6 SESSION SUMMARIES Healthcare Litigation and Malpractice Cases regarding Credentialing Eric Tostrud, JD This session will describe current trends in healthcare litigation, with special attention to malpractice cases implicating negligent or faulty credentialing. We will explain how trends in these areas hold the potential to impact physicians and medical staff. We will offer suggestions for anticipating and addressing these issues effectively and efficiently. High Risk Credentialing: How to treat Temporary Privileges and Locum Tenens with Respect They Deserve Granting temporary and locum tenens privileges is clinically and legally very high risk. This session will define the terms, identify regulatory and accreditation requirements, and provide solutions to mitigate risk and minimize the extensive use of resources. Learning Objectives: Define the regulatory and accreditation requirements for temporary and locum tenens privileges Describe the differences between temporary privileges and the use of locum tenens Implement solutions to mitigate risk and minimize the extensive use of resources The Joint Commission Beginners Laurel McCourt, MD This session will look at the medical staff chapter with the objective of explaining the survey process to those who maybe new in their role as medical staff specialists. Although the information applies to all surveys, the context will be in a more basic format. There will be ample time for questions and answers. National Committee for Quality Assurance Advanced Gerald Stewart, MPH The presentation explores common issues in meeting NCQA credentialing standards and describes potential problems areas in the credentialing process. The Joint Commission Advanced Laurel McCourt, MD This session will be an update for medical staff specialists who are veterans of the survey process. There will be a discussion of the most commonly scored standards within the medical staff chapter and the role of the medical staff specialists before, during, and after the survey. There will be ample time for questions and answers. National Committee for Quality Assurance Beginners Gerald Stewart The presentation will review the NCQA Credentialing standards and identify appropriate credentialing and recredentialing documentation and operational procedures required to comply with NCQA accreditation standards. National Practitioner Data Bank Update David W. Asp, JD This presentation will provide information on the National Practitioner Data Bank (NPBD), including historical background on the NPBD, recent concern regarding the NPBD s privacy protections, and the merger of information between the Healthcare Integrity and Protection Data Bank and the NPDB following the Affordable Care Act. The presentation will include reporting scenarios and participants will have the opportunity to ask questions following the presentation.

7 Credentialing Game - Sally Pelletier, CPMSM, CPSC This fun and interactive session will lead participants through the never ending maze of healthcare acronyms utilizing a team based approach. Networking Questions and Answer Panel This session will provide an opportunity for many questions to be answered relative to credentialing and provider enrollment. The participants will have the opportunity to learn from representatives from hospitals, health plans, and other organizational representatives with a vast knowledge of credentialing and provider enrollment to answer any questions presented in this session. Finding Your MOJO: The ABC s of Living in Abundance, Balance and Creativity Theresa Rose What is Mojo? It s that fire, that energy, that zest that makes life juicy! If you ve got your mojo working, you are on top of the world: energetic, creative and fulfilled. If you ve lost it you may have to look up to see the bottom: depressed, tired, overwhelmed, and unable to accomplish much of anything. In Finding Your Mojo, Theresa Rose describes the extraordinary impact mojo has on our productivity and health, and she humorously shares some of her most insightful (and often embarrassing) life lessons. NAMSS PASS and Update on CPCS/CPMSM Certification Christina Cabral, NAMSS Director at Large MN Dept of Health Gayle Lantto, BSW This presentation will cover MDH's role in surveying health care facilities and how MDH works with the Centers for Medicare and Medicaid Services (CMS) and State agencies. Survey processes for the various provider types will be reviewed, such as conducting observations, interviews, and document review, as well as survey timelines, follow up, and remedies. Physician & PA Licensure Jeanne Hoffman, MBA Overview of Board and initial licensure and renewal process for physicians and physician assistants. Physician assistant licensure history, Notice of Intent to Practice and Delegation Agreement. Advanced Practice NP Licensure Shirley Brekken This session will provide an overview of the Minnesota statutory provisions for Advanced Practice Registered Nurses (APRNs) and descriptions of the roles for each of the categories: clinical nurse specialists (CNS): nurse anesthetist (CRNA); nurse midwife (CNM); and nurse practitioner (CNP). Topics will include requirements for national certification, DEA verification, collaborative management, and written prescribing agreements.

