be dealt with by the pharmacist without any need to see the doctor or nurse. The Pharmacy First scheme can treat many things. Click on the Pharmacy First link on the practice website to find out more. Appointments Appointments are available on Monday and Tuesday from 8.30am to 7.30pm, and on Wednesday and Friday from 8.30am to 6.00pm. On a Thursday we offer appointments between 7.00am and 6.00pm. The practice closes every Thursday to 3.15pm for practice development. To make an appointment with the doctor, nurse practitioner or practice nurse you may either call in at the surgery or telephone us on

If you feel you need to be seen urgently we will try and arrange for you to be seen on the same day. When you contact us to request an appointment we will initially offer one within 48 working hours - this will be with a practitioner who has availability but might not be with the person of your choice. If you are unable to attend at the time which is offered, we will try our best to find a time for you to be seen which is convenient for you but will probably not be within 48 working hours. You can book an appointment up to three weeks in advance and in this case we can generally offer an appointment with a practitioner of your choice. Your appointment with the doctor is 10 minutes long. If you have a lot to discuss please tell the receptionist when you book your appointment and we will try to offer a longer appointment. Remember that other patients are waiting to be seen so we request that you keep within your allotted time. Telephone Advice If you need advice from the doctor or nurse you can telephone the surgery and speak to the receptionist who will arrange for the duty doctor or nurse to return your call. You will need to give us a telephone number (preferably a land line number) where you can be contacted that day, and provide the receptionist with some details of the problem. Practice Nurses' Appointments The practice nurses are available Monday to Friday from 8.30am to 6.00pm. They perform such tasks as: Taking blood tests Vaccinations Cervical smear tests Blood pressure checks Hearing tests Ear syringing Heart tracings Checking minor injuries Travel advice Slimming advice General medical advice Specialised clinics Some of the practice nurses are family planning trained and, therefore, can provide advice on contraception, including emergency contraception. To ensure you see a suitably qualified nurse you will be asked the reason for your appointment. When the Surgery is Closed For emergencies that cannot wait until the surgery is open, please telephone 111 and your call will be put through to the out-of-hours service. Please remember that this service is there only to provide urgent medical attention. If the service is abused by calling the doctor for minor illnesses then other patients could be put at risk. NHS Walk-in Centre Another option is to visit the Hillside Bridge Walk-in Centre at 4 Butler Street West in Bradford which is open seven days a week between 2.00pm and 8.00pm. Telephone Late Arrivals If you are late for your appointment, you may be asked to rearrange your appointment for another day. Cancellations If you are unable to keep your appointment, please let us know as soon as you are able so that the time can be given to another patient.

4 MISSED APPOINTMENTS POLICY Idle Medical Centre is dedicated to providing a high standard of care and services to all our patients. One extremely important factor is making sure we have enough appointments and missing an appointment without telling us is not acceptable. First Missed Appointment If you do not come to an appointment, or if you cancel your appointment at such short notice that we are unable to offer it to another patient, you may receive a warning letter. Second Missed Appointment If you miss two appointments you will receive a warning to say that if you miss any more appointments you will be taken off our medical list. Third Missed Appointment Your name will be removed from our list unless there are exceptional circumstances to be considered. The doctor would then make a decision accordingly. HOME VISITS In line with modern practice, patients should normally be seen in efficient well-lit and well-equipped surgeries. We will, however, see the terminally ill, truly housebound and severely ill in their home should the need arise. If you ask for a home visit, a doctor or nurse will probably speak to you when you telephone. If you telephone to request a home visit, it would help us very much if you could: Telephone before 11.00am if at all possible, so that we can plan our rounds. Provide the receptionist with some details of the problem. CHANGE OF ADDRESS OR TELEPHONE NUMBER Please inform us of any change in address, home telephone number or mobile phone number. We may need to contact you urgently. NON-NHS WORK AND FEES The doctors are happy to carry out non-nhs work. This could include medical examinations for insurance purposes, occupational purposes, driving medicals, etc. When you make an appointment please make it clear to the staff that the appointment is for a specialised medical examination so that the appropriate length of appointment can be made. These and other services such as private certificates, letters and insurance reports are not covered by the National Health Service and a professional charge will be made. You can enquire at the reception desk for a scale of charges. DISABILITIES If you have a disability, please inform the receptionist and she will help you accordingly. The premises has suitable access for all disabled patients. An elevator is