Since 1978 HSI companies have partnered with more than 16,000 approved training centres and have authorized more than 200,000 professional safety and health educators, who have certified more than 19 million emergency care providers in the US and more than 100 countries throughout the world. HSI's vision is to be the preferred training resource for safety and health training centres. What is a Training Centre? MEDIC First Aid Training Centre is a self-governing entity that provides health and safety instruction and training. Educational services are delivered by instructors with the necessary knowledge and teaching ability to explain, tell, demonstrate, supervise, and direct learning. Instruction is imparted in diverse organizations and workplaces. What is the TCAM? The TCAM is a collection of standards and guidelines that establish the criteria for quality assurance and performance improvement of EMP Canada Training Centres, authorized Instructors and Instructor Trainers. Who is the TCAM for? The TCAM is for approved EMP Canada Training Centres and Authorized Instructors and Instructor Trainers. It is also for anyone with an interest in the quality assurance and improvement practices of EMP Canada including regulatory authorities, occupational licensing boards, national and international organizations, professional associations, educational accreditors, purchasers and end users of EMP Canada training programs and products Health and Safety Institute Version 16.0

5 Why is the TCAM necessary? The TCAM documents the process EMP Canada uses to continually improve the validity, defensibility, and effectiveness of its Training Centres, and authorized Instructors. Being able to demonstrate EMP has an effective quality assurance system in place is often required for approval or acceptance by regulatory authorities. It can also help reduce legal exposure for HSI, its EMP Canada Training Centres, and Authorized Instructors and Instructor Trainers by identifying weaknesses in operating standards, procedures, and processes. Additionally, transparency is of particular importance in matters of health, safety, and quality assurance. For this reason, EMP s Standards and Guidelines for Quality Assurance are freely available. What is Quality Assurance? Quality assurance is a set of standards and guidelines to monitor and improve the performance of EMP, its Training Centres, and Authorized Instructors and Instructor Trainers so that the products and services provided meet or exceed the requirements of regulatory authorities and other approvers. What is the Difference between a Standard and Guideline? A standard is something set up and established by an authority as a rule for the measure of quantity, weight, extent, value, or quality. A guideline is an indication or outline of policy or conduct. 1 For the purpose of these STANDARDS AND GUIDELINES FOR QUALITY ASSURANCE, a standard is something an approved Training Centre, Authorized Instructor or Instructor Trainer is required to do (mandatory), while a guideline is a statement of desired, good or best practice (non-mandatory). This version of the TCAM is the first one to separate standards from guidelines. This was done in an effort to improve brevity, clarity and ease of use. How do the Standards and Guidelines Differ between H.S.I brands? There are some inherent differences between training programs that are defined in the specific Program Standard or in the Guidelines. Otherwise, the Training Centre Standards and Guidelines apply to all training program brands. What Is the Relationship Between EMP Canada and its Training Centres? EMP designs and develops training programs and authorizes qualified individuals to teach the training programs and issue certification cards to students that have met the required knowledge and skill objectives. EMP is not a franchise. Training Centre Directors and Authorized Instructors and Instructor Trainers are not EMP employees, agents, consultants, contractors, or legal representatives. EMP does not grant exclusive or protected territories. EMP does not place a restriction on goods and services that may be offered for sale, and, excluding these STANDARDS AND GUIDELINES FOR QUALITY ASSURANCE, does not impose control over or provide significant assistance in the method of operation. EMP does not control the price or fees that Training Centres charge for courses. EMP does not share in any fees that Training Centres charge or collect from their students or customers. How often is the TCAM Revised? The TCAM is revised as necessary. Why was this TCAM Revision Necessary? Since the last TCAM revision was published in May of 2010, new CPR and first aid guidelines and new MEDIC First Aid training programs have been released. Changes in program content and standards made this revision necessary. Additionally, quality assurance is a continuous process of identifying and resolving problems, examining the solutions to those problems, and monitoring the solutions for improvement. Consequently, regular review and revision of our STANDARDS AND GUIDELINES FOR QUALITY ASSURANCE is necessary to ensure that identified problems are addressed. What If I Have More Questions? 7 Training Centre Standards Training Centre Approval Each approved Training Centre must have a Training Centre Director. The Training Centre Director is the business owner, executive officer, or other individual associated with the organization that is responsible for managing the Training Centre. The Training Centre Director is responsible for ensuring that the training activities, training department, or training organization adhere to these Training Centre standards. Terms and Conditions of Training Centre Approval 1. Training Centre Directors agree that: a) His or her signature on the Training Centre Application obligates the organization i) To comply with the terms and conditions of Training Centre Approval. ii) To comply with all applicable local, provincial, federal laws and administrative rules governing occupational licensing as they pertain to the approval, delivery, and administration of required training. b) Approval as a Training Centre is a privilege, not a right, and may be