JOB PURPOSE This is a new and evolving service, within the Rehabilitation Service the post holder delivers a high quality specialist service, providing expert clinical intervention to patients and education and support to their carers. The post holder provides specialist patient assessment, case coordination, treatment and planned discharge for adults with complex Rehabilitation needs. The post holder will work autonomously in a diversity of hospital and community based settings including a case coordinating and Health Promotion role. The post holder role will include physiotherapy evaluation, audit projects, staff / student supervision / induction and providing expert advice on physiotherapy interventions. To manage a complex clinical caseload within the service, applying specialist knowledge and skills relating to physiotherapy working as an effective and efficient partner in the interdisciplinary team To provide direct clinical care which encompasses physiotherapy assessment, treatment and management of patients referred to the service. To work together with other professionals in health, education and social care to provide the most effective care to adults with a diverse range of clinical conditions. Staff Responsibility: supervision of less experienced physiotherapists as well as support workers and students. Advise members of multi-disciplinary team and other agencies on physiotherapy intervention and service. work independently to manage a delegated caseload of patients/patients to assess, plan, implement and evaluate interventions Assist in the support of patients discharged from hospital and prevention of inappropriate admissions.

4 3. ROLE OF DEPARTMENT Glasgow City CHP has a diverse population of approximately 588,470 people and employs around 5300 WTE staff and has approximately 560 independent contractors. Services Managed include, Health Visitors, District Nurses, local older people and physical disability services, Allied Health Professionals, Addiction, Learning Disability Services, Palliative Care, Local Adult Mental Health and Older People s Services. The Framework for Adult Rehabilitation in Scotland (2007) and the Community Rehabilitation and Enablement Framework Next Steps (2008) required CH(C) Ps to provide a comprehensive assessment, rehabilitation and care management service delivery model. With the disaggregation of Supported Discharge and Physical Disability Services from acute services into local areas, it provided an opportunity to develop a single locally based community service. The rehabilitation team delivers a service that enables residents to maximise their health and independence through a coordinated approach by all disciplines. The broad aims of the service will be:- Single Point of entry to access multi disciplinary rehabilitation and enablement services. Service provision to address all levels of need, complexity and vulnerability Shift in service delivery towards local community provision. 5 5. SCOPE AND RANGE 1. As a Physiotherapist within NHS GG&C Glasgow City CHP North West Sector work as part of a multiprofessional/multi-agency team contributing to patient care as agreed with line manager, delivering therapeutic treatment to a designated patient/patient group 2.Work independently with delegated patients/patients, assessing, planning and implementing agreed long and short term goals to facilitate optimum level of independence, to facilitate safe and effective discharge from service and to appropriate destination 3. Within this post, the physiotherapist will work predominantly within the Rehabilitation Team North West Glasgow and will be required to cover bases in Maryhill and Partick as/when required. 4. All decisions and judgements will be set against the parameters of the overall Strategy of the organisation, its transformational themes, its policies, procedures and guidelines, professional and clinical standards. 5. Completes allocation of interdisciplinary assessments within the team; identifying issues to enable access to the service and clinical prioritisation for team and professional interventions 6. Working within a speciality as an autonomous physiotherapist who takes full responsibility for an identified complex caseload 7.To deputise for team leader in their absence taking responsibility for operational issues In conjunction with team leader responsible for the setting, implementation and evaluation of standards of Physiotherapy practice 6. MAIN DUTIES/RESPONSIBILITIES To be professionally and legally accountable and responsible for all aspects of own work including direct and indirect patient care. To assess an individual s capacity to gain valid consent and have the ability to work within a legal framework with patients who lack the capacity to consent. As a specialist therapist with advanced knowledge and skills act independently to assess, analyse clinical and non clinical information (i.e. social information) and to provide a comprehensive clinical diagnosis for individual patients, to determine their need for Physiotherapy intervention. This will include patients with rapidly progressing conditions This will include the use of evidence based assessment tools and outcome measures. Act independently to plan, implement, evaluate, treat and progress, specialised programmes of patient care for a range of patients some with physical disabilities. Some are highly complex and will include patients with complicated health and social needs requiring multi agency input to maximise patient s rehabilitation potential - including decision making regarding discharge from care. Engaging patients in the goal setting process, to formulate treatment goals. To accept clinical responsibility for the Physiotherapy patient caseload and to organise efficiently and effectively with regard to clinical priorities and the use of own and other s time and skills. To manage the Physiotherapeutic treatment of adults with a range of conditions mainly neurological which frequently change requiring regular review. To ensure a high clinical standard of care and support other staff to do likewise.

