3 Public Health Professionals: Present and Future 2017 FOREWORD By the Dean Faculty of Medicine and Director of Chancellor Tuanku Muhriz Hospital, Universiti Kebangsaan Malaysia Medical Centre (UKMMC): Professor Dr. Zaleha Abdullah Mahdy Chancellor Tuanku Muhriz Hospital, Universiti Kebangsaan Malaysia Medical Centre (UKMMC) Assalamualaikum Warahmatullahi Wabarakatuh and Salam Sejahtera It is with great pleasure that I welcome the participants of the 21st National Public Health Colloquium organized by the Department of Community Health, Universiti Kebangsaan Malaysia. Our theme this year is: Public Health Professionals: Present and Future. To look into the future, let me first bring you a little bit of the past. The Faculty of Medicine, Universiti Kebangsaan Malaysia, since its establishment in 1972 has been known for its prime role as a centre of excellence for every aspect of the medical field. Its establishment was based on the people s aspirations for a national medical school using Bahasa Malaysia as the lingua franca. The vision is to produce local doctors who are trained and competent to address the issue of the shortages in Malaysia, especially to serve the health needs of the rural population. Looking at its initial aspiration, the existence of the Medical Faculty is: from the public to the public. This matches our aspiration too as Public Health practitioners, to give back to the community. The quest for knowledge has been from the beginning of time but knowledge only becomes valuable when it is disseminated and applied for the benefit of humankind. It is hoped that the 21st National Public Health Colloquium will be a platform to gather and disseminate the latest knowledge in public health and its associated fields. Academicians, Scientist, Researchers and practitioners of public health medicine and allied health sciences will be able to share and discuss new findings and applications of this rapidly challenging field. Finally I would like to congratulate the organizing committee for their tremendous efforts in organizing today s events. I hope that the 21st National Public Health Colloquium that begins today and ends tomorrow, will be a tremendous success and and bring rich experiences to all the colloquium participants. Thank you. Our theme this year is: Public Health Professionals: Present and Future. In line with the theme of this year s colloquium, we will look into the evolution of public health. Health care system has changed from traditional remedies to advanced technology in meeting the emerging needs of the population. Hence, we could see that there has been a major reorganization of health care in the country since the Independence of Malaysia in The reorganization and improvement encompasses all aspects of public health such as preventive, promotive, curative and rehabilitative. The main objective is to provide to the public for a better health. We need to come up with a comprehensive strategy and approach to improve prevention, treatment and control of diabetes as an urgent response to this growing chronic disease. In our journey to become a healthy nation in a healthy country, we have to play our roles as Public Health Professionals to advocate on community empowerment, on self-reliance and individual s responsibility when it comes to health. Lastly, for the participants in today s colloquium, I hope that today s event could be an avenue for you to create vision in education and work within the current new paradigm of public health and eventually be able to achieve a sustainable success in every aspect of public health. I would also like to take this opportunity to congratulate and thank the members of the Organizing committee for their diligence and hardwork in making this colloquium a success. 32 PLENARY PUBLIC HEALTH PROFESSIONALS: PRESENT AND FUTURE Professor Datuk Dr. Lokman Hakim Sulaiman International Medical University, Malaysia Global population health scenario is changing rapidly. New health challenges are looming which include re-emerging infectious diseases and emergence of new infections, evolving microbial drug resistance, rapid epidemiologic transition of disease burden and population growth. Rapidly changing demographic and ecosystem such as rural-urban migration and rapid urbanisation, rapidly ageing society, economic and political transboundary migration, and environmental degradation are impacting on human basic needs (shelter, clean air and water, and food), thus threatening the very fundamental of human survival and health security. Despite concurrent opportunities in the form of advances in knowledge, science and technology, the health system is not coping with the demand and changing scenario. The health system stress is systemic which is brought about by mismatch between competencies and the needs of the patients and the population at large, fragmentation of care and healthcare system, health labour imbalance and weak leadership. This is further compounded by professional tribalism and in public health, professional crisis of identity. In the near future and beyond, explosion in existing and new sciences can be both, an opportunity to address these challenges and ultimately address issue of heath equity between and within nations, or as a threat that can further compromise the fundamental human right to good health and healthy living. Big data and analytic, artificial intelligence and robotic, and genetic, molecular biology and precision medicine, are already making visible impact in healthcare. The science and technology continue to evolve exponentially and the opportunities are tremendous. But in the local context (and globally), the healthcare system and financing need to be reformed and transformed to be more responsive and supportive of the needs of the individual, community and population. Equally if not more important, the professional education need to be urgently reformed and transformed in order to produce human resources for health that are master adaptive learners and professionals, equipped with leadership qualities, moral values and skills to fit into the ever-changing healthcare situation and needs of the future Page 1

