2 Mission & Areas Served Statements Mission Statement Our mission is to provide financial support to the Liberty Hospital by funding programs that will have a positive impact on the patients and employees of the Liberty Hospital. The Foundation does this by raising money to support specific projects and by carefully managing the endowed funds entrusted to the Foundation. Background Statement The Liberty Hospital Foundation was established in 1984 to provide financial support to Liberty Hospital by funding programs that have a positive impact on the patients and employees of the Hospital, and therefore the community and the region. This includes constructing a Hospitality House (TreeHouse), Nursing Scholarship program, Hospice Fund, Care of the Community Fund, and new Mission-Based funds, areas designed to have the greatest impact on the hospital and Liberty community. Liberty Hospital is a public, non-profit hospital created for the benefit of its community and governed by a publicly elected board of trustees. Located just north of Kansas City, Liberty is truly a regional medical center serving Clay and Platte counties, as well as the northwest Missouri corridor all the way up to the Iowa border. Much like the area around it, Liberty Hospital has grown by leaps and bounds since it opened its doors in February What started as a 130-bed facility has now grown to 250 licensed beds and more than 1,900 employees, including nearly 300 physicians covering all specialties practice medicine at the hospital. Our physicians, nurses, and staff are extremely dedicated, and we have some of the highest patient satisfaction scores in the area. Liberty s commitment to the most up-to-date technology is evident. We were one the first hospitals to invest in the da Vinci robot, a surgical device that is used by surgeons to assist with prostatectomies and hysterectomies, allowing for better outcomes for the patient. In 2011 we upgraded our system to the latest state-of-the-art version, which further expands our capabilities in minimally invasive surgery. In 2010 the hospital installed the latest in magnetic resonance imaging (MRI) technology. The Toshiba Vantage Titan features a large 28.4-inch aperture, an ultra-short open bore and noise reducing technology. The Titan provides a more comfortable exam experience for the patient and a more precise image for the physician. Our new hybrid catheterization lab/operating room offers the best of both worlds. The room is fully equipped for both surgical and intravascular procedures. This allows the physician to act fast during an emergency situation. The Birthing Center at Liberty Hospital offers a Level II Neonatal Intensive Care Nursery. This means that we can take care of newborns with a wide variety of special care needs, allowing mother and baby to stay in the same hospital. Impact Statement 2015 Top Accomplishments: Over 2,000 patients and families stayed at the TreeHouse. Thanks to the ongoing generosity of individual donors, this one-of-a-kind hospitality house continues to provide comfortable, peaceful lodging close to Liberty Hospital, regardless of guest's ability to pay. Five local high school seniors pursuing a BSN received $2,500 scholarships and opportunities to volunteer at Liberty Hospital and learn more about their chosen career path. $50,000 was distributed to support community wellness initiatives through the LiveWell Community Grant Program. The 2015 beneficiaries included: Liberty Community Health Action Team, Rebuilding Together Clay County, Liberty Public Schools, and the Liberty Sertoma Club. The Liberty Hospistal Foundation donated $50,000 from the Professional Education Fund to Liberty Hospital to purchase a 3G Simulation Manikin to provide state-of-the-art clinical training to Hospital staff. Over $30,000 has been granted to 39 Liberty Hospital employees facing unexpected crisis situations through the Hughes Family Assistance Fund. Since its founding, the Foundation has done this by raising money to support specific projects and by carefully managing endowed funds. Liberty Hospital offers a full range of medical specialties, state-of-the-art technology, and compassionate staff dedicated to exceptional patient care. The mission of Liberty Hospital is to work in partnership with our community to improve the health and wellbeing of those we serve. We envision Liberty as a leader in providing high quality, patient-centered care, and we are guided by core values of partnership, responsibility, integrity, dignity, and excellence. In 2012 Liberty provided more than ten million dollars in charity care to patients in need. Liberty Hospital Foundation continues to build a strong track record of providing significant levels of care to children and families who are uninsured and under-served. 