2 Dear Friends, 2016 has been a year of growth and continuity at Home Care Partners. We converted the Safe at Home pilot project into a full-scale program that engages occupational therapists to develop and oversee home repair or modification projects. This innovative program addresses falls prevention, which is one of the top areas of concern identified by DC seniors. At the same time, our well-established programs and partnerships enabled us to continue to serve seniors and adults with disabilities throughout the DC area and help them remain safe and independent at home. As we enter 2017, our 60th Anniversary year, we look forward to working with the many friends and partners we have developed over these six decades to continue to serve seniors and their families in the metropolitan area. 3 Something Old, Something New Agency Mission Home Care Partners is a private, non-profit agency serving the Washington, DC metropolitan area. We enable vulnerable people to remain in their own homes in comfort, safety, and dignity. We do this by: Providing comprehensive home care services Operating a Training Institute that offers caregiver education to aides, care professionals and the general public Supporting family caregivers We are committed to excellence; we embrace diversity; and we serve those in need with priority given to those unable to pay the full cost of care. In 2016, we expanded our partnership with the DC Office on Aging to offer a new service providing home modifications to seniors and adults with disabilities through the Safe at Home program. Safe at Home helps clients reduce their risk of falling and removes barriers to mobility by installing needed equipment, such as grab bars, stair lifts, and handrails. We quickly ramped up this program beginning in January 2016, bringing on 5 new staff members headed by an occupational therapist. The program proved so popular, that the program reached capacity before the year was through. Safe at Home completed home modification projects for 193 clients and fielded over 1,600 referrals. Meanwhile, our home care aide services continued to serve seniors in DC, Montgomery, Prince George s and Arlington Counties. In 2016, we employed more than 220 home care aides who are certified as home health aides or nurse assistants. Our social worker and nurse case managers supervise the aides as well as oversee the needs of over 1,000 clients. We partner with the area agencies on aging for all four jurisdictions and the Veterans Administration and serve additional

4 clients in Montgomery County through our Light Care program and partnership with Community Ministries of Rockville. Financial Summary Our home care aides provided 234,777 hours of home care aide services to over 1,000 clients. In the U.S, family caregivers provide a huge percentage of the caregiving for their family members, valued in the billions of dollars. This is a tough job and the demands can take a toll on families. Our DC Caregiver Institute and Family Caregiver Support programs help these family caregivers by providing reimbursements for respite and health care supplies, counseling, a caregiver telephone support group, and caregiver education and resources, such as advising what types of supplies or equipment can make their tasks easier and where to find them at a reasonable cost. We also provided two home care aides to assist clients, especially those with Alzheimer s and other dementias, who need extra help while attending the Saturday Respite program at the Washington Seniors Wellness Center.
