3 1. JOB DESCRIPTION Job Title: Radiographer Band 6 Department(s): NHSGGC 2. JOB PURPOSE Justify and perform Radiographic examinations, providing direct care and a high quality diagnostic imaging service in order to assist in the management of patients referred from clinicians. To rotate through general and highly specialised modalities or be a core radiographer in a specified highly specialised imaging modality working unsupervised, assisting in the provision of an efficient and effective diagnostic service within the department of radiology. To participate in extended working day and weekend and public holiday working within NHSGG&C 3. 4 4. SCOPE AND RANGE General: Demands for Diagnostic Imaging are generated by the specific needs of each clinical area. Images must be suitable for diagnosis. Superintendent Radiographers will be available on a daily basis to provide advice when required during core working hours. A Superintendent Radiographer will agree an annual personal development plan. (PDP) Clinical Areas: Perform and direct clinical work using specialised imaging equipment to include some or all of the following: MRI CT scanning Mammography Fluoroscopy Angiography DEXA scanning Theatre Mobiles General Radiography including; A/E, GP, In-patients, Out-patients Quality Assurance Clinical Activity: As directed Clinical Provision: 24 hours, 365 days per year. Radiographic services are provided across Greater Glasgow & Clyde 24 hours/day seven days a week. Your contractual hours will therefore be worked over a proportion of these days. (i.e. incorporating weekend working). There is a requirement to work on-call/standby to cover nights, weekends and public holidays. Staff Responsibility: Participate in the training and induction of new members of staff within own area(s) of expertise. Participate in the training of Student Radiographers and Assistant Practitioners in conjunction with higher educational centres. Direct and supervise the workload of Radiographers, Assistant Practitioners and Health Care Support Workers. Deputise for Superintendent Radiographers as appropriate. 5. MAIN DUTIES/ RESPONSIBILITIES Imaging examinations use highly dangerous radiations. It is the post holder s responsibility to minimise radiation dose and risks to patients, staff, relatives, general public and self. This minimises the risk to current and future generations. Clinical: To work within Radiation protection guidelines in accordance with the Local Rules and Ionising Radiation Medical Exposure Regulations IR(ME)R 2000 to ensure that the correct patient receives the correct examination with the minimum radiation dose.

5 Act independently in the assessment of referrals for x-ray examinations, taking full responsibility for the justification and authorisation of general x-ray examinations in order to reduce unnecessary ionising radiation exposure of patients in accordance with IR(ME)R Delegated responsibility for vetting of CT referrals in conjunction with the respective CBO to meet 18 week RTT guidelines. Supervise Radiographers, Assistant Practitioners and Health Care Support Workers Daily use of verbal and written communications to query incorrect or unnecessary referrals in order to reduce patient dose or suggest a more suitable examination. Patients will have injuries or illness that will require the adaptation of imaging technique. The post holder will therefore need to utilise their developed motivational and persuasive skills to acquire accurate position and reduce mobility to produce an optimal diagnosis with minimal dose. To assess patient condition; physically, mentally and/or emotionally prior to and during each examination in order to provide the best possible image. To clinically evaluate images in order to assess quality and to determine the need for additional images to further aid patient diagnosis. Maintain a high level of expertise in the safe operation of x-ray equipment and manage faults effectively. On recognising an abnormality provide advice to clinicians on the nature of a diagnostic image either by highlighting abnormalities verbally or using the abnormality detection system (Image Commenting) to identify possible pathology. Work as part of a team to ensure effective communication and delivery of care. Prioritise workload depending on the severity of a patient condition and the direct impact on their management and organise radiographer cover as appropriate. Liaise with fellow healthcare workers and referring clinicians to provide a high quality imaging service to patients. Perform and train in specialised area which will include one or more of the following; CT, Mammography, MRI, DEXA scanning and QA. Following training and evidence of theoretical and practical competency, autonomously administer contrast media by means of intravenous cannulation in CT/MRI examinations. Be aware of the use of and the safety issues relating to the high pressure pump injector, used in CT /MRIfor injecting contrast media intravenously. Undertake aftercare of patients following injection of contrast media with respect to intravenous cannulation removal and awareness of possible adverse reactions e.g. nausea, anaphylaxis, extravasation etc. Maintain accurate patient records by input of accurate information to reflect the service provided and meet professional standards. Work independently when providing a service to patients requiring urgent imaging due to an acute trauma or illness. Be familiar with and adhere to the Control of Substances Hazardous to Health Regulations, the Health and Safety at Work Act and IR(ME)R. Undertake general x-ray examinations in satellite departments when required. Maintain a clean and safe working environment for both patients and staff in accordance with infection control policies. The radiographer works to agreed protocols. Managerial: Be able to exercise personal responsibility and make decisions in complex and unpredictable circumstances. To be held accountable for all aspects of own work.

