Course Selection Name: BU ID #: Please select the course you would like to take abroad this summer. Any changes can and should be made upon arrival in Peru. * Return the Course Selection Form by April 15 to: Boston University International Programs 888 Commonwealth Avenue, Boston, MA (P) (F) abroad@bu.edu In Lima you will have the choice of two courses. If you have not taken LS 304 (Composition and Conversation in Spanish II), we highly recommend that you select LS 304 as your course choice. A minimum enrollment of five students per course is necessary for a course to be offered. o CAS LS 304 Composition and Conversation in Spanish, 4 cr. o CAS LS 452 Topics in Latin American Literature and Culture: Contemporary Peruvian Literature, 4 cr. In Ayacucho, all students will take CAS PO 465 Peruvian Politics During and After Shining Path (4 credits) and will participate in a Local Agency Affiliation CAS IP 400 (2 credits). Both Ayacucho courses are required courses.
Community Affiliation Questionnaire, Page 1 This form needs to be filled out carefully and thoughtfully to assist in finding an appropriate affiliation for you. All students must complete this page. 3. Describe any relevant paid or voluntary work experience. 1. Briefly explain the type of organization you would like to be affiliated with and why. 4. What do you perceive as your strengths and weaknesses? 2. Please list relevant courses completed and the grades you attained in each. Please list complete course titles, not just course numbers.
Community Affiliation Questionnaire, Page 2 5. What is important to you in a professional environment? Please attach a sheet (if needed) to include any additional information that would be useful when considering your field placement. STUDENT SIGNATURE a) I understand that community affiliations are dependent on the opportunities available at the time of the placement, and on my previous experience and qualifications. b) I understand that International Programs may not be able to guarantee my first choice, but that every effort will be made to place me in my desired field. c) I understand that if I wish to change my primary field placement area, I must consult with program staff in Boston and complete a new questionnaire. d) I understand that standards of acceptable appearance and demeanor vary dramatically from country to country, and even from business to business. By participating in a Boston University community affiliation, I understand that I am an emissary of the University and the United States, and that my enrollment constitutes an agreement that I will attempt to live and work within the framework of local custom. I understand that some personal choices of dress, hairstyle/color, body ornamentation, etc., that are acceptable on campus in the United States may make a field placement difficult, if not impossible. By enrolling in the program, I agree to make every effort to adapt my personal appearance to the generally accepted norms of the community of the host country and understand that failure to do so may make placement impossible and result in a failing grade for the field placement course. I understand that where dress, hairstyle, and body ornamentation are dictated by my religion, race, national origin, or ethnicity, every effort will be made to accommodate those needs consistent with my status as a guest in a foreign country. Student Signature Date Print Name * Return the Community Affiliation Questionnaire (2 Pages) by April 15 to: Boston University International Programs 888 Commonwealth Avenue, Boston, MA (P) (F) abroad@bu.edu

Homestay Agreement, Page 1 This form ensures that you understand and agree to the terms under which program housing is provided. Please read this form carefully and make a copy for your records. You will be held to the terms of this Homestay Agreement once in Peru. Below you will find listed the terms of the homestay residence agreement for Peru program participants. All students participating in a homestay must agree to these terms and sign this contract stating so. Please make a copy for your own records. Students on Boston University s Peru program are subject to the same policies, procedures, and codes of conduct in place at Boston University. Students are prohibited from engaging in conduct, in the homestay, that poses a threat to the health or safety of persons or property, that interferes with the rights or well-being of others, or that violates any provision of the Code of Student Responsibilities, this residence agreement, or any rule/regulation of the University or International Programs, or any other applicable law. Students found in violation of these policies, rules, and regulations will be subject to University disciplinary action up to and including expulsion from the University. Students must abide by all rules and regulations that may be established by International Programs and the program directors in the interests of health, safety, the proper conduct of students, and the orderly and efficient operation and administration of the program and its residential system. Students must attend all program and individual meetings as they are convened by the Resident Director and/or program staff throughout the semester. The student is responsible for knowing and abiding by any materials and information discussed and/or distributed at any such meetings. TERMS OF AGREEMENT Key The household will provide you with a key for entrance into the house and will consider you to be an adult. The household will not be responsible for your comings and goings. In case the key is lost, Boston University declines all responsibility, and you and the household will have to work out the terms of incurred expenses. Once you finish the program, you must return the keys. In & Out You may come home at late hours. However, you must respect the sleeping hours of the household, whether it is late in the evening or early in the morning. Absences Out of courtesy and respect, you should always notify your host family if you intend to be away for an evening or a weekend and if you will be missing a meal. Meals You should expect to eat what your family eats. If you have dietary restrictions, the family should be notified at the beginning of the term. Kitchen You should not expect to have kitchen privileges. You may not use the kitchen, invite friends over to eat, etc., without first receiving permission from the family. Visitors You cannot invite friends over unless this has been formerly agreed upon with your hosts. Students are responsible for the conduct of guests or visitors (including family, friends, and relatives), who must also respect and comply with all rules and regulations while in the homestay. Any breach by any such guest or visitor shall be deemed a breach by the student. Linens The household will provide you with clean sheets and towels. Laundry The family is not responsible for cleaning the student s personal laundry and is not required to allow the use of a washing machine. Shower You can take a shower (with hot water) once every day. Room The room given to you will be the one that the household has designated for you. You will be responsible for taking care of your own room and keeping your room neat and orderly. Property Damage You are responsible for the repair or replacement of any household item you damage or destroy. Boston University declines all responsibility. Failure to settle these charges with your host family prior to departure from the program site will result in appropriate charges being assessed to your Boston University student account. It may also result in your inability to register for courses, an inability to have grades assigned, transcript holds, and delays in processing financial aid awards for future semesters. In addition, each student is encouraged to review his or her family s personal property insurance coverage. Boston University and any of its
Homestay Agreement, Page 2 officers, agents, or representatives shall not be liable directly or indirectly for loss and/or damage to personal property by fire, theft, or other cause. Phone Phone privileges are to be worked out in an arranged agreement with the household. Boston University will not be responsible in this matter. In general, you can expect to be allowed to make one brief local call per day. You should plan to use your calling card if you are allowed to make outgoing calls. You are allowed to receive calls; however, respect the sleeping hours of your hosts. Students are not able to use the host family phone to make calls to mobile phones. Conflicts If you wish to leave your household for personal reasons, you must assume responsibility for making this known one week in advance in order to maintain good relations between Boston University and the Peruvian households. The University reserves the right to dismiss any student from the program for failure to maintain a satisfactory academic record or acceptable personal behavior, or for other reasons of health, safety, or welfare of the University community. STATEMENT BY RESIDENT I have read the preceding material and agree to abide by the stipulations and conditions presented within it. I agree that failure to abide these conditions renders me liable to be returned by the Resident Director to my home campus at my own expense and/or to other University disciplinary actions.
