5 Form Instructions This Change Request Form is intended for exclusive use by a particular living document s Working Group. It is expected that if a Working Group member receives a request for change from his/her stakeholder group (OAPC, OBHG, etc.), they will follow the steps outlined in the Living Document Framework with respect to this form. Note that regardless of the outcome of the requested change, feedback should be provided back to the original requester, including the Working Group s final decision. In the form below: Requester refers to the Working Group member bringing the request forward. Working Group refers to the particular living document Working Group that the Requester is a part of. Stakeholder Group refers to the stakeholder group (OAPC, OBHG, etc.) which the Requester represents. Change Request Form v1.0 5
6 Change Request Form Living Document: Advanced Life Support Patient Care Standards Proposed Change: Revision of the Endotracheal Tube suctioning Medical Directive Rationale for the Change: Inclusion of accidental expungement of a tracheostomy tube to allow paramedics to reinsert. This was a situation that had not een previously considered Evidence for the Change: No need for further evidence Cost Implications for the Change: Training can easily be incorporated into the current training cycle Time Implications for the Change: ASAP Dependencies: Patient's with tracheostomies generally have extra supplies, ET tubes can be temporarily used as alternatives therefore there should be no impact on the Equipment standards.