denied, suspended, or revoked at any time at the sole discretion of EMP Canada. c) Evidence of falsification of any information on the Training Centre application will result in denial or revocation of approval. d) The commission of any illegal, dishonest, unethical, or unprofessional act or the conviction of any crime that is substantially related to the duties, qualifications, or functions of the Training Centre Director may result in denial, suspension, or revocation of the Training Centre approval at any time. i) If the Training Centre Director has had a government license, permit or certification suspended, revoked, or denied or has pled nolo contendere (no contest) or been convicted of a felony in any province, prior to or during his or her EMP approval period, the Training Centre Director is required to submit a detailed explanation of the circumstances. Failure to do so is grounds for immediate revocation. ii) If the Training Centre Director has had a professional certification, trade certification, or professional designation, suspended, revoked, or denied prior to or during his or her MEDIC First Aid Training Centre approval period, the Training Centre Director is required to submit a detailed explanation of the circumstances. Failure to do so is grounds for immediate revocation. iii) Such circumstances do not absolutely preclude approval but are subject to the review and decision of the Quality Assurance Board. e) EMP has the right to communicate the current status of the Training Centre (in good standing, inactive, suspended, revoked) to law enforcement officials, regulators, occupational licensing boards, professional associations, and others as EMP Canada sees fit. f) He or she will indemnify, defend, and hold EMP Canada, MEDIC First Aid, and HSI harmless from and against all claims, losses, and causes of action, liability, damages, and expenses asserted by third parties relating to or arising from any acts or omissions of the Training Centre Director, Instructor or Instructor Trainer. g) Training Centre Approval will remain in effect until inactivated, suspended or revoked. i) The Training Centre Director may inactivate, suspend, or revoke approval at any time by giving notice to EMP Canada MEDIC First Aid. ii) EMP Canada MEDIC First Aid may inactivate, suspend, or revoke approval at any time by notifying the Training Centre Director. iii) Inactivation, suspension, or revocation will be effective at the time of notice. 2. The Training Centre Director is required to: a) Maintain Internet access and a current, valid address. b) Have at least one currently Authorized Instructor or Instructor Trainer in good standing affiliated with the Training Centre. i) The Training Centre Director may be the only affiliated Instructor, or there may be many affiliated Instructors. ii) The Training Centre Director may choose to accept or decline an Instructor s affiliation with his or her Training Centre. c) Ensure that all Instructors affiliated with the Training Centre are properly authorized, either by successful completion of an Instructor or Instructor Trainer Development Course (IDC/ITDC) or by Reciprocity. 7 Training Centre Standards 2012 Health and Safety Institute Version 16.0

8 d) Ensure that all credentials used for Instructor or Instructor Trainer Authorization are properly earned and legitimate. e) Ensure that all Instructor or Instructor Trainers affiliated with the Training Centre are properly reauthorized. f) Maintain responsibility for affiliated Instructors and Instructor Trainers g) Pay the applicable Instructor or Instructor Trainer authorization or reauthorization fee. h) Ensure the purchase of at least one (1) EMP Canada (MEDIC First Aid) training program. i) Offer training and certification only for the MEDIC First Aid program(s) purchased. j) Ensure each affiliated Instructor or Instructor Trainer: i) Provides students the most current MEDIC First Aid course materials (one per student) ii) Owns the most current MEDIC First Aid Instructor Guide for the course(s) they are authorized to teach (one per Instructor). k) Ensure affiliated Authorized Instructor or Instructor Trainer has access to, understands, and complies with these standards. l) Be responsible for MEDIC First Aid training activities at all branches (other training divisions or locations). m) Maintain a current list of affiliated Instructors and Instructor Trainers and branches with contact information. n) Monitor the performance of affiliated Authorized Instructors and Instructor Trainers to ensure he or she; i) Is proficient, up to date, and teaching the most current version of the MEDIC First Aid training program. (1) Instructors and Instructor Trainers must complete an update course based on the 2010 CPR and first aid science, treatment, recommendations, and guidelines by December 31, (2) Instructors and Instructor Trainers who have not completed an update course by December 31, 2011 will have their status suspended no later than January 1st, ii) Issues 2010 MEDIC First Aid certification cards to individuals who have successfully completed a MEDIC First Aid course based on the 2010 CPR and first aid science, treatment recommendations, and guidelines. iii) Maintains good personal hygiene and an appearance that is both professional and suitable to the setting of the class. iv) Creates a learning environment free from discrimination, harassment, prejudice, and culturally sensitive. v) Ensures adequate precautions to prevent injury and minimize the risk of disease transmission. vi) Uses appropriate equipment in sufficient quantities to allow adequate hands-on practice of required psychomotor skills by each student. vii) Takes reasonable precautions to ensure the security of certification cards and the integrity of written exams (i.e., protected against theft and duplication). viii) Refrains from engaging in or permitting affiliated Instructors or Instructor Trainers to engage in illegal, dishonest, unethical, or unprofessional conduct including, but not limited to; (1) Issuing unearned or improperly altered certification cards. (2) Withholding properly earned certification cards. (3) Fraud, forgery, misrepresentation, false advertisement, discrimination, unauthorized duplication of copyrighted materials, financial or other business-related misconduct. ix) Promptly responds to complaints from EMP Canada, regulatory authorities, students, customers, and others. x) Works cooperatively with regulatory authorities, law enforcement officials, students, customers, affiliated Instructors, and EMP to investigate and correct any situation that endangers the goodwill associated with HSI, its brands, MEDIC First Aid training programs, approved Training Centres, or authorized Instructors. xi) Provides reasonable accommodation to all persons with disabilities who seek access to MEDIC First Aid programs consistent with the current Disabilities Act (ADA). 8 Training Centre Standards 2012 Health and Safety Institute Version 16.0