6 Work as part of a multi professional team to ensure effective communication and delivery of care Incorporate additional skills gained from CPD into clinical practice and disseminate to team members and professional colleagues. Facilitate and contribute to the development and implementation of evidence based practice within this area of clinical speciality. Maintain accurate, comprehensive and up to date patient documentation, records and accurate mandatory statistical information to reflect care provided and meet professional and local standards To provide specialist advice, teaching and training to other members of the multi disciplinary team, external agencies e.g. social work, patients, carers and families regarding patient management to ensure a consistent approach to patient car Communicate effectively, work collaboratively with and make recommendations to all relevant colleagues to maximise patient care and promote effective multi -Professional working, including case conferences, Team meetings, goal setting and discharge planning. 6. Case coordination The post-holder co-ordinates the interdisciplinary intervention by team members on an individual patient basis including:- Responsibility for co-ordinating the interdisciplinary goal planning process. Co-ordination of discharge planning and making decisions regarding follow up when necessary. Balancing the needs of the clinical caseload management, alongside key working caseload to assist in the management of the team waiting times overall. Ensuring that all necessary information is collated and disseminated to the interdisciplinary team to maximise quality of service to the patient and other service users.

7 Liaising and working collaboratively with other agencies e.g. initiating referrals, sharing of information, recommending appropriate service delivery, initiating and leading at case conferences and discharge planning. Securing discharge from the service and arranging recall where appropriate. Supervisory 1. Delegate appropriate tasks and supervise Specialist Physiotherapists and Team Assistants to maximise efficiency and achieve desired quality of care. To plan organise and prioritise the case load ( case coordination and Physiotherapy) and assist Specialist Physiotherapists and Team Assistants to achieve success in this and in the management of their delegated caseload. 2. Take responsibility for the day to day management and delivery of services to the speciality area as agreed 3. with the Team leader. Balancing this work with the case coordinator duties to ensure that each episode of 4. care within the team is well managed. 5. To ensure team reports and discharge summaries are written in accordance with service procedures and 6. ensure timely delivery to all patients and members of the wider care team. 7. To organise and deliver clinics ensuring that they are co-ordinated with sufficient treatment space and time for patients. 8. Contribute to the ongoing development of a robust framework to monitor and evaluate standards of care and clinical outcomes. 9. Demonstrate professional expertise within the speciality and with peers in other Divisions. 10. To organise and induct rotational Physiotherapy staff and students and participate in the induction of other 11. team members and colleagues outwith the Personal Development Planning (PDP) and Performance Review system to 12. promote personal and service developments. To undertake Specilaist Physiotherapists PDP s 13. Identify gaps and pressures in the service and discuss remedial action with Team leader. 14. To identify opportunities to improve the service in order to provide the best patient care possible within the resources available. To develop and implement policy changes which affect the whole Rehabilitation service acting in partnership wit the professional lead d the Team leaders. 15. To be actively involved with clinical audit and Clinical Effectiveness projects to support the Clinical Governance agenda. Comply with the organisational and departmental policies and procedures and to be involved in reviewing and 16. updating as appropriate. 17. To actively participate in the service development opportunities that may be driven by external driver s e.g. Health and social work integration 18. Undertake delegated managerial tasks on an infrequent basis, as directed and in the temporary absence of, 19. the Team Leader. Educational 1. Ensure that a comprehensive in-service Physiotherapy training programme is provided 2. Actively participate in Physiotherapy and rehabilitation service in-service training programme to promote