33 SYMPOSIUM SYMP 1-1 PUBLIC HEALTH EDUCATION IN SAUDI ARABIA: NEEDS AND CHALLENGES Dr. Mahmoud Abdulrahman Mahmoud Public Health Department, College of Medicine, Al-Imam Mohammad Ibn Saud Islamic University (IMSIU) In the last decade, public health (PH) has come to the frontlines in Saudi Arabia. The recent outbreak of a novel corona virus (MERS-CoV) highlighted the importance of PH services and the need for a competent PH workforce. The urgency and panic induced by infectious disease outbreaks explain the heightened interest. Decision makers' interest in public health was observed through a series of decisions, including creating a position for Deputy Minister for Public Health, changing the name of "Directorate of Primary Healthcare Centers" to "Directorate of Public Health" in all health regions, and initiating a special scholarship program to prepare health administration professionals in collaboration with US based universities. A distinguished group of PH leaders in Saudi Arabia was gathered in a structured workshop that was organized by the Al-Imam Mohammad Ibn Saud Islamic University, college of medicine to discuss the current status and future needs of PH education in the Kingdom of Saudi Arabia. The workshop highlighted the need for PH education development and outlined the challenges ahead. The main challenges laid out by participants in the workshop were; the development of an appropriate PH curriculum, appropriate training spots for practical placement, development of research priorities for PH to satisfy the needs of PH programs and agencies, attracting the most qualified academic staff, the enrolment of highly motivated students, and finally, the establishment of a quality assurance program to ensure the quality of PH education programs. The development of a framework for graduate competencies in PH was perceived to be a top priority. Moreover, setting a PH workforce surveillance system; building partnership between PH academic institutions and PH services providers; implementing national campaigns to explain what PH is about and illuminating the role of PH workers were also of utmost importance. SYMP 1-2 REPRODUCIBLE RESEARCH IN PUBLIC HEALTH Associate Professor Dr. Kamarul Imran Musa Department of Community Medicine, School of Medical Sciences, University of Science Malaysia Health Campus, Kelantan Reproducibility is the ability to re-compute analysis for a given dataset. Replicability is the chance that an independent experiment produces a consistent result. Guidelines and recommendations have discussed the importance of transparency and have urged authors to report model structures, equations, assumptions, technical details and analysis codes. An accurate model needs to be transparent and a valid model must be useful. Reproducible Page 2

34 SYMPOSIUM research improves transparency because of the code, data, metadata and software sharing. Mandatory data sharing can be an option to make analysis as complete and as transparent as possible. It also increases dissemination, meta-analysis and understanding of results. Journal can ask authors to state if their data are accessible and reanalyseable. A datathon model assembles experts from different scientific disciplines in a venue to support collaboration. During the collaboration, collaborators use open-source analytics software and device methodology to analyse potential research questions and clinicians work alongside data scientists to write code, extract and analyse data and reformulate queries in real time. The components to perform reproducible and replicable study include availability of raw data, statistical code and correct data analysis. SYMP 1-3 SUB SPECIALITY OPPORTUNITIES FOR PUBLIC HEALTH PROFESSINALS IN MINISTRY OF HEALTH Dr. Sha'ari Ngadiman Director, Pahang State Health Department Public Health Medicine is branch of medical practice which is under-recognized yet vital. Unlike traditional clinical medicine that focuses on disease identification and management on the individual level, public health seeks to promote the health of communities, environmental health, controlling communicable and non-communicable diseases, managing health services either on a local, regional, national and even global level. In assuring quality of these services, qualified Public Health Medicine Specialists are needed. Besides formal training such as MPH and DrPH/PhD, successful public health physicians require specific skills beyond scientific knowledge and clinical care. The eight core competencies of public health professionals, are; Analytic/Assessment Skills; Policy Development/Program Planning Skills; Communication Skills; Cultural Competency Skills; Community Dimensions of Practice Skills; Public Health Sciences Skills; Financial Planning and Management Skills and Leadership and Systems Thinking Skills. In recent development, Ministry of Health Malaysia had approved sub-specialist training for Public Health Medical Specialist in Communicable Disease Epidemiology, Noncommunicable Disease Epidemiology, Family Health, Occupational Health, Environmental Health and Health Services Management. However, for Military services, additional subspecialist training is in the pipeline i.e. Military Medicine. Special sub-specialist training through fellowship training based on sub-specialty requirement. Page 3

