No. of sessions and programmed activities 10 programmed activities as per consultant contract for England. Each Group is led by a triumvirate consisting of Group Director, Group Nurse Director and Group Medical Director and the triumvirate structure is mirrored at the directorate and locality service level. Planned Care Group Community Care Service covers the six localities which make up Northumberland, Tyne and Wear and encompasses all mainstream Community Mental Health services, including day services, IAPT Services, Working Aged Adult services, Older Peoples services and Learning Disability services managed by the Trust. We work in close partnership with primary care and local authorities in these areas. The Service is divided between two Directorates; one encompassing all Community Services in Northumberland, North /cont d page 2

3 Tyneside, South Tyneside and Sunderland and the second covering Community Services in Newcastle and Gateshead as well as the Trust wide teams managed within Planned Care (CBT, CAT, Psychotherapy and Personality Disorder Hub Team). Inpatient Care Group Inpatient Care Services are divided into two directorates: Adult Mental Health Services and Older People s and Learning Disability Services. Our Inpatient Care Services include adult assessment and treatment (inpatient) units, stepped care (inpatient rehabilitation) units, functional and organic older peoples and learning disability assessment and treatment (inpatient) units, psychiatric intensive care units (PICU), crisis resolution home treatment teams, liaison psychiatry, court liaison, and electroconvulsive therapy. Specialist Care Group Our Specialist Services are divided into two directorates: Specialist Adult Services and Children and Young People s Services. Our Specialist Services provide a range of assessment, treatment and rehabilitation services for patients that include forensic and specialist services, neuropsychiatric, neuro-rehabilitation, autism, addictions and services for children and young people. Specialist services includes a regional affective disorders service, a regional eating disorders service, a gender dysphoria services, mother & baby services, services for the deaf and a range of psychological therapies. Service details This is a community post working with an established community mental health team serving the Northumberland area. The population served is approximately and covers the central and west of Northumberland which includes the districts Ashington, Morpeth, Bedlington, Hexham and Prudhoe. The consultant in post will work closely with the community team which is made up of colocated members of the local authority and Northumbria Healthcare. The post holder will provide senior expertise in assessment and treatment of adults with a learning disability and mental health problems. There is a population of Learning Disability Forensic patients living within Northumberland for whom the post holder will have responsibility. The post holder will be expected to work closely with the Risk and Independence team for the management of these clients in the community. In addition, there is the Community transitions team who work closely with some of these forensic clients and the post holder can draw on their expertise should they feel it necessary. The community learning disabilities team is based at Foundry House, The Oval, Bedlington, Northumberland, NE22 5HS and Dene Park House, Corbridge Road, Hexham, NE46 1HN The post holder will have good links with the other consultant colleagues within learning disability services. Currently there are six consultant colleagues and 1 specialty doctor working within learning disability services in community. At present, NTW has 1 consultant working in inpatient LD /cont d page 3

4 service, 4 consultants working in forensic learning disability service and 3 consultants and a specialty doctor working in children and young people s LD service. The Medical staff meetings are the monthly forums to meet up with these colleagues, which the post holder would be encouraged to attend alongside the trust wide medical staff meetings that include consultant colleagues from all the other specialties within NTW. The post holder will also join the well-established peer group for Community Consultants in Learning Disability for the purposes of revalidation. In-patient services There is an in-patient acute treatment and assessment ward at Rose Lodge in Hebburn. Clinical Support staff and facilities associated with the post:- Multidisciplinary Team There is access to a comprehensive Multidisciplinary team which has achieved excellence is several areas: 10 community nurses with specialist areas of expertise in dealing with and monitoring risks around Forensic clients, patients with Autism, physical health and challenging behaviours. 3 Support workers 10 Social Workers who are expected to monitor risks for clients placed within various care settings. The post holder will be expected to work closely with the social care team to monitor risk in the community. 1 Speech & Language Therapist 2 Occupational Therapists Physiotherapists who work across the county. Positive Behaviour support team covering Northumberland include:- Psychologists Nursing 2 Assistant psychologists in this team. 5 Office accommodation The post holder has his own office that is based at West Farm House, Cramlington IT support All Consultants are provided with a personal computer system with and Internet access. The Trust has now adopted an all-electronic case record system for patients in the Learning Disability Directorate. The consultant will be provided with suitable electronic equipment to access the electronic case record system in the community. Workload The anticipated workload for the aforementioned post would be around 200 patients with new referrals of about 5 per month. Currently there is 3monthly to 6 monthly follow up arrangements practiced however the post holder will be encouraged to develop his own practice depending on best practice guidance. There are about 8-10 service users within the caseload that have links with forensic services. The links to Northumbria healthcare are detailed in appendix 1 Currently