6 Frequently assess workload to provide better utilisation of radiographic resources. Prioritise scheduled workload to accommodate for unplanned or urgent cases Be conversant with and take responsibility in the absence of the Superintendent Radiographer for routine and emergency stock and non stock ordering, and the management of stock rotation in all areas. Rearrange distribution of personnel for sickness, absence or emergency procedures. Anticipate and prepare for extended examinations or significant delays and take appropriate action. Be actively involved in departmental audit and Quality Assurance to maintain effective Clinical Governance. Delegate appropriate tasks and supervise, Radiographers, Assistant Practitioners and Health Care Support Workers to deliver high quality patient care. Educational: Participate and input to mandatory training and actively pursue Continuous Professional Development keeping an up to date personal record. Maintain knowledge of technological advances in methods of diagnostic imaging in order to promote a culture of continuous improvement within the department Develop knowledge and understanding within an evidence-based framework and transfer to situations encountered in practice. Maintain an in depth knowledge of specialist equipment used. As a reviewer, carry out annual PDP interviews with Radiographers, Assistant Practitioners and Health Care Support Workers. Be actively involved in the training and assessment of Radiographers, Assistant Practitioners and student radiographers on clinical placement, providing direct supervision at all times. 6. SYSTEMS AND EQUIPMENT A variety of specialised investigative and diagnostic imaging equipment is utilised which comprise of multifunctional controls. The equipment is operator dependent and requires specific skills to achieve images of diagnostic quality. Below is a list of the radiology equipment. Post holders use the majority of the equipment relevant to their department. Radiographic equipment costs range from 30,000 to 1million. Imaging Equipment Ceiling suspended general purpose x-ray tubes, floor to ceiling track x-ray tubes, rise and fall x-ray tables, static height tables, floating top table, erect and supine bucky assemblies and operator consoles. 7 Direct Digital Radiography (Electronic image acquisition/post processing/ storage and retrieval) DEXA scanner Ancillary Equipment The following equipment is used on a daily basis: Patient hoists and transfer equipment Immobilisation devices such as foam pads and bucky bands Label printers associated with the Radiology Information System (RIS) Suction, oxygen, emergency drugs trolleys Lead rubber equipment for radiation purposes Systems The following systems are used daily: Computed Radiography (CR): all images are viewed on remote operator panels prior to being accepted for reporting. This allows for all details to be checked and images to be manipulated to enhance diagnostic quality. Radiological Information System (RIS): used to input and access patient information relating to radiological examinations in order to review previous procedures and clinical history and record current examination details. Use software programmes such as Microsoft word/excel to create documents and tables for quality assurance and audit. Access the Internet and NHSGGC Intranet for personal and professional development PECOS (Electronic stock ordering software) Datix - Incident Reporting System Q Pulse - Document Quality System. PACS 7. DECISIONS AND JUDGEMENTS Independently evaluate clinical information provided by diverse clinical disciplines to decide whether requested examinations are justified or reject if inappropriate (advise/ discuss with referring doctor if exam is not justified). Consider mechanism of injury prior to x-ray procedures and decide upon the appropriate views to obtain the best possible diagnostic outcome. Provide leadership and supervision to Radiographers, Assistant Practitioners and Health Care Support Workers in difficult situations such as busy periods or patients requiring multiple examinations. Assess the time taken for each examination and be able to appoint examinations timeously, making sure that expensive equipment is used effectively, has little downtime and that staffing levels are appropriate. Liaise with Superintendent Radiographers regarding operational issues which may require review and/ or change in order to enhance service. Assess and understand a wide range of clinical information, from a variety of clinical disciplines. Interpret clinical information and decide whether requested examination can be justified. Be actively involved in the implementation, development and review of radiographic procedures and protocols.