9 Record Maintenance 1. The Training Centre Director is required to: a) Keep clear, legible, and orderly records (paper or digital) for no less than three (3) years, including: i) Participant-level (student) rosters ii) Participant -level class evaluations (or summaries of class evaluations) iii) Participant -level skill performance evaluations (where required) iv) Participant -level written exam results (where required) v) Verification of Medical Oversight Form (1) Only applies to Training Centres who are certifying public safety personnel. vi) A copy of the Instructor or Instructor Trainer Application (originals sent to EMP Canada) and all associated credentials for the length of the affiliation with the Training Centre. (1) If the Instructor or Instructor Trainer attained authorization through reciprocity, the Training Centre must retain a copy of the original authorization that was used (for example, his or her Canadian Heart and Stroke Foundation of Canada Basic Life Support Instructor card.) (2) If the Instructor or Instructor Trainer attained authorization through an Instructor or Instructor Trainer Development Course (IDC/ITDC), the Training Centre is required to retain a copy of the course completion documentation and send the originals to EMP Canada including: (a) Completed IDC course roster. (b) Record of IDC exam scores. (c) Instructor Candidate performance evaluation skill sheets. b) Any and all documentation related to formal complaints and quality assurance reviews. c) Any and all documentation related to formal complaints and quality assurance reviews. In the event of an accident or incident training documents may be pertinent to an investigation. As such they may be requested for up to two years after the incident took places, which may be beyond the expiry date of the certification card. Some Provincial regulations require documents that are pertinent to an investigation to be retained for up to 5 years. Training Centres are restricted by Canadian Laws that limit the use or disclosure of information for purposes other than those for which it was collected, except with consent or as required by the law. Information that is collected must be protected by safeguards appropriate to the sensitivity of the information. This would require classroom training documents to be kept in a file cabinet that has the capability of being locked. Once a document has served its purpose, this same documents must be disposed of in an appropriate and legal manner. 2. Training Centres may use a digital or print roster of their own making; however, these rosters must have the same data fields (i.e. name, class type, etc.) and the same attesting statement that appears on the original (MEDIC First Aid) roster. 3. Training Centres have an option to transfer documents digitally to EMP Canada Registry. The Training Centre must sign this agreement. (Digital Document Transfer and Records Storage Training Centre Agreement) EMP Canada will grant the Training Centre approval and sign the agreement before any documents are transferred digitally to EMP Canada. This agreement is a revocable privilege for quality control. This agreement may be terminated by either party at any time without prior notice. Upon termination in writing, Training Centres must immediately cease the digital transfer of course documentation with the EMP Canada Registry. 9 Training Centre Standards 2012 Health and Safety Institute Version 16.0

10 Quality Assurance Reviews (QAR) A QAR is an evaluation of Training Centre records and program delivery activities to ensure consistency with these STANDARDS AND GUIDELINES FOR QUALITY ASSURANCE. The purpose of a QAR is to continually improve the quality and defensibility of HSI; EMP Canada it s approved Training Centres, Authorized Instructors and Instructor Trainers. The QAR is not designed to be punitive. If weaknesses are found, EMP Canada will work with the Training Centre Director in a helpful and reasonable manner to make the necessary corrections. 1. The Training Centre Director is required to: a) Promptly respond to a Quality Assurance Review request. i) Failure to respond may result in the suspension or revocation of Training Centre approval. b) A QAR request requires that the Training Centre submit documentation including, but not necessarily limited to rosters from the last three (3) classes conducted and those documents identified in the Record Maintenance section 1 & 2 above. c) A Training Centre may be selected for a QAR only once per calendar year, unless the Training Centre Director is the subject of a formal complaint. Delinquent Invoices 1. The Training Centre Director is required to: a) Work with EMP to develop and nurture a strong, mutually beneficial business relationship built upon honest and respectful transactions; commercial and otherwise. b) On receipt of notice that a Training Centre has a delinquent invoice; i) Over 60 days, HSI will file a complaint against the Training Centre Director seeking immediate payment. ii) Over 90 days, HSI will suspend the Training Centre approval. iii) Over 150 days, HSI will revoke the Training Centre approval. 