1 NOVEMBER 2010 NEWSLETTER Melbourne Cup Day Public Holiday Melbourne Cup Day, Tuesday 2nd November 2010 is a public holiday in Victoria, Fred Help will still be available between the hours of 8.30AM to 9.00PM using the usual phone number Dispensing for Nurse Practitioners and Midwives From the 1st September 2010, nurse practitioners and/or Midwives endorsed to prescribe under state or territory legislation can apply for approval as PBS prescribers (authorised nurse practitioners/ authorised midwives). Information for nurse practitioners/midwives to become authorised PBS prescribers is available from Medicare Australia. The medicines listed for prescribing by authorised nurse practitioners are identified by NP for Nurse Practitioners and MW for Midwives in the PBS Schedule. Nurse practitioners and/or Midwives must not write PBS prescriptions for other medicines. As part of the current update, a process will run to add suffixes to all your current prescribers listed on your database. This will add M for drugs able to be prescribed by Medical Practitioners, -D for those for Dentists and E for those for Optometrists. From the 1st November 2010, the suffixes U and F will be added to those drugs which can be prescribed by Nurse Practitioners and Midwives. As of 1st November 2010, presentation of a Nurse Practitioner or Midwife script will require the following: The script type is selected as U for Nurse or F for Midwife (these characters have been selected because other uses have limited the characters available). Setting of this script type will limit the choice of prescribers to just this type and will limit the valid PBS items displayed to just those for this prescriber type. 4 Nurses and Midwives have the ability to order emergency bag items similar to Doctor s Bags. To dispense these items use the script type B as for Doctor s Bag items. In Patient Details for the Nurse or Midwife the default is <N> for No which needs to change to <Y> for YES to signify that this Nurse/Midwife is entitled to order emergency bag items. In Lists/Drugs, the items available for Nurse or Midwife Emergency Bags are shown with suffixes of G for Nurses and H for Midwives. Nurse Practitioners and Midwives scripts can be deferred, made owing, have reg24 applied, come under the 30/14 day rules and can be PBS items as long as they show on the drug list as valid for this prescriber type. PharmPay and on-line claiming are exactly the same for these new prescriber types as previously for all practitioners. Scripts dated prior to 01/11/2010 by Nurses or Midwives can only be dispensed as Private items. Scripts from these prescriber types add to Safety net as do normal scripts. Repeat forms will show the prescriber type for the original prescriber. This will show as either NP or MW after the prescriber number on the Repeat form. Continued over page
5 Shared Care Model (SCM) SCM is for prescribing by nurse practitioners where care of a patient is shared between a nurse practitioner and medical practitioner in a formalised arrangement with an agreed management plan. Further information can be found in the Explanatory Notes for Nurse Practitioners in The Schedule of Benefits. Continuing Therapy Only (CTO) CTO is for prescribing by nurse practitioners as continuing therapy only, where the treatment of, and prescribing of medicine for a patient has been initiated by a medical practitioner. Further information can be found in the Explanatory Notes for Nurse Practitioners. There is no requirement for nurse prescribers to indicate on the script that they are prescribing under a Shared Care Model (SCM) or Continuing Therapy Only (CTO) for items contained in those categories. Medicare Australia will not be monitoring this or rejecting these scripts for no annotation being present. The nurse practitioner will be responsible for ensuring they are prescribing within the conditions as recommended by the PBAC. Pharmacists will also not be required to monitor this to ensure compliance with the conditions. Kaspersky Antivirus Software and Fred Dispense The Kaspersky Antivirus Software has been found to delete essential files in the Fred Dispense Update process, resulting in Fred Dispense being unable to start up after the update is run. If this error appears, the likely answer is interference by the anti-virus software. To resolve this issue re-run the Fred Dispense Program update, when the update is running and a warning appears to delete the winifred.dll choose No. If you are unable to re-run the update please call Fred Help. It has also been suggested that you can enter the Kaspersky Console, click on "Settings", check the "Do not delete suspicious objects option and re-run the update. Please Note: If you choose to keep this setting in place after the update has been successfully completed this may expose the pharmacy to other problems with unwanted programs.