the services within Northumberland are undergoing transformation to establish better pathway of patient care alongside Northumbria trust. The post holder would be encouraged to be part of this transformation process to establish evidence based pathways. Continuous Professional Development (CPD) and supervision The post holder is expected to be in good standing for CPD and to maintain their good standing via an annual Personal Development Plan (PDP). The post holder will be expected to participate in one of several established peer groups in which colleagues meet to draw up PDPs. The Trust strongly supports participation in the CPD programme of the Royal College of Psychiatrists. Study leave The post holder is entitled to study leave as per the conditions of the consultant contract for England. Consultants are required to take study leave entitlement in line with learning objectives identified in Personal Development Plans (PDP). There is an established system of mentorship for newly appointed consultants in the Trust. The consultant is encouraged to choose a trusted and respected senior colleague with whom to meet on a regular basis. Operational Leadership The post-holder s immediate line management responsibility is held by the Service Manager, Jose Robe, who is supported by a Lead Consultant for Learning disability Services working across all trust localities. Consultants chose from a list of Consultant colleagues trained as appraisers. An online 360 appraisal tool ( is provided and paid for by the Trust.. The Deputy Medical Director for Medical Revalidation, Appraisal and Performance is Dr Eilish Gilvarry who reports to the Executive Medical Director and Responsible Officer (RO) Dr Rajesh Nadkarni. Job planning arrangements This occurs annually with the lead consultant following appraisal. Trust and Directorate objectives are formulated jointly by the Community Services and the Lead Consultant. Local service objectives may be defined and included in the job plan. Directorate and service objectives, together with key issues from appraisal, are used to negotiate the consultant s personal objectives for the year ahead. We encourage consultants to develop specialist skills in line with the Integrated Business Plan or New Developments and these would be supported through the job planning process. Special Interest The trust encourages Consultants to develop and maintain a special interest where this is beneficial to our care group. This will be accommodated in job plans. The development of special interests will need to be in line with the Trust and National transformation agenda and other commissioned services. /cont d page 6

7 Teaching, Research and Clinical Governance Psychiatry training in NTW NHS Foundation Trust The core training posts are available in learning disability psychiatry in Northumberland Tyne and Wear NHS Foundation Trust 1 trainee in general adult LD 1 in Forensic LD 1 in Child and Adolescent Mental Health Services (CAMHS) LD (plus a further paediatric trainee). Northumberland Tyne and Wear NHS Foundation Trust is also part of the Northern Deanery Higher Specialist Training scheme in Learning Disability along with Tees Esk and Wear Valleys NHS Foundation Trust. Usually there are upto 4 senior trainees placed with us at any one time, up to 2 in general adult LD, and 1 in forensic LD and 1 in LD CAMHS. We encourage consultants to become trainers for specialist trainees and this opportunity would be open tor the post-holder. Educational Supervision All trainee doctors receive one hour of educational supervision time per week, over and above any day-to-day clinical supervision. If post holder identified as an educational supervisor then he would have the appropriate amount of time allocated as part of the agreed job plan for this activity. Teaching Programme There is a monthly postgraduate programme of case conferences, journal clubs and audit meetings on Thursday mornings. The post holder is expected to participate in these as part of their SPA allocation. The Learning disability service has a very active, well established and highly appreciated weekly undergraduate teaching program. The post holder will be included with other Learning disability Psychiatry colleagues in this programme of educational meetings based at Northgate hospital/ Ferndene hospital Prudhoe Within NTW, there is a college tutor with responsibility for learning disability psychiatry (Dr Ruth Bevan). She is responsible for the coordination of the learning disability educational programme. This is for both junior doctors and consultants. The post holder is expected to participate in the local teaching programme. This takes the form of both being a regular attendee, and contributing to the programme by providing occasional teaching sessions about aspects of learning disability psychiatry.. MRCPsych Course There is a half-day release course for all ST 1-3 trainees who are preparing for their professional exams. The course is co-ordinated by Professor Alan Thomas. 8 It is expected that the post holder will contribute to the teaching of Medical Students who are on placement within the Service. This responsibility will be shared with other locality based Consultants as well as colleagues from other disciplines. The contribution of each individual Consultant will be agreed through the process of Group and Individual Job Planning. RESEARCH and UNIVERSITY LINKS The Trust has recently approved a Research and Development (R&D) strategy, which aims to make NTW NHSFT one of the most research active mental health and learning disability trusts in the UK and internationally, while also aiming to ensure that our service users benefit from this locally. The Trust is an active participant in local and national Clinical Research Networks (LCRN), and currently hosts the local leadership for the mental health and Dendron (Dementias and Neurodegenerative Diseases) Specialty Groups of the LCRN. NTW Trust has strong links with Newcastle University. A number of academic psychiatrists employed by the University undertake clinical activities within NTW and some NTW staff have honorary positions with the University in research or teaching roles. Research within