2. Any and all current and active affiliated Authorized Instructor or Instructor Trainers will be notified of the revocation and encouraged to affiliate with another Training Centre or start their own. a) Training Centre revocation will not reflect negatively on any affiliated Instructor or Instructor Trainer unassociated with the financial misconduct. Instructor and Instructor Trainer Authorization Authorization grants permission to teach a MEDIC First Aid training program to and issue certification cards to persons who successfully complete the course. 11 Terms and Conditions for New Instructor or Instructor Trainer Authorization Authorized Instructors and Instructor Trainers Agree that: 1. New Instructor or Instructor Trainer Authorization may only be obtained by meeting the requirements for successful completion of an Instructor or Instructor Trainer Development Course (IDC/ITDC) or by Reciprocity. 2. His or her signature on the Instructor or Instructor Trainer Application obligates the applicant to: a) comply with all applicable local, provincial, federal laws and administrative rules governing occupational licensing as they pertain to the approval, delivery, and administration of required training, whether inside or outside North America. b) comply with the terms and conditions of Instructor or Instructor Trainer Authorization. 3. Authorization is a privilege, not a right, and may be denied, suspended, or revoked at any time at the sole discretion of EMP Canada. 4. All credentials used for authorization are properly earned and legitimate (licenses, certifications, or registrations) and that evidence of falsification of any information on the application will result in denial or revocation of authorization. 5. The commission of any illegal, dishonest, unethical, or unprofessional act or the conviction of any crime that is substantially related to the duties, qualifications, or functions of a MEDIC First Aid Authorized Instructor or Instructor Trainer may result in denial, suspension, or revocation of the Instructor or Instructor Trainer Authorization at any time. a) If the Instructor or Instructor Trainer has had a government license, permit or certification suspended, revoked, or denied or has pled nolo contendere (no contest) or been convicted of a felony in any state, prior to or during his or her authorization period, he or she is required to submit a detailed explanation of the circumstances. Failure to do so is grounds for immediate revocation. c) Such circumstances do not absolutely preclude approval but are subject to the review and decision of the EMP Quality Assurance Board. 6. EMP has the right to communicate the current status of the Authorized Instructor or Instructor Trainer (in good standing, inactive, suspended, and revoked) to law enforcement officials, regulators, occupational licensing boards, professional associations, and others as EMP Canada sees fit. 7. His or her signature on the Application indemnifies, defends, and holds MEDIC First Aid, EMP and HSI harmless from and against all claims, losses, and causes of action, liability, damages, and expenses asserted by third parties relating to or arising from any acts or omissions of the Authorized Instructor or Instructor Trainer. 8. Authorized Instructors and Instructor Trainers are required to: a) Maintain affiliation with one primary Training Centre. i) Authorized Instructors and Instructor Trainers may teach for as many Training Centres as they wish, but they must be affiliated with only one primary Training Centre. ii) Instructors may establish their own Training Centres or may affiliate with one that is already established. iii) Authorized Instructors and Instructor Trainers are free to change their Training Centre affiliation at any time. b) Be proficient, up to date, and teach according to the program standard of the most current version of the MEDIC First Aid training program used. i) Instructors and Instructor Trainers must have completed an update course based on the 2010 CPR and emergency care science, treatment, recommendations, and guidelines prior to issuing 2010 certification cards. c) Have good personal hygiene and an appearance that is both professional and suitable to the setting of the class. d) Provide access to the most current MEDIC First Aid training materials to students for use during and after the course. e) Assure the learning environment is free from discrimination, harassment, prejudice, and culturally sensitive. f) Take adequate precautions to prevent injury and minimize the risk of disease transmission in the classroom. g) Have appropriate equipment available in sufficient quantities to allow adequate hands-on practice of required psychomotor skills by each student. h) Refrain from engaging in illegal, dishonest, unethical, or unprofessional conduct including, but not limited to: i) Issuing unearned or improperly altered certification cards. ii) Withholding properly earned certification cards. iii) For Instructor Trainers; withholding properly earned instructor authorization cards or failing to promptly submit Instructor Applications without good or sufficient reason. 11 Training Centre Standards 2012 Health and Safety Institute Version 16.0

12 iv) Fraud, forgery, misrepresentation, false advertisement, discrimination, unauthorized duplication of copyrighted materials, financial or other business-related misconduct. i) Promptly respond to complaints from EMP, Training Centre Directors, regulatory authorities, students, customers, and others. j) Work cooperatively with the Training Centre Director, regulatory authorities, law enforcement officials, students, customers, EMP Canada and HSI to investigate and correct any situation that endangers the goodwill associated with HSI, its brands, EMP Canada, MEDIC First Aid training programs, approved Training Centres, and Authorized Instructors and Instructor Trainers. k) Provide reasonable accommodation to all persons with disabilities who seek access to MEDIC First Aid training. l) Comply with all applicable local, provincial, and federal laws and administrative rules governing occupational licensing as they pertain to the approval, delivery, and administration of required training, both inside and outside North America. 