8 How to Run Productive Meetings Using Roberts Rules of Order Dan Janal Most organization waste time in meetings. Reports show that meetings are frequently unproductive. However, if a meeting is run by a set of rules and procedure, meetings can be product and agreements can be reached quickly. For more than 100 years, Robert Rules of Order have been the Gold Standard for running meetings. This session will show you how to run efficient meeting using Roberts Rules of Order. 9 SPEAKER BIO S Eric Tostrud, JD is a partner with Lockridge Grindal Nauen in Minneapolis, where he chairs the firm s health care litigation department. He is a 1990 summa cum laude graduate of William Mitchell College of Law. Immediately following law school, he clerked for United States District Court Judge Edward J. Devitt and then for United States Circuit Court of Appeals Judge George E. MacKinnon in Washington, D.C. Mr. Tostrud frequently represents physicians and medical staffs. In addition to a busy law practice, Mr. Tostrud also teaches frequently as an adjunct professor at both the University of Minnesota Law School and William Mitchell College of Law. Sally Pelletier, CPMSM, CPCS is Director, Credentialing Services, with The Greeley Company, a division of HCPro, Inc., in Danvers, MA. She brings more than 20 years of credentialing and privileging experience to her work with medical staff leaders and medical services professionals across the nation. Sally advises clients in the areas of accreditation, regulatory compliance, credentialing and privileging assessment and redesign, medical staff services department assessments, and leadership and development training for medical services professionals. Sally began her career in 1992 as the Medical Staff Coordinator at The Memorial Hospital in North Conway, NH. She has served as Secretary and as the Northeast region representative on the Board of Directors for the National Association Medical Staff Services (NAMSS). Her other leadership roles for NAMSS included chair of the Governance, Management, and Manpower Committee; Chair of the Bylaws Committee; and Chair of the Credentialing Elements Task Force, as well as serving as a member of the Conference Committee and the Credentialing Consensus Alliance. In addition, she served as president of the New Hampshire Association Medical Staff Services, from which she received the 2008 Excellence in Medical Staff Services Award. Sally presents at state and national seminars on a variety of topics related to accreditation standards and survey preparation, medical staff leadership training, credentialing and privileging, and physician competency management. He holds a Bachelor of Science in Biology and a Master of Healthcare Services Administration from the University of South Carolina. Gerald has worked with health care organizations, government agencies, providers and educational institutions, and during his journey progressed from literally working in a sewer to serving as an executive officer within corporate America. Gerald joined NCQA in March 2001 and is currently the Director of Policy Accreditation. His primary responsibility is to provide policy clarification support to customers and to improve the NCQA Standard and Guidelines, though he will not take responsibility for covering for Big Yellow. Gerald is the son of a southern sharecropper and is one 16 children. His life is a continuously unspooling story, and each new chapter is more interesting that the last. He is quiet and unassuming man, whose mantra is, No matter where I am, help me to make a positive difference in someone s life, health or wellbeing.

