2 Aim of Macmillan GP Facilitator To produce recognisable improvements in the quality of palliative and cancer care provided by the primary health care teams within the trust.
3 The Role 2 half days per week Based in Omagh Remit for Omagh and Fermanagh Community based Vacant post L/Derry, Limavady and Strabane
4 Support of GPs and others in Primary Care Setting CPD Advanced Care Planning Education FACILITATOR S ROLE Networking Motivating and Facilitating Change Communication Skills Raising Public Awareness
5 CPD Induction. Shadowing SPC Services, hospital and community. Macmillan induction / courses. Linking with other service providers Care for Cancer, Compassionate Communities, Cancer Focus. Training ACST, ACP Facilitators course.
6 Advanced Care Planning Planning, preparing and delivering ACP awareness to nursing homes, public, health care professionals. Training in progress to become ACP facilitator attended Macmillan Training day. Quality initiative pre and post assessment of nursing homes audit. Liaising with NIAS on emergency nursing home referrals.
7 Networking and Public Awareness Networking across a wide range of organisations e.g. NICaN, Macmillan, Marie Curie, PHA, RCGP and Trust networks. Raising Public Awareness World Hospice And Palliative Care Day October attendees public facing event. Macmillan share point stands in GP practices 10.
8 GP Support 28 practices within southern sector of Trust Communicated via letter and and also spoke at the recent WHEAT day. Provided information on Macmillan services Shared information on developments for palliative care within the trust. Requested feedback on ongoing difficulties in primary palliative care.
9 Education within Primary Care GP masterclass on prescribing in End of Life, Management of Advanced Stage Parkinsons and Terminal Care in Diabetes. Nurse education programme on End of Life issues and Advanced Care Planning. World Hospice and Palliative Care day what s important at end of life. Sharing information re Project Echo and partaking in Project Echo education on management issues in advanced dementia.
10 Trust Involvement A member of the ICP End of Life Project board. Member of the Trust s Bereavement group. Attendance at the Specialist Palliative Care team meetings.
11 Ongoing Projects and Audit Issues arising regarding unnecessary hospital admissions for patients in the last year of life. Education events held for nursing home staff regarding patients choices and preferences for their end of life care. Audit is ongoing of hospital admissions before and after the educational meeting.
12 Ongoing projects Working with Parkinson s UK in developing on line learning module for GP s and Allied Health Professionals on management of End stage Parkinson s Disease. Continuing audit on emergency hospital admissions from nursing home and use of ACP.
13 Involvement with Voluntary Organisations Met with local groups and service users Promotion of services with in the primary care setting. Shared contact information, referral details and services offered with all GP colleagues.
14 Involvement with NIAS Ongoing project assessing A&E admissions of End of Life patients. Assessing the uptake of Key Worker and ACP and the events that lead to emergency hospital referral.
15 Challenges Encountered Lack of feedback from GP colleagues. Crisis within the Omagh and Fermanagh area with GP shortages and overwhelming demand in other areas of primary care. Time management within my role.
16 The Future? GP feedback has suggested a need for clinical involvement for our community palliative care patients. Extension of Rapid Response nurses to a 24 hour service. Extension of the Day Hospice to the Southern Sector of the Trust.