8 Within their specialised modality, implement training of Radiographers, Assistant Practitioners and Health Care Support Workers. Within specialised modality assist in monitoring and evaluating professional standards. Assess mental, physical and emotional condition of patient prior to examination, in order to determine the best possible methods to employ, using existing resources and to minimise risks for both staff and patients. During examination, technique should be modified should patient s condition change. Assess condition of patient to determine if a reaction is exhibited after a contrast agent has been administered. Have the ability through accumulated knowledge and experience to discharge outpatients with confidence and without requiring radiographs to be checked by a radiologist. Be aware of how quickly controlled and stable situations, can soon become emergency, life threatening conditions and respond with speed and accuracy to minimise delays in treatment or diagnosis. On recognition of abnormal image appearances, make a decision on whether further imaging is required. Support this judgement by highlighting abnormalities and discussing radiographic findings with referring clinician if necessary. Assess images for diagnostic and technical quality Delegate tasks to Radiographers, Assistant Practitioners and Health Care Support Workers to achieve the desired quality of patient care. Assess ability and competences of students to perform radiographic tasks. 8. COMMUNICATIONS & RELATIONSHIPS Daily contact with: Patients: Providing complex information by explanation of procedures, listening to the patient s requirements in order to encourage compliance with the imaging process, e.g. concerns over radiation dose, regulations regarding pregnancy. Some patients have a barrier to understanding or are unable to communicate; the Radiographer must try to allay fears by ensuring that patients have the benefit of informed choice, particularly when coming for invasive procedures. Providing and receiving highly complex and sensitive information e.g. dealing with IV drug abusers Communication skills are adapted to meet the needs of patients who may be anxious, aggressive or intoxicated, with a variety of mental and physical abilities, in pain or of ethnic origin. The barriers to understanding must be overcome using clear, comprehensive, sympathetic and persuasive skills. Patients will have injuries or illness that will require the adaptation of the imaging technique and utilisation of developed motivational and persuasive skills to ensure the correct position and to reduce mobility, thus producing a high quality diagnostic image. When requested, provide factual information regarding the risks associated with an imaging procedure. E.g. risk of radiation to the pregnant woman. Advise Radiographers, Assistant Practitioners and Health Care Support Workers with their handling of patients concerns. Relatives / Carers: Provide information using tact and diplomacy in the context of the standards of professional and personal conduct and within the regulations governing the Data Protection Act.

9 Highly developed communication skills are required for providing and receiving complex and sensitive information. Provide reassurance and receive information. Ask and instruct relatives / carers for assistance with methods of immobilisation Radiography staff: Consult senior staff for advice. Delegate tasks to Radiographers, Assistant Practitioners and Health Care Support Workers Supervise and provide constructive criticism to Radiographers, Assistant Practitioners and Health Care Support Workers.. Training of Student Radiographers. Providing constructive criticism as part of the assessment process. Impart information of a technical nature to students and less experienced Radiographers, regarding specialist equipment and Radiographic practice. Pass on patient information when transferring patient care to colleagues. Provide handover information for each change of shift Medical Staff / Other Health Care Professionals Advise/ discuss incorrect or unnecessary referrals in order to reduce patient radiation dose. Relating highly sensitive patient information to, and discussing with referring clinicians and colleagues. Diplomatic skills are required during discussion with referrers. Liaise with medical and nursing staff: e.g. to ensure patients arrive at allocated time, are correctly prepared for examination and are using the appropriate mode of transport. Liaise with and advise the multidisciplinary team, nursing staff and radiologists in all areas, where an interventional procedure is being carried which requires imaging to provide maximum patient care. Provide advice on guidelines for relevant imaging examinations. Provide advice on the nature of an image. Seek help and advice when necessary. Student Radiographers Decide whether the student, following suitable tuition and objective assessment is capable of performing an examination safely with the patient s consent. Undertake student clinical assessments and provide constructive criticism irrespective of circumstances. Educate the student to the level of practice set out in the academic aims and objectives module handbooks and as guided by the professional body. 9. DEMANDS OF THE JOB (physical, mental, emotional and working conditions) Physical Skills: Positioning of patients demands a high degree of accuracy to minimise radiation dose i.e. avoid repeat exposure and to demonstrate the correct anatomical features required. Have the expertise to operate highly specialised and expensive equipment. (CT/MRI/Mammography equipment). Highly developed hand to eye coordination is required when manipulating X-ray equipment and images.