10 Dr. Laurel McCourt as a surveyor at The Joint Commission, Dr. McCourt is trained under the Comprehensive Accreditation Manual for Hospitals and the Accreditation Manual for Office Based Surgery. She has been a surveyor since Dr. McCourt is an honorary member of the Medina General Hospital Medical Staff in Medina, Ohio, where she served as the Physician Quality Liaison from April 2007 to January For 14 years preceding this, Dr. McCourt served as the Medical Director of Brunswick Medical Care Center, a satellite facility of Medina General Hospital. Dr. McCourt is board certified in Family Medicine and Urgent Care Medicine. She received her MD degree from Northeastern Ohio Universities College of Medicine in She completed her residency at Aultman Hospital in Canton, Ohio. David W. Asp, JD advises healthcare providers in litigation, regulatory, and compliance matters under state and federal law. He has particular experience defending healthcare providers in matters involving allegations of fraud, including claims under civil RICO and anti-kickback statutes. He also has represented physicians in disputes with third-party payer and claims involving peer-review reporting. Before joining Lockridge Grindal Nauen, P.L.L.P., Mr. Asp was a Presidential Management Fellow at the U.S. Department of State where he worked as part of an office tasked with negotiating information-sharing agreements with foreign governments. He is also a Policy Fellow at the University of Minnesota s Humphrey School of Public Affairs. Networking Question and Answer Panel Hospital Expert United Hospital Adi McCarthy, MPNA, CPCS, CPMSM began my credentialing career in 1999 at Medical Credentialing Service of Minnesota, the CVO for the state. Many things have changed over the years. We used to receive a disk from the ABMS with all of the providers and their board certifications. Today, that is all done electronically. Our main method of requesting information was through snail mail. I can t remember the last time I sent something in the mail, or even had to buy stamps. I ve watched credentialing change over the years and often ask myself why I still do this job. It s because there is a sense of accomplishment when you know disaster is averted by the work you do. When MCSM closed its doors, I transitioned to United Hospital and have been there ever since. My area of expertise ranges from initial to reappointment to just about anything that could happen in a medical staff office. I received my CPCS certification in 2005, my CPMSM in 2008 and in 2012 received my Master s degree in Public and Nonprofit Administration. MN Credentialing Collaborative Expert Dolly Schmidt, MBA is the Executive Director of the Minnesota Credentialing Collaborative, collaboration with MMA, MHA and MCHP centered on simplifying the credentialing process. Schmidt has 20 plus years of experience in health care administration, as a VP with the predecessor to Allina (Health One) and as the Senior Executive for the Surgical Groups at the University of Minnesota. She has an MBA from the University of Minnesota and is now experiencing the pain of funding college tuition for her kids.

11 Health Plan Expert UCare Wendy Kulla, CPCS has been in the Credentialing field for over 15 years. Over the past 12 years, she has been employed at UCare, a managed care organization located in Minneapolis, Minnesota that provides health care services for the Minnesota Public Programs population, as well as Medicare. She is currently the UCare Provider Credentialing and Demographics Manager, which has broadened her scope of management from not only credentialing but also to the provider set up process. She is CPCS Certified and has also been an active member of the MAMMS for eleven years. She has been on the MAMMS board for the past six years and served as vice-president, president and is currently past-president. Wendy is the proud mother of a daughter in college at University of Minnesota and lives in Andover, Minnesota. Health Plan Expert Preferred One Donna Larson has been with PreferredOne for 18 years and currently holds the Director of Credentialing position. She is a member of MAMSS and NAMSS. PreferredOne is located in Golden Valley, Minnesota and services enrollees in multiple states. Clinic Expert/Consultant MJ Hamborg Consulting Services Marie Hamborg, LPN, CPCS, CPMSM is the founder and current CEO of MJ Hamborg Consulting Services for Medical Staff Services/Credentialing. She has over 22 years experience in the health care field. She is a current member of MAMSS and NAMSS as well as the President for MAMSS. Marie has overall experience in credentialing, privileging, develop and revise credentialing policies and procedures, quality improvement activities, compliance with the National Committee on Quality Assurance, the Joint Commission, Healthcare Facilities Accreditation Program, the Minnesota Department of Health, and the Centers for Medicare and Medicaid Services. Clinic Expert ACMC Shelly Berg is the Credentialing Coordinator for Affiliated Community Medical Centers (ACMC), in Willmar, MN. ACMC is a multi-specialty healthcare network with affiliate locations in Benson, Granite Falls, Hancock, Litchfield, Marshall, New London, Redwood Falls, and Willmar. Shelly has been in the healthcare field for 15 years. In this position Shelly is in charge of credentialing 109 physicians and 52 allied health providers, and works with multiple hospitals and health plans. Along with credentialing, Shelly is also the Administrative Secretary to the CEO, Medical Director and Administrator of ACMC. Her position also includes helping to plan and implement annual educational symposiums which reach multiple clinic and hospital physicians. Shelly has been attending the MAMSS conferences since She was the Southern Regional Representative from January 2007 January 2011 and the MAMSS Vice President from January 2011-Present. Christina Cabral, NAMSS Director at Large