6 How to Restart erx Services If you scan an erx barcode and receive the message The eprescription could not be retrieved. Are the erx Services running? Please dispense manually. This indicates that the erx services, which are required for the erx process, are not running as they should. These services need to be restarted, follow the below steps to do this: 1. Right click the My Computer icon and select Manage 2. Click on Services and Applications 3. Select Services 4. Locate the two erx Adaptor Services 5. Right click and select Start (the top erx adaptor needs to be started first) Running the Monthly Update Each month we receive calls from sites that are unable to run the update. Very often this is due to Fred Dispense and Fred Connect being up and running when trying to do this. So, before attempting to run the updates you need to complete the following two steps: 1. Close Fred Dispense on ALL terminals 2. Close Fred Connect To do this latter step, right click the Fred Connect icon located in the system tray on the bottom right hand side of the screen and select stop. Fred Dispense Version To check the version of Fred Dispense you are running within Fred Dispense click on: Help > About Fred Dispense > Version Date. Upgrading to Windows 7 If you are thinking of upgrading to Windows 7, please note that Fred Health only supports Windows 7 Professional, Enterprise or Ultimate. These versions provide the features and structure required for Fred products. 8 PBS Changes Effective November 2010 We have enclosed in this newsletter a summary of the important changes for your convenience. For the full Schedule of Pharmaceutical Benefits visit accessed via the Fred Dispense Help Menu. This page enables you to search by the drug name in the schedule immediately. Following are amendments that do not appear, or are not fully covered, in the Schedule of Pharmaceutical Benefits Summary of Changes. Manufacturers The following manufacturer s details have been amended in the Index of Manufacturers codes section of the PBS Schedule. Streamlined Authorities New authority required streamlined items have been listed and various items have had a change to the restriction from authority required to authority required streamlined effective from 1 November 2010 as detailed in the Summary of Changes. The streamlined authority codes effective from 1 November 2010 will be listed in the Schedule of Pharmaceutical Benefits.
9 PHARMACEUTICAL BENEFITS These changes to the Schedule of Pharmaceutical Benefits are effective from 1 November The Schedule is updated on the first day of each month and is available on the Internet at Fees, Patient Contributions and Safety Net Thresholds The following fees, patient contributions and safety net thresholds apply as at 1 November 2010 and are included, where applicable, in prices published in the Schedule Dispensing Fees: Ready-prepared $6.42 Dangerous drug fee $2.71 Extemporaneously-prepared $8.46 Allowable additional patient charge * $3.83 Additional Fees (for safety net prices): Ready-prepared $1.07 Extemporaneously-prepared $1.41 Patient Co-payments: General $33.30 Concessional $5.40 Safety Net Thresholds: General $ Concessional $ Safety Net Card Issue Fee: $8.35 *The allowable additional patient charge is a discretionary charge to general patients if a pharmaceutical item has a dispensed price for maximum quantity less than the general patient co-payment. The pharmacist may charge general patients the allowable additional fee but the fee cannot take the cost of the prescription above the general patient co-payment for the medicine. This fee does not count towards the Safety Net threshold. 11 MONTHLY UPDATE PROCEDURE FOR STEP 1 Please tick 3 and initial when step is completed > PERFORMING A BACKUP A backup must be performed before running the Fred Dispense Monthly update. The pre-update backup takes a copy of the Fred Dispense data and programs before you upgrade to a later version. It is recommended that this backup is taken away from the pharmacy and stored off-site. It is recommended that you perform the backup after receiving the update notification or before running Step 3 for the program update. STEP 2 Please tick 3 and initial when step is completed > DOWNLOAD UPDATE USING FRED CONNECT ( Comm Ser ver ) Within 24 hours of receiving the Monthly Update Fred Connect (Comm Server) automatically downloads the Fred Dispense Update. Alternatively you can use Fred Connect (Comm Server) to download the update manually 1. Right click on the Fred Connect icon found in the system tray and left click on Show 2. Left click on the Plugins Tab then left click on the Utility Configuration Tab 3. Left click on the Select Program Updates to Download button. A window will appear showing the list of updates available. Scroll to the bottom of the list and tick the box next to the update required. Left click the Download Selected Updates button to begin downloading the update. 4. The Fred Connect (Comm Server) program will switch to the Downloads tab where the update download progress is shown. At this time you may wish to wait until the download completes. Alternatively Fred Connect (Comm Server) can be sent back to the system tray by left clicking on the cross on the top right hand side of the window. The update will continue to download and dispensing can continue. STEP 3 RUNNING THE PROGRAM UPDATE The next time the Fred Dispense program is re-opened after the update has downloaded; a prompt will appear to run the program update. At this point Fred Dispense and Fred Connect (Comm Server) should be exited on all computers. 1. Close Fred Dispense on all computers using <ALT + X> 2. Close Fred Connect (Comm Server) by right clicking the Fred Connect icon in the system tray and choosing STOP. You can then choose YES to the prompt to run the program update. You DO NOT need to wait until the end of the month to run the Program Update step. Please tick 3 and initial when step is completed > STEP 4 Please tick 3 and initial when step is completed > RUNNING THE DRUG UPDATE This step is to be run after close of trade on the last trading day of the month. 1. Exit Fred Dispense and Fred Connect 2. Left click on the Windows Start Menu and (Comm Server) on all computers choose Programs, Fred, Fred Dispense, Drug Update. 3. After performing the drug update Fred Dispense will run with new Drug details and prices. This can be checked by pressing <F7> Quick Drug Price lookup from Fred Dispense and typing TEST <ENTER> to check if the current month s Test Drug is displayed at the top of the screen.