Newcastle University is based within a number of Institutes. Clinical academics involved in teaching are based in the School of Medical Education (see The majority of clinical academic psychiatrists are based within the Institute of Neuroscience (see A smaller number are based within the Institute for Health and Society (see A key theme for the University is ageing for which there is an over-arching Institute for Ageing ( The two largest groupings of academic psychiatrists are engaged in research related to the neurobiology and treatment of dementing illnesses (linked with a Biomedical Research Centre on Ageing and a Biomedical Research Unit on dementia) and affective disorders. Additionally research is undertaken around childhood and developmental disorders (particularly autism), psychosis and forensic psychiatry. In the 2008 Research Assessment Exercise (RAE), the majority of research in the Psychiatry, Neuroscience and Clinical Psychology Unit of Assessment was officially classified as world-leading, internationally excellent or recognised internationally in terms of originality, significance and rigour. Research grants in excess of 10M are held at any moment in time, awarded from a range of bodies including the Medical Research Council (MRC), the National Institute for Health Research (NIHR) and various charities. Opportunities may arise to participate in research being carried out by the Trust or University based staff in conjunction with the NIHR Clinical Research Network and the postholder will be encouraged to participate in these research studies, while being allowed opportunities to pursue their own research interests where this occurs in alignment with the needs of the service and the interests of the Trust. The Trust s R&D strategy includes initiatives to develop talent in evaluation and R&D; the Trust s R&D office will support promising grant applications with advice and mentoring; and there is also the opportunity to bid for dedicated research time (from Local Clinical Research Network) small grants within the Trust (funded by Research Capability Funding) where ideas are likely to lead to future successful NIHR funded grants. The post-holder will also be expected to be an active participant in quality improvement methodologies such as audit; ensuring it aligns with the Trust s annual audit priorities. Medical students from Newcastle University are hosted within the Trust. Teaching at Newcastle University has equal parity with research. The MBBS programme consistently /cont d page 8

9 rates highly across the standard ranking tools and in formal GMC review. The University has campuses in Newcastle and Nusajaya, Malaysia. NTW clinical academics involved in teaching are actively involved in pedagogic research as well as innovation of service. Involvement in clinical teaching is expected for all clinicians and the Trust has an active medical education department with opportunities for personal professional development in both undergraduate and post-graduate teaching. Post holders wishing to become trainers will be expected to gain the key competencies as set out in the GMC s Recognising and Approving Trainers standards. Library Facilities There is a medical library with a dedicated librarian. There is a large collection of books, journals in print and other publications Clinical Governance / audit NTW considers clinical governance to be essential in maintaining good practice. To this effect there is a Deputy Medical Director of Quality and Safety who takes the leadership for clinical governance for the Trust. All consultants are encouraged to take part in audit activities. The trust has an active audit cycle with an annual audit plan. We have a track record of working with external agencies in developing and maintaining good practice such as the Prescribing Observatory for Mental Health UK. Audit is supported by a number of clinical audit officers and is lead by the Director for Research and Development, Innovation and Clinical Effectiveness (Dr Paula Whitty) Section 12 and Approved Clinician Training The post holder will be expected to hold approval under Section 12 (2) for the Mental Health Act 1983 revised 2007 and also be trained as an approved clinician. /cont d page 9

10 Role of the Consultant The consultant will be expected to provide clinical leadership to the team, through good multi-professional working as outlined in the document New Ways of Working for Consultant Psychiatrists (DoH, 2005). The consultant will have shared responsibility for the clinical care of those patients referred to the team and will be expected to work closely with the team to ensure the provision of an effective service. Principle Duties: To provide medical assessment, diagnosis and formulation to the Community Learning Disability Team patients. On completion of assessment advise on diagnosis and treatment within the team, primary care and to external providers. To provide (or ensure referral to the appropriate service for the provision of) up to date evidence based interventions for the patients of the West and Central Northumberland Community Learning Disability Team To provide leadership, consultation, advice and guidance within the West and Central Northumberland Community Learning Disability Teams respectively To work collaboratively with Senior Clinical Colleagues in the West and Central Northumberland Community Learning Disability Teams respectively To work collaboratively with colleagues in other mental health teams. To take a lead role with the Team Manager and other colleagues in service improvements and development. To contribute to the ongoing training and development programme within the team. To lead, collaborate and advise on research and audit projects in the team, within the clinical governance agenda. External duties To assess and make recommendations under the Mental Health Act. To collaborate with GP s, hospital doctors and primary care clinicians regarding the physical health care of the West and Central Northumberland Community Learning Disability patients. To liaise with the West and Central Northumberland Community social work team Other duties To complete administrative tasks necessary for the fulfilment of the clinical role. /cont d page 10

