2 Aim of Telehealth To support people with long term conditions to develop confidence to manage their own health, improve health outcomes and reduce incidences of exacerbations, GP attendances and hospital admissions. Improve Patients Quality of Life The department of health is encouraging the use of technology as a tool to support the more traditional approach to health care. It does not replace clinical visits when needed, but provides clinical teams with information to make informed decisions about their patients.

3 2010 Telehealth Pilot Started in Somerset Timeline 2012 GP Accreditation begins and CCS launched 2010 Roll out to all Community Matrons in Somerset 2013 Employ Full-time Telehealth Supervisor 2011 Procurement of 425 more kits 2014 SWCSU win contract to run the Telehealth scheme on behalf of Somerset CCG

4 Pilot started in Tunstall kits Pilot Stage Focusing on COPD and Heart Failure only Referrals from 7 Community Matrons

5 Telehealth Technical Support Team Telephone support Home Technical Support visits Equipment Installation and Education Storage Not medically trained

6 Procurement 425 further kits from SAFE patient systems Getting ready for wider roll-out Equipment to fit the requirements of new pilots