8 own personal development and that of staff. 3. To provide both spontaneous and planned specialist advice, teaching and training to other members of the multiprofessional team, educational staff and others (e.g. patients relatives and carers; voluntary sector, Enhanced Homecare) to promote knowledge and to enhance patient care. This can be on a general basis or 4. can be specifically adapted to Patient s individual needs. 5. To provide formal and informal teaching sessions for the Physiotherapists and Team Assistants in rehabilitation Deliver agreed formal presentations to promote service provided by the team to a variety of audiences. 6. Active involvement in the clinical training and formal assessment of undergraduate and post graduate Physiotherapy students.. 7. To undertake research or audit projects relevant to the Physiotherapy and the rehabilitation service area, disseminating findings at a local level and where appropriate, area wide. 8. To be responsible for own continued professional development and maintain professional portfolio. 9. To incorporate current available evidence into interventions to promote best practice. 10. To actively be involved in the clinical training and formal assessment of undergraduate and post graduate Physiotherapy students. 7a. EQUIPMENT & MACHINERY On a regular (daily basis) post-holder would have cause to use:- Telephone Personal Computer or laptop (involving use of Microsoft, Word, Outlook, Excel, Powerpoint, and Internet Explorer) Network Printers Facsimile Photocopier Physiotools The post-holder will daily use a vehicle to drive to patient homes, clinics and meetings The post-holder is required to understand and ensure the safe use of patient care equipment including (but not limited to) specialist beds, hoists, sliding sheets and wheelchairs To understand and ensure the safe use of Physiotherapy equipment as applied to or provided to patients. This includes equipment such as electrotherapy, exercise prescription and other e.g. orthotics/appliances and walking aids To supervise other staff to make sure this occurs and that equipment is stored safely.

9 To advise patients and carers regarding maintenance checks on equipment as applied to or provided to patients that are prescribed by Physiotherapists. To ensure the security and safe transportation and delivery of equipment. To monitor and review equipment issued by Physiotherapist in liaison with team and government standards. To maintain a small physiotherapy stock of items such as acupuncture needles, theraband, strapping, orthotics, walking aids and exercise equipment. 7b. SYSTEMS The post-holder is required to:- Follow relevant Board and Divisional Policies and Procedures. Collate information in response to data collection requests e.g. surveys, research data, information requests, waiting times data etc. where relevant accessing the rehabilitation EMISS system for information. Be competent in the use of the EMISS patient information system and EQUIPU computer system for ordering equipment, repairs and uplifts of unwanted equipment for relevant local authority areas. Be fully conversant with current rehabilitation documentation systems including patient consent, procedures, screening and recall systems, personal safety incident reporting, risk assessment and infection control. To refer on, advise and liaise with external services providing specialist equipment. This includes wheelchair services, bioengineering and orthotics. Have an awareness of current university systems for student assessments and formally assess students on clinical placement. Maintain a diary of patient appointments 8. DECISIONS AND JUDGEMENTS Decide the appropriateness of patient referrals and level of priority for the multidisciplinary team and relevant profession through interpreting clinical facts and team policies. Decide the appropriateness of own professional specific interventions, or make the clinical judgement to refer on to other agencies for the most effective service provision. Act as an autonomous practitioner making clinical judgements and decisions within the scope of professional practice, for patients with highly complex needs. This requires specialised analysis and assessment of clinical facts, which requires consideration of a range of options.

10 Be accountable for the planning and undertaking of clinical interventions and support for a patient group covering a wide range of physical disabilities including; rapidly progressive degenerative conditions, recent diagnosis and palliative care. Decide on the appropriate length of clinical interventions and when to discharge patients from the profession and the multidisciplinary team. Liaise with the necessary people and services involved in the patient s ongoing care, and make judgements regarding disclosure of clinical information and team input. Judge the appropriate level of responsibility and competence of junior staff and team assistants, and delegate tasks where appropriate. Judge a patient s capacity to consent to treatment and decide on the need to seek specialist support and opinion where appropriate. Be accountable for the assessment of clinical/environmental risk and judge the patients/carers/relatives/other agencies understanding of this risk and need for information and decide how these risks should be managed. Interpret agreed guidelines on clinical practice in relation to the role of the profession within a community based multidisciplinary team, and decide how best to apply new clinical guidelines and protocols. Balance clinical and professional demands to meet the needs of patients and the service. Decide when to seek advice from more experienced colleagues/peers and clinical lead as required on complex issues. 9. COMMUNICATIONS AND RELATIONSHIPS Convey highly complex clinical information to a wide range of individuals and organisations requiring the post holder to:- Convey details of complicated treatment plans and interventions to patients in a manner that is sensitive to their level of knowledge and understanding. Present highly sensitive clinical information to patients, for example those who are palliative or terminally ill, and their relatives/carers where there is strong resistance to the facts requiring an empathetic and supportive approach. Receive unpleasant information and be exposed to stressful situations whilst in a community setting e.g. bereavement issues, rapid clinical deterioration, pain, emotional relatives, distressed patients, difficult social/relationship issues. Provide advice and respond to sensitive social situations e.g. where children are overburdened with caring responsibilities, neglect issues or substance/alcohol abuse.

