Review/Action - Approval of Minutes of the Board Meeting held September 15, (Holli Witherington, Chair). IV. Discussion and possible action on Disciplinary Matters. The Board may conduct an evidentiary hearing, consider a proposed consent order, or consider other associated matters or motions. (Holli Witherington, Sandra Balzer, Ric Pierson). A. Femi Somade, LADC #387. Review of Compliance with Consent Order of November 15, The Board may convene an Executive Session pursuant to 25 O.S. 307 (B) (8). B. Callie Hathcoat, LADC #949. Complaint # , Revocation of ADSAC Assessor Certification. Proposed Consent Order. The Board may convene an Executive Session pursuant to 25 O.S. 307 (B) (8). C. Dandra Rentie, LADC #603. Request to clarify language of Consent Order of July, 2012, Number 2 (b) and (c) (iii) and Compliance Review of Consent Order of July The Board may convene an Executive Session pursuant to 25 O.S. 307 (B) (8). V. Discussion and possible action on Candidates for Licensure. The Board may suspend or deny the application, or take other action. A. Joseph Barchue, LADC candidate. Review of compliance with Voluntary Agreement of May 12, 2014 for Additional Licensure Requirements B. Jennifer Floyd, LADC candidate. Complaint # Arrest incident report, providing services while under the influence, billing for services not performed C. LaTosha Hickman, CADC candidate. Complaint # Improper dual relationship. D. Rebecca Hammerschmidt, CADC candidate. Application voided second time for failure to test within one year. 1

Request for extension was tabled in September 15, 2014 meeting until today. 2. Kimberly Thomas, LADC candidate. Failure to test within one year. 3. Meredith McGrath, LADC candidate. Failure to test within one year. 4. Shelly McCarty-Noyes, LADC candidate. Personal health and family issues. 5. Jacinta Dike, LADC candidate. Petitioned for an extension in July, tabled until November. B. Discussion and possible action on whether Drug of Hate CEU counts as alcohol/drug specific. C. Discussion and possible action regarding approval of Clifton D. Thomas, Ph.D., LPC, LMFT to supervisor for LADC/MH candidate. D. Kandis Griffin, LADC candidate. Applied June, Inactive for 2 years (10/31/2012). Wants to go under supervision again. E. Gina Redwine, LADC candidate. Inactive for 3 ½ years (03/07/2011), wants to go back under supervision. F. Deborah Fallis, LADC candidate. Originally applied for LADC on 12/29/2008. Reapplied on 09/29/14 and is requesting past requirements that were previously completed be accepted. She was in the testing process. G. Elizabeth Norton, CADC candidate. Requesting she be allowed to practice as LADC candidate while she completes Master s degree coursework. H. Lorena Alvarez Hull, LADC candidate. Inactive since 2012, want to resume supervision. I. Lindy Kroenert, LADC #81. Offering to surrender license following non-compliance with CEU audit. J. Discussion and possible action on supervisors petitioning to supervise more than 20 candidates. 1. Ronald Snoddy, LADC #213. Currently has 31 supervisees. 2. Franklin Marlin, LADC #534. Currently has 26 supervisees. K. Discussion and consideration of materials and testing for supervision trainings and discussion with ODAPCA. L. Discussion and possible action on Oklahoma Ethics Commission rule regarding lobbying activities. M. Discussion and possible action on recommendation to get an Attestation of Sobriety form at time of application. 2

Discussion and possible action on joint letter with ODMHSAS regarding LADC scope of practice and billing issues. P.
