Earlier in the year, Bristol Hospital was named the third safest hospital in Connecticut by Consumer Reports magazine. Patients and families continue to praise the efforts of our staff with consistent patient satisfaction scores for the Emergency Center, inpatient care units and ambulatory surgery. Bristol Hospital s array of services was expanded with the addition of many new physicians and surgeons in the areas of geriatric and pulmonary/critical care medicine as well as bariatric, breast, general and orthopedic surgery. Bristol Hospital also launched the Center for Orthopedic and Spine Health and opened the new Beekley Center for Breast Health and Wellness. Meanwhile, our network affiliation with Yale New Haven Health System continues to evolve and grow stronger. With our integration into the Yale supply chain we were able to save hundreds of thousands of dollars, which further enhanced our high-quality, low cost high value position. We also have implemented a transfer process with Yale that has been used successfully throughout the past year. This has benefited our community by seamlessly coordinating tertiary care services when they are needed. This was a strong fiscal year as we ended 2013 with a positive bottom line and significant market share growth in several key service line areas. And none of this success is possible without the hard work and dedication of our employees, medical staff and volunteers. Thank you for being Everyday Extraordinary. Working with our patients, physicians, employees, volunteers and community, Bristol Hospital and Health Care Group will utilize our core values to achieve the vision which: Our efforts in providing safe and effective care were given high marks by the Joint Commission as part of the regular reaccreditation survey. During their visit, the Joint Commission surveyors commented that our organization has an outstanding culture and that we should be especially proud of our focus on quality and high reliability.. 2/Annual Report 2013 Creates a culture of safety, quality and service that is embraced as an individual and team responsibility Ensures a user-friendly continuum modeled on providing patient-centered care and services Continually assesses and promotes new services and technology Serves as the responsible steward and advocate for the health of our community Bristol Hospital received national recognition when Health Information Management Systems Society Analytics announced that we became the first hospital in the State of Connecticut and the first in the country utilizing the Meditech 6.x platform to be certified as a Stage 7 level organization on the EMR Adoption Model SM. This recognition shows how a community hospital can advance patient safety and care efficiency with a fully paperless deployment of an acute care electronic health record. We also continue to move forward in the pending acquisition of Bristol Hospital by Tenet Healthcare Corp., which purchased Vanguard Health Systems last summer. The program includes a skilled team focused in joint health, spine and pain relief, physiatry and rehabilitation, and sports medicine. One of the newest additions to the Center for Orthopedic and Spine Health is orthopedic surgeon David Rubins, MD, FAAOS. Dr. Rubins has more than 20 years of joint replacement experience and specializes in minimally invasive hip and knee replacement, MRI-assisted knee surgery and revision total joint replacement. One of Dr. Rubins areas of special interest is the direct anterior (DA) approach for total hip surgical repair. The DA approach is a minimally invasive technique not widely used and involves a smaller incision, no detaching of muscles or tendons, improved component positioning, equal leg lengths, decreased dislocation rates and decreased patient hospital stays. 6/Annual Report 2013 Bristol Hospital: Celebrating Quality Care/7

5 Bristol Hospital s Beekley Center for Breast Health & Wellness officially opened with a ribbon cutting event on Oct. 24. The new, 6,100-square-foot center offers patients comprehensive breast care and wellness in a spa-like setting. The breast health center includes a water wall, fireplace and intimate design aimed at making patients feel at ease. Each patient will have a nurse navigator who will assist with appointments and other details of her treatment. The Beekley Center for Breast Health & Wellness is made possible by a generous $1 million donation from the Beekley Family Foundation and Beekley Corporation. A team of stakeholders consisting of breast cancer survivors, volunteers, and Bristol Hospital and Beekley Corporation employees visited other breast centers in the region, and had significant input into the design and layout of the center. The event raises funds for the Saint Vincent DePaul Society shelter in Bristol. 16 2013 Highlights Bristol Hospital was named the third safest hospital in Connecticut according to Consumer Reports magazine. Bristol Hospital s third place safety ranking appeared in the May 2013 issue of Consumer Reports, and is based on a composite score that was calculated by a hospital s performance in five key safety measures including readmissions after discharge; complications; clear communications in such areas as drugs and discharge planning; overuse of CT scans; and infections. Bristol Hospital and Health Care Group Inc, was the recipient of a generous $75,000 grant that was awarded by the Connecticut Health and Educational Facilities Authority (CHEFA). The CHEFA grant was used to upgrade and renovate the hospital s current behavioral health area. This includes providing a more efficient patient experience from assessment and triage in the Emergency Center to treatment in the Inpatient Behavioral Health Unit. The renovations include upgrading the current crisis area in the Emergency Center for patients with mental illness and substance abuse/ dependence as well as numerous renovations in the hospital s Inpatient Behavioral Health Unit. John Tichon of Food and Nutrition Services was named the 2013 Employee of the Year for Bristol Hospital and Health Care Group, Inc. Tichon was presented with the award at the hospital s