situated at the far end of the building. If you have difficulty hearing we have a portable loop system which can be requested at the reception desk. REPEAT PRESCRIPTIONS If the doctor agrees, you may be able to obtain repeat prescriptions from reception. You can do this by: Ordering over the internet (ask the receptionist for instructions and your password). Ticking the items you require on your last prescription and posting your request along with a stamped, addressed envelope. Ticking the items you require on your last prescription and posting it in the black postbox which you will find in the lobby. All repeat prescriptions will include information as to when you need to see a doctor for a review of your medication. This is because we believe that patients on long-term medication should be seen regularly. Please ask for our separate leaflet about repeat prescriptions. Allow 48 hours to process your repeat prescription and allow another 24 hours on top of this if you have your medication delivered by the pharmacy; remember to allow extra time for postal delays, especially on public or bank holidays. We would be grateful if you could inform the reception staff if your medication has been changed following a hospital visit. ELECTRONIC PRESCRIPTION SERVICE The Electronic Prescription Service allows your prescription to be sent electronically to a pharmacy of your choice. To sign up, you need to speak to your preferred pharmacy and complete a form to confirm that they can receive your prescription directly. Once you have signed up, your chosen pharmacy will receive your prescription electronically and you will not have to pick up your paper prescription from us. This service is optional and you can continue to collect paper prescriptions as you do now if you prefer. For more information, please speak to your pharmacist or a member of reception. PRESCRIPTION COLLECTION SERVICE If you take medication on repeat prescriptions you may wish to take advantage of a free service offered by several of the local pharmacies. They will offer to collect prescriptions from the surgery and have the medication ready for collection at an agreed time at their premises. In certain special circumstances they may also be prepared to deliver, such as to the elderly, disabled and housebound. To enable this service, you will need to complete a form, which can be found within this booklet or acquired from your pharmacy, and pass it back to the pharmacy of your choice. Then, when you need repeat medication in future, you should order the medication from the surgery and the pharmacy will be able to arrange collection.

5 INVESTIGATIONS AND TEST RESULTS Results can only be given to the patient (unless we have received written consent to give the result to another person). If a sample is required by the doctor please use the container and bag provided. Make sure your name is on the container and that it reaches us before 12 noon so that it can be transported in our usual delivery van. Please allow at least five working days for the results of blood and urine. Other tests and investigations, eg X-rays and cervical smears, take longer and you will be informed of the timescale at your appointment. Please note that we are unable to give you the results of tests which were hospital-based. You would normally have to wait until you re-attend the hospital or until the hospital makes contact with you. If you would like a copy of your referral letter please let us know. PRACTICE STAFF The Practice Manager is Mr Malcolm MacDonald. The Assistant Practice Manager is Mrs Tracy Sutcliffe. The Patient Services Manager is Mrs Denise Barrett. All three can be contacted through the main surgery number and are happy to speak to patients regarding any issues they may have. Reception Staff The receptionists are here to help you, but as you will see when you visit us, their job is very demanding. They answer the telephone, deal with enquiries from all directions, and process repeat prescriptions - often all at the same time! So please be patient. Administration Staff Although you may not see much of them, there are secretaries and administrators working behind the scenes dealing with your medical records, referrals, post, etc. STAFF ATTACHED TO THE PRACTICE District Nurses The district nurses deal with all home nursing to patients referred by the doctor and other agencies. They assess nursing needs and carry out treatment in the home including blood tests. The district nurses can be contacted by telephone during the following times: Monday-Friday 8.15am pm The duty nurse can be contacted during evenings, nights, weekends and bank holidays on The district nurses have an answer machine on which you can leave a message. Health Visitors The health visitors are all specially trained nurses who can give advice on health care especially for expectant mothers and children under five. They can be contacted by telephone during the following times: Monday-Friday 8.30am pm The health visitors have an answer machine on which you can leave a message. Midwives The midwives look after you during your pregnancy and for two to three weeks after the delivery of your baby. They are happy to discuss problems or answer any questions you may have. You can contact the Wrose team of midwives on between am, any weekday or by phoning the lower ground floor reception on and speaking with the receptionist. Counsellor Referrals for counselling are made by the doctors. IDLE MEDICAL CENTRE PRACTICE CHARTER Our Charter aims to tell you about the services we provide and how to make the best use of them. You will