10 After obtaining an IV Injection Certificate, will perform IV cannulations and inject X-ray contrast and anti-spasmodics. Trauma situations emphasise the need for speed and precision, as well as accuracy, in positioning patients. Positioning patients for examinations requires the operator to manipulate the x-ray tube, patient and film, at the same time, whilst constantly assessing the condition and needs of the patient. For mobile units there is a requirement to precisely manoeuvre large, unwieldy equipment in restrictive conditions, whilst maintaining sterile no touch constraints. These units must then be positioned using multi-planar locks. This requires significant eye to hand co-ordination and spatial awareness. Transferring and positioning patients for all radiological examinations demands an on-going high level of physical strength and fitness. A frequent occurrence within the normal working day. Possess significant keyboard skills for the entry of data into the RIS, CR and PACS. Possess basic life support skills. Physical Demands: Maintain a level of physical fitness to frequently move ceiling mounted X-Ray tubes throughout three dimensions during all shifts. Ensuring a safe environment for both patients and staff requires that all heavy lead lined doors be closed and locked for each examination Walk long distances when pushing mobile X-Ray units to carry out ward radiography throughout a shift when required. This equipment weighs up to 477Kg. For the positioning of every patient there is a requirement to stand, walk and bend. Cleaning of X-ray equipment and changing mattress sheets involves bending and lifting. Up to 75% of patients may need transferred from trolleys, beds and chairs onto X-Ray tables which require the use of safe lifting and handling skills, with or without mechanical aids when required to ensure patient and self safety. Carry large cassettes to and from the X-Ray facility to the processor or CR Reader. These cassettes weigh up to 7kg. For every ionising radiation examination outwith the x-ray department a lead rubber apron must be worn. These aprons weigh up to 7Kg and are worn for approximately 2-6 theatre cases / day, an average time ranging from 20minutes- 2 hours and ward radiography averaging 13 cases/ day. To independently meet the high work rate and demands of a shift requiring alertness for 16 hours. Mental Demands: Intense concentration is required when assessing patient s condition and performing examinations to a high diagnostic standard throughout every shift period. Prolonged concentration is required during certain examinations, which can last for several hours. Prioritising workload requires diplomatic skills in discussion with referrers who all believe their patient should take priority. When on duty at night take full responsibility for the specialised imaging service, managing patients, relatives, staff and equipment. Should imaging equipment malfunction/breakdown during a shift, (occasionally during a theatre case) evaluate the situation and provide an immediate solution. The patient/clinician must be informed of the situation. The diagnosis of the patient s condition is delayed which in turn increases both the physical and emotional demands of the Radiographer.