Each of us has a responsibility to address the issues that affect us as individuals, as members of families and communities, and as citizens of the world. It is of vital important that health professionals can recognize and prevent occupational and environmental disease and injury at both the individual and population levels. Dramatic changes continue to impact both occupational health ranging from the recognition of new workplace health hazards to the changing nature of work itself and environmental health ranging from climate change to how airborne contaminants adversely affect health. And dramatic changes continue to impact how we obtain, analyze, communicate, and use information for research, practice, and advocacy in this field. Moreover, relationships between occupational health and environmental health are increasingly recognized: Occupational health hazards can affect communities. Environmental health problems frequently originate in workplaces. And workrelated hazards, environmental degradation, poverty, and social injustice are often interrelated. Information alone will not prevent occupational or environmental diseases and injuries. Prevention also depends, in part, on developing the popular and political will to support prevention and to implement specific measures. Our society woefully undervalues the importance of prevention. Informed public health professionals, through their values, vision, and leadership, can help develop the popular and political will to ensure that occupational and environmental diseases and injuries are recognized and prevented, and that occupational and environmental health is achieved and maintained. SYMP 2-3 PROMOTING SAFETY CULTURE THROUGH UNDERSTANDING OF HUMAN BEHAVIOUR Dr. Halim Ismail Public Health Consultant, Faculty of Medicine, Chancellor Tuanku Muhriz Hospital, Universiti Kebangsaan Malaysia Medical Centre Making an organization to change and adopt safety culture is a daunting task faced by many occupation safety and health practitioner from the likes of safety officers to occupational health doctors. Each organization has its own unique culture which have been engraved and adhered to and to elicit change is not an easy task. The employer and their employees need to embrace the benefit of working safely while at the same time strive for production outputs of their organization. In order for occupational safety and health practitioner to be able to ensure Page 4

36 SYMPOSIUM the safety and health of both parties i.e. the employer and their employees, they need to equip themselves with the understanding of how a human behave therefore they are able to identify the areas that they need to address while promoting safety behaviour and eventually able to change the organization into adopting and embracing safety and health as part of their organization culture. SYMP 3-1 MEDICATION THERAPY ADHERENCE CLINIC SERVICES IN MALAYSIA Dr. Salmah Bahri Senior Director of Pharmacy Services, Pharmaceutical Services Divisions, Ministry of Health Malaysia A rapid rise in non-communicable diseases (NCDs) such as heart disease, stroke, cancer, chronic respiratory diseases and diabetes, has threatening socioeconomic impacts, especially when it involves lengthy and expensive healthcare treatment. Poor medications adherence is among the factors that attenuates optimum clinical benefits and reduces effectiveness of health systems. Hence, a well-formulated patient-centered care approach known as Medication Therapy Adherence Clinic (MTAC) was introduced by Pharmaceutical Services Division, MOH in 2004 to minimise this impact. Since then, MTAC has helped and supported ongoing improvements in health quality measures. It continues to be on a growth curve and pharmacists value is being recognized in integrated care models. Till now, there are 14 types of MTAC available and 839 MTAC services extended to most MOH facilities in Malaysia. To-date, and in line with National Healthcare Transformation to strengthen early detection and timely treatment at primary care, Cardiovascular Care Bundle (CCB) MTAC is our recent MTAC development. CCB-MTAC focuses on 4 types of disease management, namely Diabetes, Hypertension, Dyslipidemia and Ischemic Heart Disease (IHD). MTAC service design provides comprehensive medication therapy management and empowers patients to take an active role in managing their medications. It aims to enhance patient therapy outcome and safety, thus reducing future healthcare cost. This service emphasizes on individual patient needs and collaborate with other healthcare providers in providing the best patient care. Page 5