be treated in a friendly and courteous manner by a trained member of staff. We feel it is important you know who you are speaking to, and surgery staff will wear name badges and identify themselves on the telephone. Please help by treating us as you would wish to be treated yourself. Confidentiality will be maintained at all times and your right to privacy respected. Please tell us who you want to see and how urgent you think your problem is. Let us know if more than one person in the family needs to be seen at an appointment or home visit. We run an appointment system and we will always try to give an appointment to suit your needs. Please ask if you would like a chaperone during an examination or a private room to discuss any matters. The telephone will be answered promptly and your call will be dealt with as soon as a receptionist is available. Emergencies will be dealt with immediately, but please be patient as more than one call could be coming in at the same time. All the services offered by the practice are listed in this practice booklet. To register as a new patient with the practice. You must live in the practice boundary area and be able to produce documentary evidence of this. Family Doctor Service Registration forms must be completed. This is to enable previous medical records to be retrieved. All new patients are requested to see the health care assistant for a brief health check where any special needs can be discussed. Please read this practice booklet carefully. As well as referring you on to hospital consultants, the practice has contact with other helping agencies and will be happy to refer you to them. Please ask if you have difficulties getting help with your problems. We welcome suggestions to improve our services. Complaints and compliments should be made to the patient services manager or to the doctors. Complaints will be received sympathetically and investigated promptly. We aim to provide a safe and clean environment for you. Please help us maintain this environment by putting litter in the bins. We are committed to a non-smoking policy on the premises and grounds.

6 GENERAL PRACTICE TRAINING SCHEME Idle Medical Centre is a training practice. Doctors who have acquired experience through work in hospitals and who are hoping to enter family practice, spend six months with us in order to gain experience in the sort of problems encountered in general practice. As a training practice your medical records may be used for educational purposes and because of this we will ask you to sign a consent form when you register. CHOICE Under this scheme your GP can offer you a choice for some specialists should your GP decide that you need to be referred. If you prefer, your GP can make the choice for you, but you now have the right to choose for yourself. If you do not want the GP to make the choice, you can discuss this with an advisor at the surgery. You don't have to make your mind up straightaway. You may want to talk it through with your family or friends before reaching your decision. A booklet is available from the lower ground floor reception for your information. You can also visit CLINICS We run the following clinics, most of which are by appointment. Please telephone or call at reception for further details. INR/Warfarin Clinic Weekly clinic for patients prescribed Warfarin, to check blood and advise on treatment levels accordingly. Appointments are booked through the practice nurses. Diabetic We encourage all patients who have diabetes controlled by diet or tablets to attend our diabetic clinic on a regular basis. Travel Advice and immunisation by appointment with the practice nurse. A charge will be made for certain immunisations and vaccines which are not covered by the National Health Service and a list of these charges is available from reception. Remember to contact us on at least six weeks before travelling. Asthma or COPD Morning and afternoon times are available. We encourage all patients who have, or think they may have asthma to attend our clinics. Appointments are booked by calling Spirometry appointments are available on Mondays and Wednesdays. Baby Massage Contact the health visitors on for an appointment. The health visitors have an answerphone so be prepared to leave a message. Well Baby Clinic The health visitors and GP like to see all new babies at six weeks and for regular checks thereafter. Contact the health visitors on for more details about this drop-in clinic. The postnatal check and six-week check baby examination are done by appointment within this clinic by a GP. Stop Smoking Counselling is available on request. Contact reception for more details. Antenatal These clinics are led by the Wrose team of midwives. If you become pregnant, you will be asked to book in at the lower ground floor reception. You will be seen regularly during your pregnancy, either at the surgery or at the local hospital or both. This is to monitor your progress and to discuss any problems or anxieties you may have. You can contact the Wrose team of midwives on between am any weekday, or by phoning the lower ground floor reception on and speaking with the receptionist. Minor Surgery Minor operations can be carried out in our treatment room. Please discuss this with your doctor who will then arrange for you to be put on our waiting list. Pre-conceptual Counselling An appointment with a doctor is required; you can contact reception to arrange this. Menopause Appointments to see a doctor can be booked via reception. Yellow Fever Vaccinations Idle Medical Centre is an authorised Yellow Fever Vaccination