11 Engage in decision making that could be contentious requiring a high degree of persuasiveness and negotiation skills e.g. agreeing clinical interventions with non-compliant patients, agree service boundaries with other services. Use highly developed interpersonal skills to persuade and motivate individuals and break down barriers e.g. agree clinical interventions with potentially resistant or hostile relatives/carers, agree moving and handling practices that may be rejected on initial patient contact. Deal with patients who have communication difficulties, and ensure that they have optimum understanding and accessible information and means of communicating their needs. Maintain effective cross agency communication pathways to ensure a consistent approach to care e.g. formal/informal communication with GPs, social work colleagues, the voluntary sector, housing, education, care agencies and the multidisciplinary team within the acute services. Participate in the training of a wide range of audiences e.g. delivering student tutorials, involvement in interteam training sessions and presenting at multi-agency training days. Ensure that patients from ethnic minorities have accessible information in relation to the team, and access to interpreting services when necessary. Recognise social and cultural practices across a diverse population. Receive verbal complaints directly from patients/carers/other agencies and respond appropriately to these events. 10. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB Physical Activity Sitting in a car driving Working in confined spaces in patients homes. Transporting/using equipment from the office to car to patient s house. Use IT equipment and be aware of own posture/positioning. May adopt static postures for lengthy periods whilst assessing dependent patients (e.g. end stage Motor Neurone Disease/MS) to maintain movement patterns or decrease spasticity.

12 Manual Handling of patients. This may include very immobile, obese, anxious patients using moderate to maximum effort often from lying to sitting position, sitting to standing and transfer onto chair and back in one session. Daily walking, climbing stairs and bending. Supporting of patients limbs during treatment. Manual physiotherapy techniques several times a day including facilitation of movement and this requires a specific level of dexterity, response to sensory feedback, and co-ordination of movement Supporting and/or assisting patients with walking and stair practice this may include sudden and unpredictable changes in movement or falls which require physical support and/or lowering to the floor ensuring patient and staff safety. This may include working in confined, cluttered spaces. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Balancing professional standards, needs and priorities with sometimes conflicting service requirements of the Community Physical Disability Team. Working with patients with chronic degenerative conditions, maintaining motivation to address health and social care issues, which may involve palliative or terminal care. Frequent exposure to emotionally demanding and complex social situations, necessitating the frequent need for clinical judgements to be made whilst working alone in the community setting.

14 Maintaining communication with a large number of external agencies, to ensure that recommended services and input are being delivered to the patient in an optimum time frame. 12. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB The post-holder must have a formal qualification leading to entitlement to registration with the Health Professions Council as, Physiotherapist. This would normally be through a degree level qualification. Minimum post qualification experience of 5 years including evidence of neurological disease and treatment, preferably in a rehabilitation service or practice is required. The post-holder will be expected to be able to show evidence of post-qualification training Continuing Professional Development relevant to the role including post-graduate courses or short-courses in relevant specialist areas of practice. The post-holder would be expected to be fully familiar with local policies and procedures within 6 months of taking up post through a period of planned induction and supervision. Knowledge of research methods and practice whilst not essential would be highly desirable as would experience of working in a multi-disciplinary community based setting. The post-holder must be computer literate at a basic level. A current full driving licence is required. The post-holder must be able to demonstrate an ability to manage a Physiotherapy clinical caseload effectively co -ordinating services in the team and to contribute fully to the working of the multi-disciplinary team.