11 Cope with the mental and physical demands of working in acute areas, independently, often having to provide images on severely injured, abusive or violent patients whilst working in the A/E X-Ray department. Cope with the mental demands of multitasking between specialised modalities sometimes on a daily basis. Emotional Demands: Perform radiographic/imaging examinations on terminally ill patients. Care and understanding is required when dealing with patients who may be anxious, distressed or terminally ill. Provide examinations in traumatic circumstances for critically injured patients in the accident and emergency department, sometimes bleeding heavily with distorted or severed limbs and/or severe burns. Move quickly from a routine examination to an acute setting due to sudden and unpredictable demand e.g. X-Ray of a painful finger with no history of trauma to a victim of a road traffic accident requiring multiple X-rays, independently taking responsibility for the diagnostic imaging service to that patient. Perform radiographic procedures on injured, distressed and unco-operative adults involving family members and carers occasionally. This includes children with nonaccidental injuries. Daily Working Conditions: Dealing with unpleasant odours, uncontained body fluids and infections frequently throughout all shifts, therefore requiring the use of universal infection control precautions. Frequent exposure to verbal abuse from patients / carers, which can be in a lone working situation with little or no support. Occasional risk of Physical abuse from patients / carers, which can be in a lone working situation with little or no support. Be required to wear heavy lead rubber aprons during Fluoroscopy Procedures sometimes in a hot theatre environment for maybe 3-4 hours at a time depending on the case. Risk of exposure to scattered Ionising Radiation particularly during fluoroscopic procedures. Moving from cold air-conditioned areas to hot air conditions in the working areas. Working constantly in artificial lighting with little or no natural daylight. Ensuring patient safety for the duration of their time in the x-ray department. Should a situation arise where the patient appears to be a potential danger to themselves or others, necessary measures would be taken Periods of time spent in front of VDU s processing data. Working with equipment that has constant low level noise. 10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Aware of how quickly controlled and stable situations can escalate to become emergency, life threatening conditions. When working with ionising radiation / high strength magnetic fields, accuracy is of prime importance with a narrow margin for error. Provide supervision and assistance to Radiographers and Assistant Practitioners performing x-ray examinations as well as being actively involved in service delivery. Dealing with patients who may become emotional and/or aggressive. Balancing mental and physical speed and accuracy ensuring that the service to patients is never compromised.

12 Be prepared to operate Imaging equipment in differing and demanding environments on a daily basis. Be able to manage and prioritise an unpredictable workload effectively and interact successfully with fellow healthcare professionals. Cope with the mental and physical demands of working in acute areas independently, sometimes having to obtain images on severely injured, abusive or violent patients. As the Radiographer on duty overnight to meet the work rate demands of emergency CT examinations and plain film imaging, Exposure to cases where the patient's prognosis is poor. Combining training in new techniques or newly procured equipment with normal patient workload. Keeping up to date with Continuing Professional Development therefore always improving the service provided. Frequently working in unpleasant conditions such as body odour, bodily fluids, excrement and contact with infectious or contagious materials. 11. KNOWLEDGE, TRAINING AND/OR EXPERIENCE REQUIRED TO DO THE JOB BSc (Hons) Radiography. HPC registration. Evidence of clinical training and personal motivation. Good communication skills. Ability to take responsibility and make decisions and act in a supervisory capacity. Team worker. IT skills. In addition to experience gained at Radiographer grade with proven competencies in applying theory to practice, communication and interpersonal skills, health and safety, organisation skills, self motivation, professional development and information and documentation handling, the post holder must also have significant clinical experience at Radiographer level with evidence of clinical training and experience in at least one specialist modality. Evidence of Post Graduate Training including training courses and study days. The College of Radiographers IV injection certificate, which has been delivered by an accredited University or equivalent. Operate high pressure Intra-venous injector pumps. Training on individual pieces of equipment used by post holder. Training / refresher on Ionising Radiation (Medical Exposure) Regulations 2000 and Ionising Radiation Regulations 99.

Your application form provides you with an opportunity to provide a supporting statement. This section is one of the most important in the application form as the information that is provided here will be evaluated and used to decide if you are invited to attend interview. This section gives you the opportunity to describe and demonstrate the particular qualifications, skills, abilities, knowledge, relevant experience and other qualities that make you a good candidate for the post. Part D (Equal Opportunities) This part of the form is optional and the information you provide in this section exercises no part of the selection process.