9. Authorized Instructors and Instructor Trainers who are currently teaching are NOT required to: a) Maintain participant (student) level certification. i) If an Authorized Instructor or Instructor Trainer must show evidence of participant-level certification in order to meet occupational licensing or other organizational requirements, then he or she must meet the required knowledge and skill objectives for successful completion of the student level course. ii) As an Instructor or Instructor Trainer cannot appropriately evaluate his or her own skills and knowledge, the evaluation must be conducted by a different Authorized Instructor or Instructor Trainer and be properly documented (performance evaluation, written exam where required, participant-level certification card, and roster). Terms and Conditions for Instructor or Instructor Trainer Reauthorization Instructor or Instructor Trainers Agree that: 1. Reauthorization obliges the individual to the terms and conditions of authorization. 2. Instructor Reauthorization requires; a) The Authorized Instructor to have taught (or co-taught) at least one (1) class (MEDIC First Aid) during the previous authorization period (one year). Occupational requirements may vary dependent on provincial regulations. 3. Instructor Trainer Reauthorization requires; a) The Authorized Instructor Trainer to have taught (or co-taught) at least one (1) Instructor Development Course (MEDIC First Aid) during the authorization period. 4. Instructors and Instructor Trainers are required to document the MEDIC First Aid classes taught during their authorization period and provide this evidence of teaching to the Training Centre Director for the purpose of reauthorization. a) Evidence means at least one (1) class roster or student record (MEDIC First Aid). b) The Training Centre Director is required to produce this evidence when requested by EMP Canada. Expired Instructor or Instructor Trainer Authorization After three years, Instructor and Instructor Trainer Authorization is no longer valid. 1. An Instructor or Instructor Trainer who has met the minimum teaching requirements for reauthorization during their authorization period is allowed a maximum grace period of 90 days past the expiration date of their authorization to reauthorize. This grace period does not extend the Instructor authorization period. 2. An Instructor or Instructor Trainer who has not met the minimum teaching requirements for reauthorization during their authorization period, or whose expired authorization is past the maximum grace period of 90 days must meet the Terms and Conditions for New Instructor or Instructor Trainer Authorization. a. This requirement may be waived under special circumstances, including but not limited to; active military service and illness or injury. For more information, please contact the Quality Assurance department at Master Instructor Trainer Appointment Master Instructor Trainers are training experts that have been appointed by EMP. They are authorized to conduct all levels of MEDIC programs within their scope of expertise or healthcare practice, including the development of Instructors and Instructor Trainers. They may be assigned by EMP to conduct quality assurance activities such as course monitoring. 13 Certification The term certification means verification that on the indicated class completion date a participant demonstrated achievement of the required knowledge and hands-on skill performance objectives to the satisfaction of a qualified and authorized EMP MEDIC First Aid Instructor or Instructor Trainer. Certification does not guarantee future performance, or imply licensure or credentialing. Certification is documented by the legitimate issuance of a correctly completed EMP MEDIC First Aid certification card. Requirements for Legitimate Issuance The Training Centre Director and its Authorized Instructors and Instructor Trainers are required to: 1. Legitimately issue proper EMP MEDIC First Aid certification cards. a) Legitimately issue means that the proper EMP MEDIC First Aid certification card bearing the EMP Canada MEDIC First Aid name and logo designed, produced, and sold by EMP is awarded only to an individual who actually attended a MEDIC First Aid program taught by a properly authorized EMP MEDIC First Aid Instructor or Instructor Trainer who evaluated the individual s knowledge and skill competency and determined them acceptable in accordance with the programs standard. b) Documentation of skill competency as designed and outlined in the applicable EMP MEDIC First Aid Instructor Guide is the primary evidence of legitimate issuance of certification. i) 2012 EMP MEDIC First Aid certification cards may only be legitimately issued to individuals who have successfully completed a MEDIC First Aid course based on the 2010 CPR and emergency care science, treatment, recommendations, and guidelines. c) EMP Canada cannot legitimately issue a MEDIC First Aid certification card to a person who has completed MEDIC First Aid courses conducted by an independent Training Centre. i) Legitimate issuance of certification cards is the role of the authorized Instructor, not EMP Canada. Instructors bear the responsibility of ensuring and documenting that each student has met the required knowledge and skill competency objectives for certification. d) The responsibility to legitimately issue MEDIC First Aid certification cards may not be assigned or transferred to anyone other than the or authorized Instructor(s). e) Once a MEDIC First Aid certification card has been rightfully earned by a student, it remains valid until the stated expiration date. f) Training Centre Directors who charge fees and collect payment for courses, fail to issue properly earned MEDIC First Aid certification cards, and fail to respond promptly to a formal complaint, will be suspended or revoked. i) Customers affected by the Training Centre actions will be directed to seek remedy by reporting the incident to the economic crimes unit of their local law enforcement agency. 