annual Employee Services Award Dinner by President and CEO Kurt A. Barwis, FACHE. This year s Nightingale Award for Excellence in Nursing recipients from Bristol Hospital included (from left): Barbara O Neill, RN, BS, MEd, operations manager for the Emergency Center; staff nurse Patricia Kenney, RN, BSN, CPAN; and staff nurse Susan Ferreira, RN. In collaboration with the Connecticut Hospital Association, Bristol Hospital and Health Care Group, Inc. began a journey to establishing a High Reliability Organization. High Reliability is a culture and methodology that radically reduces systems failures and catastrophic events. As part of the journey, all Bristol Hospital employees are required to attend the Safety Starts With Me presentation. The presentation helps employees gain an understanding of the hospital s safety initiative and the principals of human error prevention. Safety Starts with Me provides employees with information and tools that include four safety habits for error prevention with an explanation of how they are used in the hospital setting. 17 2013 Highlights The seventh annual Rally for the Cure golf tournament took place Sept. 7 at the Chippanee Country Club in Bristol. The event included approximately 80 golfers and raised $7,100 for Bristol Hospital s Beekley Center for Breast Health and Wellness. The 18th Annual Bristol Hospital Development Foundation Golf Classic which took place June 10 at the Golf Club of Avon raised more than $75,000 for Bristol Hospital. The funds will support numerous programs at the hospital. Since it began, the Bristol Hospital Golf Classic has raised more than $1.3 million for the hospital and this year s tournament featured approximately 172 golfers. Bristol Hospital was among a select group of hospitals nationwide recognized by the U.S. Department of Health and Human Services (HHS) for reaching gold, silver, and bronze levels of outreach for organ and tissue donation and registration. Bristol Hospital conducted awareness and registry campaigns to educate staff, patients, visitors, and community members about the critical need for organ, eye, and tissue donors and, by doing so, increased the number of potential donors on the state s donor registry. The hospital earned points for each activity planned between September 2012 and May 2013 and was awarded bronze recognition through the Workplace Partnership for Life Hospital Campaign, a program launched in 2011 by HHS s Health Resources and Services Administration. 32/Annual Report 2013 Joseph Cherneskie, MD a physician of the Bristol Hospital Multi-Specialty Group, Inc received recognition from the National Committee for Quality Assurance (NCQA) and the Patient-Centered Medical Home 2011 (PCMH) program for using evidence-based, patient-centered processes that focus on highly-coordinated care and long-term participative relationships. He was the first physician at Bristol Hospital to receive Level 3 recognition which is the highest PCMH honor. The event featured volunteer recognition and service awards, board member recognition and family testimonials. Bristol Hospital s Bariatric Surgery Program received a Level 1 Accreditation by the American College of Surgeons Bariatric Surgery Center Network (ACS BSCN). The accreditation shows that Bristol Hospital has met the essential criteria of a surgery center that is fully capable of supporting a bariatric surgery care program and that its performance meets the requirements by the ACS BSCN Accreditation Program. Bariatric Surgery Centers that are accredited under the ACS BSCN program standards are part of the Metabolic Bariatric Surgery Accreditation Quality Improvement Program administered by the American College of Surgeons. 18 2013 Highlights Bristol Hospital Patient Representative Andy Adams was one of the recipients of the Bristol Boys and Girls Club PLUS Award. Adams was honored for her volunteer work in the community with such organizations as Bristol Rising, the Bristol Parking Authority and the American Clock and Watch Museum. Adams has worked at Bristol Hospital since Staff from Bristol Hospital recently celebrated the accreditation of the Bristol Hospital Sleep Center by the American Academy of Sleep Medicine. The five-year accreditation welcomes the Bristol Hospital Sleep Center into the ranks of the AASM-accredited centers, which provide the highest quality of patient care. The AASM is committed to ensuring that those in the field of sleep medicine provide excellent health care and enhance the awareness of sleep as an important element for health, public safety and quality of life. Health Information Management Systems Society (HIMSS) Analytics announced that Bristol Hospital has become the first hospital in the State of Connecticut and the first in the country utilizing the Meditech 6.x platform to be certified as a Stage 7 level organization on the EMR Adoption ModelSM (EMRAM). HIMSS Analytics developed the EMR Adoption Model in 2005 as a methodology for evaluating the progress and impact of electronic medical record (EMR) systems for hospitals in the HIMSS Analytics database. The EMRAM has eight stages, ranging from zero, (a predominantly paper environment), to seven, (a predominantly paperless environment). Tracking their progress in completing these eight stages, hospitals can review the implementation and utilization of information technology applications with the intent of reaching Stage 7, which represents an advanced electronic patient record environment. The annual Bristol Hospital Ball, The Best is Yet to Come, took place Nov. 16 at the Aqua Turf Club in Plantsville. Bristol Hospital recently received more than $443,000 in financial incentives by participating in Energize Connecticut energy efficiency programs offered through Connecticut Light & Power and Yankee Gas. These incentives helped offset the cost of a $1.75 million project that included upgrades to the hospital s energy infrastructure, ranging from the installation of energy efficient cooling towers and a heat exchanger to the chilled water return, to the installation of an energy management system to optimize the water chiller plant s operation.