Centre. There is a charge for yellow fever vaccinations and we can offer vaccinations to people who are not registered at our surgery. Contact reception to make an appointment. Flu An influenza vaccination is particularly recommended for patients over 65 years of age or those with heart, lung and kidney disease, diabetes and residents of nursing or rest homes. An invitation to attend the practice will be sent to these patients, requesting they make an appointment to receive their vaccination. If you do not receive an invitation by the end of September and you wish to receive the vaccination, please contact the practice and arrange an appointment. If you are unable to attend the surgery, a home visit will be arranged to undertake this facility. Physiotherapy A physiotherapist is available in the practice. Your doctor will refer you for this if necessary. Telephone: The physiotherapists have an answerphone so be prepared to leave a message.

7 Chiropody Tuesday and Thursday all day and Wednesday morning. Your doctor will refer you to the chiropodist if necessary. If you phone the chiropodist be prepared to leave a message on the answerphone as they may be consulting when you call. Telephone: YOUR HEALTH We encourage all our patients to share the responsibility for their health, both by preventing disease and treating existing diseases. Many serious diseases can be prevented by a healthy way of life. You may discuss ways of promoting improvements in your general health with your doctor or with the practice nurses. Particularly important are: Blood Pressure High blood pressure increases the risk of heart attacks and strokes. Very often it can be reduced by adjustments in lifestyle alone. We advise adults under 60 years old to have their blood pressure checked every five years and patients over 60 years old to have their blood pressure checked annually. Breast Screening This is recommended for all women from the age of 50. Breast self-examination is encouraged for all ages. Cervical Smears This test is recommended for all women in the age range at three yearly intervals and women in the age range at five yearly intervals. It can detect early cancer at a curable stage, before symptoms have appeared. We have a routine invitation system. If you are over 65 you can still be screened by making an appointment with a practice nurse. A Healthy Diet You are what you eat! If your diet is not balanced, your body has ways of letting you know. For example, you may be overweight or underweight or you may suffer from constipation. A poor diet can lead to more serious problems such as heart disease and high cholesterol levels, etc. A healthy diet helps fight off disease and makes you look and feel good.the practice nurses can give you help with this. Exercise * It helps reduce weight. * It helps reduce stress. * It makes you feel and look better. Once you have found a form of exercise you enjoy, do it: * Often enough - two or three times a week for minutes. * Hard enough - to make you fairly breathless. * Long enough - it must become part of your life for good. Remember: if you are not used to exercise start slowly and build up gradually. Smoking Smoking is a major cause of cancer, bronchitis and heart disease. Babies of smokers are, on average, 200 grams smaller than those of non-smokers. Counselling is available on request. SICKNESS CERTIFICATE For your first week of sickness you do not need to see a doctor for a sick note. This can be obtained from your employer and is called an SC2. If your employer insists on a doctor s sick note for the first seven days there will be a charge. Evidence that you are sick If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay). It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a Statement of Fitness for Work (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise. You could also provide evidence from someone who is not a medical practitioner, eg a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP. Statement of Fitness for Work - Fit Note The fit note was introduced on 6th April With your employer s support, the note will help you return to work sooner by providing more information about the effects of your illness or injury. For more information go to GENERIC MEDICINES About Your Medicines Some types of drugs are manufactured by a variety of companies and the appearances can differ. They should work in the same way and there is no cause for alarm. Where Do Your Medicines Come From? New drugs are developed by drug companies who patent them and give them a special brand name. This is so that other companies cannot copy them. It also helps people remember the name of their medication. The other name for medicines which hospitals sometimes use is the generic name - this is the chemical name. One example of this is paracetamol which is commonly known by the brand name Panadol. What is a Generic Medicine? After the patent has run out for a branded medicine other companies can manufacture it under a generic name. The medicine is just as safe and effective as the original branded product but it is usually much cheaper. Using generic medicines saves the NHS millions of pounds and allows this money to be spent on you and other patients. Why Do Generic Medicines Look Different? The original colour and shape of branded medicines are sometimes included in the patent, so you may notice your generic medicines are different in colour, shape, size and also taste. This does not alter the effect of the medicine. In some cases, tablets and capsules have special coatings so that the medicine lasts longer in the body.