4 Programs Programs Kyleigh's Gift Description Category The Kyleigh s Gift fund was created in memory of Kyleigh Elizabeth Weller, daughter of Rodger and Leann Weller. Kyleigh passed away at birth on April 5, 2012, at the Liberty Hospital Birthing Center. The Weller family created Kyleigh s Gift as a way to not only keep her memory alive, but to recognize the loving care and support from the Birthing Center doctors, nurses and staff. As of March 2014, Kyleigh's Gift provides sleep sack swaddles for all newborns at Liberty Hospital. Sleep sacks are a powerful tool to help encourage safe sleeping practices for newborns and help prevent infant deaths. The fund is also exploring meaningful ways to serve bereaved families in the Liberty Hospital community. Health Care, General/Other Maternal & Infant Care Population Served Infants to Preschool (under age 5),, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success In the short term, to provide 100% of families in the Liberty Hospital Birthing Center with safe sleep educational materials. And on a case by case basis, to provide safe sleep resources (crib/ pack 'n play, safe car seat, etc.) to families with great financial need. Additionally, to provide 100% of bereaved families with resources for both parents and siblings to cope with their loss. To significantly lower preventable infant deaths, including SIDS, in Liberty Hospital families through safe sleep education and to provide meaningful support to bereaved families at Liberty Hospital and in our community. The program is monitored by the Kyleigh's Gift Committee at the Liberty Hospital Foundation as well as the Birthing Center Director and dedicated Social Worker(s) for the Birthing Center. Input will also be given by the Bereavement Committee at Liberty Hospital. In 2015, a family came to the Birthing Center for delivery. In addition to the new baby, the family had several other young children and could not afford a crib or separate sleeping area for the baby. The parent was aware of safe sleep practices but had no means to carry them out. Kyleigh's Gift purchased a basic pack 'n play for this family to provide a safer sleeping option for the infant, and has since provided funds for 10 pack 'n plays for the Birthing Center. 4

5 Medication Assistance Program Description Category The Medication Assistance Program (MAP), established in 2014, provides patients with resources to help find affordable, life-sustaining medications and establishes primary care physicians to support their long-term health. As of 2015, 130 patients have been served and they are saving a combined total of $90,000 per month on their medication costs. The program has expanded to provide additional transportation and home health resources for patients. MAP also collaborated with Liberty Public Schools to provide vouchers to under-insured and uninsured students in need of acute care. Health Care, General/Other Medical Expense Assistance Population Served General/Unspecified,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success In the short term, to provide resources and assistance to 100% of patients referred to MAP. Between , MAP served 129 patients and they are now saving a total of $91, per month on their life-sustaining medications. Solutions offered to these patients include: assistance with enrollment into Medicaid or Medicare programs, establishing a primary care physician relationship, seeking generic medications, transportation assistance to/from office visits, and working with physicians to streamline treatment plans. Only $ of funds was needed to bridge a gap between long-term prescription coverage and immediate need. Long term success will be measured by continued positive outcomes for the patients. Outcomes that will be monitored by the MAP coordinator are: continued compliance with prescription regimens, keeping scheduled appointments, and reporting health and wellness outcome over time. Success can also be measured by reduced charity care dollars and reduced unpaid emergency department bills from repeated, preventable visits by the MAP patient population. The programs is monitored by the Director of Case Management at Liberty Hospital, the MAP Coordinator, additional Case Management staff and the Liberty Hospital Foundation. In 2015, MAP expanded services beyond inpatient Hospital referrals to include all of the clinics owned by Liberty Hospital, and all affiliated physician practices. MAP has also expanded to provide transportation services for qualifying patients to/from physician visits, as well as lifesustaining equipment including: Life Vests, blood pressure cuffs, and weight scales. 5