2. Promptly issue MEDIC First Aid certification cards that are legible (typed, computer-generated or handwritten). a) Participant names must be correctly spelled. b) The Authorized Instructor, Registry Number, Class Completion Date, Expiration Date, Training Centre Phone No., Training Centre I.D. and the Training Centre name, must be filled in on each card. c) In circumstances where issuance of certification cards is contingent upon a contractual agreement between the Training Centre and its customer, the Recognition of Participation may be issued to students until certification cards are issued according to the terms of the agreement with the customer. d) In circumstances where the Training Centre has an employee relationship with students who have been issued certification cards, the Training Centre has the right to retain the certification card according to employment policies and practice. 3. Issue a MEDIC First Aid certification card bearing the EMP MEDIC First Aid name and logo that was designed, produced, and sold by EMP Canada for all courses advertised as a MEDIC First Aid course. a) The Training Centre Director and its Authorized Instructors and Instructor Trainers may not issue certification cards or other certificates in any form produced by the Training Centre as a substitute for a MEDIC First Aid certification card in courses that are advertised as a MEDIC First Aid course. b) The Training Centre Director and its Authorized Instructors and Instructor Trainers may purchase and use MEDIC First Aid training materials to teach courses and issue certification cards or other certificates in any form produced by the Training Centre in courses that are not advertised as a MEDIC First Aid course. i) In this case, EMI Canada does not; 13 Training Centre Standards 2012 Health and Safety Institute Version 16.0

14 (1) Permit EMP Canada or MEDIC First Aid names, logos, or trademarks to appear anywhere on advertisements, course completion documents, print or electronic course rosters, certification cards or wall-type certificates produced by the Training Centre and/or its authorized Instructors. (2) Recognize the instructor, course, certification cards or other certificates. (3) Accept responsibility or liability for the quality of Instructor or Instructor Trainer or the fitness, approval, recognition, instructional content, quality, or outcome of the course. Formal Complaints A formal complaint is a written allegation that a Training Centre Director or an Authorized Instructor or Instructor Trainer has committed a dishonest, unethical, unprofessional or illegal act. Anyone may submit a formal written complaint against an EMP Canada approved Training Centre Director or Authorized Instructor, including EMP Canada. EMP Canada will not act on anonymous or verbal complaints. EMP Canada is not a regulatory authority or law enforcement agency. Formal Complaint Process 1. The complaint process is intended to document and resolve complaints in reasonable and consistent manner. 2. EMP Canada will keep the complainant s identity confidential throughout its inquiry. a) Some complaints, because of the specific nature of the events involved, are difficult or impossible to keep confidential. b) Those with a legitimate complaint should not expect any retaliatory action. c) If reasonable evidence exists to suggest that a Training Centre Director or Authorized Instructor or Instructor Trainer has retaliated against another approved Training Centre Director or Authorized Instructor or Instructor Trainer for filing a complaint, EMP Canada may opt, among other actions, to revoke the approval or authorization of the person(s) retaliating. d) Complaints that are discovered by EMP to have been falsely made for malicious, competitive, or political reasons are grounds for revocation. 3. To substantiate allegations of very serious quality assurance concerns, EMP Canada reserves the right to anonymously attend classes conducted by MEDIC First Aid Authorized Instructors and Instructor Trainers affiliated with an approved Training Centre. 4. Upon receipt of a written complaint, EMP will initiate its complaint process within 30 days. a) EMP will notify the Training Centre Director and Authorized Instructor or Instructor Trainer and request a thorough and explicit response within a reasonable time period. b) The Training Centre Director and/or authorized Instructor(s) are required to promptly respond within the stated time period. Failure to respond will result in the suspension or revocation of Training Centre approval and/or Instructor or Instructor Trainer authorization. c) After notifying the Training Centre Director and/or authorized Instructor(s) of the complaint and throughout the inquiry, EMP may; i) Request additional Training Centre documentation as necessary. ii) Restrict or prevent the sale of certification cards to the Training Centre or its affiliated Instructors. 5. Following review of the response, EMP will consider and carefully balance the response against the complaint. At the conclusion of the inquiry, EMP may: a) Dismiss the complaint without further action. b) Require the Training Centre Director and/or Authorized Instructor or Instructor Trainer to execute a legally binding Compliance Agreement in order to maintain approval and/or authorization. c) Suspend or revoke the Training Centre approval and/or Instructor or Instructor Trainer authorization. 6. EMP will follow up with the complainant and respondent in a timely and professional manner regarding its decision. a) Upon suspension or revocation the Training Centre must stop conducting all EMP Canada MEDIC First Aid training and may no longer represent that it is authorized to provide MEDIC First Aid courses or training. b) The Training Centre must stop using, in any other manner whatsoever, the marks, symbols, and other identifying characteristics of MEDIC First Aid. Appeal 1. A Training Centre Director and/or authorized Instructor(s) that has been suspended or revoked may submit a persuasive and earnest request for reinstatement to EMP s Quality Assurance Board (QAB). After reviewing the appeal, the QAB may: a) Reinstate the approval or authorization without further action, 14 Training Centre Standards 2012 Health and Safety Institute Version 16.0