11 To train and supervise junior medical staff, medical students and other allied professions. To share responsibility with Consultant colleagues for ensuring a College Tutor is available, subject to approval by the Royal College of Psychiatrists. On call category and supplement The consultant makes a pro-rata contribution to the out-of-hours rota covering Learning Disability emergencies across the Trust (Northumberland, Newcastle, North Tyneside, Gateshead, South Tyneside and Sunderland). The rota is currently 1 in 11 and provides third on call supervision to ST 4-6s and CT 1-3s. There is a CT 1-3 first on call every night and a ST4-6 on 3 times a week. When there is no ST4-6 the consultant provides second on call cover which includes responding to emergencies in the community. Remuneration is at Band A of the new Consultant contract (3%). Annual leave The post holder will be entitled to annual leave as specified in the consultant contract. Cross cover arrangements The post holder will be expected to provide cross cover for his or her colleagues amongst the Learning Disability consultants in the Trust for annual, study and emergency sick leave. Provisional Job plan and Timetable The post holder will typically have 7.5 programmed activities allocated to direct clinical care, to include: Weekly complex care meetings where patients in crisis are discussed. These meetings are sometimes chaired by the Northumberland CCG and have been shown to be effective in preventing admissions to hospitals. Collaboration with other members of the team at CPA Meetings etc. Planned patient contact in clinics, wards or home visits. Liaison with Consultants from inpatient wards and other mental health teams. 15 Terms and conditions and salary scale As per new consultant contract for England. Any change to this contract would be by bilateral consultation and negotiation. a) The post is subject to the Terms and Conditions Consultants (England) 2003) and General Whitley Council Conditions of Service, as amended from time to time. b) The current salary scale will be in accordance with the New Consultant Contract and Local Agreed Framework. c) Progress through thresholds will be automatic. The criteria for progressing is: a. Fulfilling Job Plan b. Cooperating with appraisal c. Satisfactory achievement of objectives d) For pre-employment health assessment purposes, as part of the Trust s Occupational Health Service, the successful candidate will be required to complete a Health Questionnaire and Hepatitis B Status Questionnaire. These will be treated in strict confidence. The successful applicant may be required to undergo a medical examination. e) To comply with HSC 2000/019 the successful candidate will be subject to completion of a declaration stating that they are not subject to fitness to practice proceedings by a UK or overseas licensing or regulatory body. f) The successful candidate will be expected to undergo clearance by the Criminal Records Bureau to an enhanced level. Rehabilitation of offenders and fitness to practise In addition to the Rehabilitation of Offenders Act you are not entitled to withhold any information relating to fitness to practise. You must disclose whether or not you have been or are the subject of fitness to practise proceedings, or if any fitness to practise proceedings are being contemplated, by a licensing or regulatory body in the UK or another country. Failure to disclose a criminal offence, having been bound over or cautioned or that you are currently the subject of criminal proceedings which might lead to a conviction, an order binding you over or a caution, or fitness to practise proceedings undertaken or being undertaken by an appropriate licensing or regulatory body, may disqualify you from appointment, or result in summary dismissal/disciplinary action and referral to the General Medical Council for consideration if such a discrepancy came to light. 16 Conduct, performance and health of medical staff The Trust is committed to providing safe and effective care for patients. To ensure this there is an agreed procedure for medical staff that enables them to report, quickly and confidentially, concerns about the conduct, performance or health of medical colleagues (Chief Medical Officer, December 1996). All medical staff practising in the Trust should ensure that they are familiar with the procedure and apply it. Data protection Act 1998 While you are at work you will have access to information about other people. You may find this information out as a part of your work, or you may simply see, hear or read something while you are working. All this information, whether about patients, friends, relatives or other employees is confidential. It is essential that all this information is kept confidential. The Data Protection Act applies equally to information which is stored either on a computer or in a manual record, and anyone revealing this information is liable to prosecution. The Act says in particular: 1. that any information about any person must be obtained, stored and used fairly and lawfully. 2. the reason why information is being held must be specified. 3. that information can only be used for the purpose for which it was obtained - it cannot be passed on to someone else to use it for any other purpose This third part cannot be stressed too strongly. If anyone employed by the Trust passes on information in any unauthorised way both they and the Trust may be prosecuted. Unauthorised access, modification, transfer, disclosure or deletion of computer held records are criminal offences under the Computer Misuse Act 1990 and make the offender liable to a fine or 5 years imprisonment or both. These offences constitute gross misconduct and may result in summary dismissal of the offender. Unauthorised access, modification, transfer, disclosure or deletion of manual records will attract similar disciplinary action. From November 1987 anyone is able to see, and have explained to them, any information about them which is held on any computer. Please remember that any requests to reveal information must be made through a responsible person and that a fee must be paid.