6 Professional Education Fund Description Category The Professional Education Fund houses a variety of scholarship programs at the Hospital and in the community to promote excellence in healthcare workers in the Liberty Hospital service area. The William Jewel College RN to BSN scholarship is currently supporting five Hospital employees with 100% scholarships. The High School BSN scholarship in 2015 provided five $2,500 scholarships to local high school seniors who wish to pursue a career in nursing. Health Care, General/Other Health Care, General/Other Population Served General/Unspecified,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success In the short term, the program hopes to support the development of new clinical staff within the Liberty Hospital community as well as provide support and new opportunities for current Liberty Hospital staff. An example of short term success is the creation of the William Jewel College RN to BSN scholarship - which currently provides a 100% scholarship to five nurses to continue their education at a highly recognized local unversity. Long term success of the program will build on the partnership between the Professional Education Committee and the Clinical Education department in the Hospital. A long-term commitment was made by the Foundation Board in 2015 to allocate $50,000 to purchase a 3G Simulation Manikin for ongoing training to be provided to graduate nurses, current staff, physicians, and a variety of staff. The program is monitored by the Professional Education Committee at the Liberty Hospital Foundation as well as Foundation staff. Since the high school scholarship program's creation in 2001, over half of the students who receive the long-term scholarship have been/are employed at Liberty Hospital. Since the program transition in 2013, the scope of the program has increased by providing 19 new scholarships to area high school seniors. 6

7 LiveWell Community Grant Program Description Category The LiveWell Community Grant Program supports organizations within the Hospital community with new, innovative, collaborative health and wellness programs. The first round of grants awarded in May 2015 totaled $50,000. Up to $50,000 will be awarded in 2016 in $5,000 - $25,000 increments. Community Development, General/Other Organizational Development & Training Population Served General/Unspecified,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success In the short term, grantees are required to complete a 6 month report following their award. They are required to describe if and how goals and outcomes are being met. Another goal of the committee is to try to find ways to address the needs, even if an applicant is not awarded, through connecting appropriate organizations or resources. Long term success will be measured through reporting metrics from grantees. Full grant reports are due one year after initial funding. Grantees are also encouraged to bring updates and reports to the Liberty Hospital Foundation Board as well as publicize success within the community. Qualitative goals of the program include: creating new collaborations between area agencies, and finding creative solutions to community-wide health issues. The program is monitored by the LiveWell Community Grant Committee as well as Foundation staff. The LiveWell grant cycle runs from June 1 - May 30 annually, and the 6/12 month reports of the program will be received by February 29 and June 30 of this year. Formal measurements of outcomes and goals will occur upon receipt, though many organizations have reported gains to the Foundation already in the first half of the year. The Liberty Community Health Action Team (LCHAT) received $25,000 and funds were used to hire a part-time LCHAT Coordinator in August; and a Spot Vision Screener and hearing aids/hearing health items have already been purchased for the Liberty Sertoma Club and Liberty Public Schools grants. 7

8 Hughes Family Assistance Fund Description Category The Hughes Family Assistance Fund was established in 2012 to provide relief to Liberty Hospital employees facing times of unexpected crisis. To date the fund has provided 40 families with over $ in assistance. Through the Employee Giving Campaign, this fund will continue to be sustained through the ongoing donations from fellow Hospital employees. The fund also houses the Holiday Family Adoption program within the Hospital. None of the above Population Served Adults,, Short-Term Success Long- Term Success Program Success Monitored By Examples of Program Success In the short term, the program hope to provide assistance to 100% of eligible applicants to the fund within a 72 hour timeframe and within the funding limits set by the committee. Long term success of the program will require different measures of success, based on individual cases of the applicants. In 2015, measures were established to refer all applicants to Case Management for further counseling and resources to establish long-term solutions for financial distress. Case Management has also provided a list of resources and organizations for the Foundation to provide to applicants, even if their particular circumstances do not qualify them for the emergency crisis fund. The program is monitored by the Hughes Family Assistance Fund Committee, the Case Management department at Liberty Hospital, and Foundation staff. A recent recipient of the fund shared with staff that the assistance provided to her helped restore her passion for her work as a nurse. Many fund recipients have become donors to the fund once their circumstances are stable. Thanks to the success thus far, the program is fully-funded by annual employee donations. 8