15 b) Reinstate the approval or authorization and require the Training Centre Director and/or Authorized Instructor or Instructor Trainer to execute a legally binding Compliance Agreement. c) Deny the appeal. i) When the appeal is denied, EMP will not consider restoration of approval or authorization for a period of not less than 5 years. 2. EMP will communicate its decision to the individual or entity making the appeal within 30 days. Quality Assurance outside North America H.S.I Training Centre approval, Instructor or Instructor Trainer authorization, and quality assurance oversight outside North America is provided by HSI. Training Centre Directors and Authorized Instructors and Instructor Trainers outside North America agree to comply with the Terms and Conditions of Training Centre Approval and Instructor or Instructor Trainer Authorization, as described in these STANDARDS AND GUIDELINES FOR QUALITY ASSURANCE. This includes the requirement to comply with all applicable local, state, provincial, national, or federal laws and administrative rules as they pertain to the approval, delivery, and administration of required training. Training Centres and Authorized Instructors and Instructor Trainers interested in offering MEDIC Programs outside North America should complete a contact request or call MEDIC First Aid International Training Centre approval, Instructor or Instructor Trainer authorization, and quality assurance oversight in Canada is provided by HSI and EMP Canada (a member of the HSI family of brands). Training Center approval, Instructor or Instructor Trainer authorization, and quality assurance oversight in Greece, Japan, and New Zealand is provided by MEDIC First Aid International Regional Offices in those countries. Persons offering or teaching MEDIC First Aid in these countries must comply with the terms and conditions of approval and authorization as required by the International Regional Office. This includes, but is not limited to, all applicable local, state, provincial, and federal laws and administrative rules governing occupational licensing as they pertain to the approval, delivery, and administration of required training. Persons interested in teaching MEDIC First Aid in Greece, Japan, and New Zealand should contact the International Regional Office. For other countries, please complete a complete a contact request or call International Disclaimer Country-specific laws, regulations, treaties, and customs concerning emergency care training and occupational licensing requirements are numerous, complex, and vary from country to country. They may prescribe detailed practices, rules, standards, and other conditions. Training Centre Directors and Authorized Instructors and Instructor Trainers must know, operate, and teach within these requirements. HSI does not proactively seek approval of its training program brands for use outside of North America. Training Centres interested in pursuing approval of a MEDIC First Aid program in a specific country or region of a country, should first explore the possibility with the MEDIC First Aid International Regional Office or the governmental body in charge of qualification or approval in the state. HSI will provide assistance as it is requested and practical. HSI will make reasonable exceptions and accommodations to its STANDARDS AND GUIDELINES FOR QUALITY ASSURANCE when necessary to comply with country-specific laws, regulations, treaties, customs, or operational realities. Training Centre Logo Only approved Training Centres may use the EMP Canada or MEDIC First Aid Training Centre logos to advertise in print and on the Internet according to the following requirements: 1. Only currently-approved EMP Canada MEDIC First Aid Training Centres may use the logo. 2. The Training Centre shall not contest ownership of any copyrights, trademarks (Marks), or other intellectual property rights involving EMP Canada or MEDIC First Aid Training programs, certificates, materials, and products. 5. The Training Centre shall not place the EMP Canada or MEDIC First Aid Training Centre logo on any certificate, course, program, training materials, or products in any format, invented, created, or produced by the Training Centres or others so as to give the impression that the certificate, course, program, materials, or products were created, endorsed, recommended, approved, or sold by the Health & Safety Institute or its affiliates. 6. The Training Centre shall not use marks, words, or symbols confusingly similar to Marks in connection with any Training Centre trade name, corporate name, or business name, nor as a trademark or service mark nor shall the Training Centre display the MEDIC First Aid Training Centre logo in a more prominent position than its own logo or name. 7. No Mark shall be combined with any trademark or service mark or any other words or symbols to form, in effect, a new trademark or service mark, or to imply that the Marks are owned by anyone other than Health & Safety Institute or its affiliates. 8. Training Centre shall not transfer or assign these limited rights to use the Training Centre logo to a third party. 9. HSI continues to revise and release MEDIC First Aid training programs to meet the 2010 Guidelines. Consequently, this version of the TCAM contains MEDIC First Aid program standards that reflect both 2010 and previous guidelines. TCAM program standards that reflect previous guidelines will be updated as G2010 MEDIC First Aid programs are released. 23 Personnel Equipment Documentation At least one remote, current, and properly authorized MEDIC First Aid Instructor in good standing. A good quality webcam. A headset with a microphone for the best audio quality. A computer that meets the system requirements (above). As required by the Program Standards of the course for skill assessment, including performance evaluation checklists and student course rosters. Recording and saving remote skill assessment is highly recommended. At least one individual with a current and valid provider (student level) certification in the course for which the individual is seeking re-training and re-certification. As required in the MEDIC First Aid Program, including appropriate resuscitation and CPR mannequin, ventilation device(s), dressings and bandages, etc. In the absence of a full body mannequin, at least one other person who can play the role of an injured or ill person for required performance evaluations. 