3 Realising Our Potential Carers Strategy 2015/2017 Patient safety and clinical outcomes UHSM recognises the beneficial impact that carers have on patient safety and clinical outcomes. We will identify carers details when a patient is admitted to improve communication. We will accurately document carer engagement within 72 hours Our strategy focuses on the following three key areas: of the cared persons admission. We will audit the recording and identification and engagement of carers within the medical and nursing records. We will improve patient and carer knowledge of the trust complaint process and how to raise a concern. We will ensure that complaints, incidents, investigations and lessons learnt are shared with carers. We will provide staff training on how to identify a carer, safeguarding, Information sharing and confidentiality in relation to carer Involvement. Carer & Staff engagement We will develop carer s awareness training across all areas of the trust and include an overview at staff induction for new starters. We will ensure that both staff and carers feel equipped and skilled to work in partnership together. We will identify a carer lead in each Division to take forward the carers agenda. We will develop the role of the Carers Champion and provide training so that they can disseminate information to both carers and staff. We will canvas carers views on what type of forum they would Prefer and develop that in partnership. We will develop the Trust internet so that there is specific Information available for carers. We will offer bereaved carers access to our memorial services and bereavement support afternoons. To be one of the top 10 NHS Providers Recruitment in the country Retention f Patient and Carer Experience We will recognise the importance of carers expert knowledge when caring for patients. We will support and promote the Annual Carers week. We will ensure that information leaflets for carers are available within all areas and also improve signposting resources. We will ensure that carers have unrestricted visiting when they are providing support to patients. We will ensure that carers have access to welcoming identified spaces within the trust. We will ensure that carers have access to overnight accommodation when available within the trust. We will utilise carers stories as well as patient stories. Value for money We recognise the National economic value and contribution that carers make. We will prioritise the implementation of carer support systems so that carers can continue caring. We will work in partnership with voluntary and charity organisations to increase access to carer support services. We will ensure that carers have subsidised parking costs.

4 Carers Strategy The objective of the strategy is to support the organisation realise its potential as top 10 in respect of Patient Experience which includes patients carers. This forms part of the Trust s Nursing and Midwifery Strategy and the Dementia Strategy. The Patient Safety and Clinical Outcomes aspect of the strategy will help to: Identify carers when a patient is admitted to hospital Accurately document carer engagement within 72 hours of the patients admission. Plan to audit the recording and identification and engagement of carers within the medical and nursing records. Ensure that patients and carers have an awareness of how to complain or raise a concern. Ensure that lessons learned from complaints and incidents are shared with patients and carers Ensure that training is provided to staff on all aspects pertaining to carers. The Carer and Staff Engagement aspect of the strategy will help to: Develop carer s awareness across all areas of the organisation. Ensure that staff and carers feel equipped and skilled to work in partnership together. Identify a carer lead in each Division to sit within the strategy working party. Develop the role of the Carers Champion. Deliver a carers forum, the format of which will be dictated by canvasing carers views. Develop the trust internet with specific information for carers. Offer bereaved carers access to our memorial services and bereavement support afternoons.

5 Carers Strategy The Patient and Carer Experience aspect of the strategy will help to: Recognise the importance of carers expert knowledge when caring for patients. Support and promote annual carers week. Ensure that information and signposting resources are available in all areas for carers. Ensure that carers have unrestricted visiting when providing support to patients. Ensure that carers have access to welcoming identified spaces within the trust. Ensure that carers have access to any available overnight accommodation. Ensure that carers stories are also utilised as part of the patient story across the organisation. The Value for Money aspect of the strategy will help to: Recognise the National economic value and contribution that carers make. Ensure that we prioritise the implementation of carer support systems so that carers can continue to care. Ensure that we work in partnership with voluntary and charity organisations to increase access to carer support services. Ensure that carers have subsidised parking costs..

6 The Importance of Carers positive patient and carer experience will affect reputation of UHSM. Why this is a strategic priority UHSM recognises that carers have a beneficial impact on patient experience, patient safety, clinical outcomes and the National contribution regarding value for money. What we are going to do Promote the Trust vision and values, to spread the message that carers. are a valued member of our team. Promote the benefits offered to carers by utilising positive patient and carers stories to all internal teams and to local press. Enhance citywide links with external carers partners to promote our willingness for partnership engagement and working.. Encourage our staff to recognise the value of carers and the contribution that they make. We will recognise that some of our own staff may be carers, so we will ensure that appropriate support is offered to those individuals. Effectively use social media to communicate our offering through Twitter, LinkedIn. What outcome we are expecting Raised awareness and I the recognition of the importance of carers by all staff at UHSM. Improved Patient Experience metrics. Improved staff experience by those staff who are carers.

7 Implementation, Monitoring and Review The strategy will be delivered through the development of a detailed implementation plan. The plan will be lead by the Carers Working Group. Membership of the group will include the Divisional Carers Leads, A Dementia Team Representative, the Quality Matron, the Patient Experience Matron and members of relevant Carers groups such as Carers UK. The implementation plan will be divided into work streams with milestones, deliverables, key performance indicators and leads assigned. Short, medium and long term priorities and actions will be identified for the two year period to 2015/17. This will be lead by a Carers Working Group. The implementation plan will detail the resources required. Where additional resources are required, a business case will be prepared for BDISC review in line with the Trust governance structure. Implementation of the strategy and progress against the anticipated outcomes will be monitored through the Professional Forum and reported into Board. This strategy will be kept under review, in light of any significant impacts of large scale strategic developments. Work is already underway to progress a citywide Carers Strategy with a number of acute providers including input from citywide CCG s. the basis for this strategy will be the UHSM work.