8 Checklist for Patients Discuss any worries about your medicines with your pharmacist or doctor. Know your medication by its generic name and strength. Take your prescription to the same pharmacy each time if possible. Generic prescribing is simple and can save the NHS money. You and other patients can benefit from this extra money and the improvements to healthcare that it can bring. COMPLAINTS We hope that most problems can be sorted out easily and quickly, preferably at the time they arise and with the person concerned. We would like you to let us know of any problems you have experienced as soon as possible - ideally, within a matter of days or at most a few weeks - because this will enable us to establish what happened more easily. Your complaint should be addressed to the patient services manager Mrs Denise Barrett, either by phone on , by letter or in person. You may wish to see our practice complaints procedure and a copy of this is available from any reception desk. Patient Advice and Liaison Service (PALS) Tel: Health Service Ombudsman for England Tel: COMPLIMENTS, COMMENTS AND SUGGESTIONS It is always good to hear when something goes particularly well. Compliments are welcome, as they enable 'best practice' to be shared and rewarded. We all prefer to hear praise rather than criticism - it can make all the difference to the day. We would also value any comments. The practice is registered under the Data Protection Act to hold computerised patient medical records. This enables all information relative to your health to be updated speedily and accurately (including letters from hospitals, etc) ensuring full information is available to the healthcare professional in the practice consulting rooms. HOW WE USE YOUR HEALTH RECORDS Everyone working for the NHS has a legal duty to keep information about you confidential. We have a duty to maintain full and accurate records of the care we provide to you and to keep records about you confidential, secure and accurate. We may share information with the following main partner organisations: strategic health authorities, NHS trusts (hospitals or primary care trusts), general practitioners (GPs), special health authorities, ambulance services. We may also share your information, with your consent and subject to strict sharing protocols about how it will be used with: social services, education services, local authorities, voluntary sector providers, the private sector. Pick up a leaflet from reception entitled How we use your health records for more information. SHARING INFORMATION This practice can share your patient s record with other clinicians. This means everyone who is caring for you can be fully informed about your medical history, including your medication and allergies. To discuss who can and can t see your record and to give consent to share your information speak to your GP today. FREEDOM OF INFORMATION - PUBLICATION SCHEME The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A copy of this is available from the patient services manager. VIOLENT AND ABUSIVE BEHAVIOUR The practice adheres to the policy for dealing with patients who show violent and/or abusive behaviour, as agreed by NHS Bradford & Airedale, to provide a safe and secure environment for patients, staff and visitors. This has been introduced in the context of the government's Zero Tolerance Initiative. PATIENT PARTICIPATION GROUP Our Patient Participation Group is designed to identify any opportunities to improve on current service delivery performance in order to build on what the group acknowledges as the strengths and weaknesses/service delivery threats of the practice. The PPG also act as a sounding board for new ideas and assist when appropriate in the introduction of new working arrangements by promoting the changes, along with providing constructive feedback on issues relating to the practice which are brought to their attention by other patients.