24 Training Centre Guidelines Training Centre Guidelines are non-mandatory statements of desired, good, or best practice and other related quality assurance information Attributes of a Proficient Instructor These attributes were compiled by a group of professional health and safety instructors. They are intended to aid in determining the qualities of a skilled instructor. 1. Teaches often 2. Has good people skills 3. Manages time effectively 4. Objectively evaluates student 5. Properly documents the course 6. Has adequate subject knowledge 7. Understands adult learning styles 8. Intelligent, ethical, and dependable 9. Has an appropriate, professional appearance 10. Has demonstrated competency in essential skills 11. Can teach with or without audiovisual presentations 12. Motivated, well-prepared, self-confident, and patient 13. Can effectively use problem solving scenarios as teaching tools Research has shown that when teaching resuscitation to laypersons, a health care provider is no more effective than a trained lay person. 2,3 However, health care professionals generally prefer instructors who are not only trained and enthusiastic, but who also have knowledge and experience providing medical care (clinically competent). 4 Even then, the teachers' attitude toward students, rather than their professional abilities, has been reported as being the crucial difference between effective and ineffective teachers. 5 To help motivate the learner and establish credibility, training of health care professionals and professional rescuers with a duty to respond should be conducted by instructors with experience in the learners occupation. Programs should be tailored to the learners occupational setting, role and background. 6 Substantial hands-on practice is needed to meet psychomotor skill performance objectives. 7 Guidelines for New Instructor or Instructor Trainer Authorization MEDIC First Aid Instructors and Instructor Trainers are authorized by two methods; successful completion of an Instructor or Instructor Trainer Development Course (IDC/ITDC) or Reciprocity. Authorization via IDC or ITDC The Instructor or Instructor Trainer candidate attends a MEDIC First Aid IDC/ITDC (classroom, blended, or apprenticeship) and meets the requirements for Instructor or Instructor Trainer Authorization as described in the IDC/ITDC program standard. Authorization via Reciprocity HSI and EMP Canada defines reciprocity as the acceptance of current credentials from another nationally recognized organization or institution as the basis for authorization as a MEDIC First Aid Instructor or Instructor Trainer. Teaching Credentials HSI and EMP Canada recognizes the following teaching credentials as equivalent or exceeding its MEDIC First Aid Instructor Development Courses; 1. AAP Instructor 2. Instructor Status Authorization via Crossover from another Agency Individuals that have a current First Aid Instructor Status with another recognized national First Aid agency (Red Cross, St. John Ambulance etc.) may also choose to deliver EMP Canada equivalent First Aid programs. In order for the qualified Instructor to crossover to deliver equivalent EMP Canada programs they will need to provide the EMP Registry with proof of their current Instructor status, purchase the applicable instructional materials (Instructor Guide, program DVD, and Student Guide), review the instructional materials and participate in the applicable Instructor Development process. Instructor Development is offered to the qualified Emergency First Aid, Standard First Aid and Advanced First Aid crossover Instructor using a remote verification process (videoconferencing) conducted by an EMP Instructor Trainer. The crossover candidate then has a peer Instructor (qualified with EMP Canada) monitor the delivery of their first program for quality 25 Training Centre Guidelines 2012 Health and Safety Institute Version 16.0

26 assurance. The successful candidate submits the appropriate Instructor Application, Membership Agreements and monitoring form that the peer instructor completes to EMP Canada. When EMP receives the documentation, the Registry will process the application to indicate the Active Teaching status of the Instructor. Establishing Reciprocity Reciprocal acceptance by EMP Canada is not guaranteed, see Instructor crossover standards. One form of reciprocity is where an equivalent authorization is given in return for the one presented. For example, an individual who is a current and valid Canadian Heart and Stroke Foundation of Canada Basic Life Support Instructor is given equivalent authorization as an EMP MFA Basic Life Support Instructor. This is a straightforward condition of reciprocity in kind (Fig.1). Another form of reciprocity is based on the combination of participant (student) level credentials and teaching credentials (Fig 2). The combination can be used to determine what MEDIC First Aid program(s) the individual is qualified to teach. For example, to receive a Certified Law Enforcement Instructor credential, a person must demonstrate his or her ability to teach. To receive a current and valid Basic Life Support certification at the professional provider level, a person must demonstrate his or her ability to perform basic life support skills. Therefore, a person who is both a Certified Law Enforcement Instructor and certified Basic Life Support provider has demonstrated both participant and instructor level knowledge and skills. He or she would qualify for authorization as an EMP MFA Basic Life Support Instructor. Here is another example of reciprocal instructor authorization based on a combination of participant level credentials and teaching credentials; a St. John Ambulance-authorized Trainer who is also certified in adult and pediatric Emergency and Standard first aid, CPR, and AED. This individual could receive reciprocity as a MEDIC First Aid Emergency and Standard First Aid, CPR, AED Instructor. Here is an another example; a certified EMS and Paramedic Instructor could receive reciprocity that would enable him or her to teach nearly all MEDIC First Aid programs. There are many potential combinations that could be used.