12 Theresa Rose is a master inspirational performer that electrifies audiences from head to toe. With her authentic, hilarious stories and priceless nuggets of wisdom, she brings a profound message of personal empowerment, overcoming adversity and downright juicy living! Her mission is to help women move back into a state of balance and joy, and her partner on that mission is her beloved hula hoop. Theresa's superhero alter-ego is HoopWoman, a rambunctious firecracker who believes that we have the capacity to blow the doors off our potential and embrace our true awesomeness. Theresa's one-of-a-kind, visually-stunning, laugh-'til-you-almost-pee programs are a yummy combination of storytelling, audience playtime, heartfelt inspiration, and real-life advice on how to fill our tanks. Of course, there is also serious hooping involved. In addition to being a hardcore hoop dancer, Theresa is also a nationally-acclaimed speaker and award-winning author. In 2012, she was absolutely giddy at being named a top five finalist in the "So You Think You Can Speak?" competition at the Annual Convention of the National Speaker's Association. Her giddiness was taken to a whole new level when she was chosen to deliver a TED Talk on "The Hoop Revolution" at the TED Sarasota event on 12/12/12. She is also incredibly proud to be a member of the prestigious National Speakers Association and serve as the Director of the Institute of Professional Speaker Development. Theresa not only lights up a room onstage, she also brings some serious mojo to the page. Her first book, Opening the Kimono: A Woman's Intimate Journey Through Life's Biggest Challenges, is what Theresa would describe as "a PG-13 Erma Bombeck meets Sex and the City." Opening the Kimono won not one, but two fancy-pants literary awards including the Royal Palm Literary Award from the Florida Writers Association and the Living Now Book Award from Independent Publisher. Theresa's fresh, unorthodox articles on personal empowerment, spirituality and motivation have been published in dozens of magazines and journals, and she penned a sassy monthly column called "Sex and the Suburbs" for Creative Loafing newspaper. Seen as an expert on personal power, Theresa has been interviewed by CNN.com, NBC, Glamour, Fitness, Good Housekeeping, Women's Day and many other publications. Her formal education includes earning a bachelor's degree in Business Management from Eckerd College, a ministerial ordination from the Center for Sacred Studies, and a certificate from massage therapy school. (She signed up for the last one because she thought it sounded really cool to give and receive massages every day. She was right!) Her stories tap into her current life as a speaker, writer, spiritual director, wife and mom, as well as her past experiences as a massage therapist, healing center owner, corporate senior manager and VP, overachiever, overeater, administrative assistant, temp, and drive-thru girl at Mickey D's Theresa's natural habitat is the stage, and her dream of making people laugh started as a wee child. Many a night was spent in the early '70s with Theresa lining her stuffed animals against the closet doors so they could enjoy yet another riveting performance of singing, dancing and telling jokes into her pink hairbrush. The hairbrush has since been replaced with a microphone and a hula hoop, but her child-like passion for performance and inspiration remain. Even though Theresa is proud of her accomplishments as a professional speaker and author, she is most proud of the opportunity she has been given to serve as a cosmic alarm clock, helping people across the country wake up to their own magnificence. After living all over the map, Theresa finally settled back into her beautiful hometown of Minneapolis. She is grateful every day for the life she enjoys with Michael, her handsome-as-heck husband, Emma, her precocious but lovable middle-school daughter, and Duncan, her take-your-breath-away-cuz-he's-so-completely-adorable King Charles Spaniel. Gayle Lantto, BSW is a licensed social worker and unit supervisor with the Minnesota Department of Health. She has been employed by MDH for 29 years, 16 years in a quality assurance program, and then transferred to the Licensing and Certification program where she was a surveyor for nine years and a unit supervisor for the last four years. She supervises a metro survey team of healthcare professionals including nurses, a dietitian, and an occupational therapist. Additionally, she oversees the laboratory surveyors and support staff for the Clinical Laboratory Improvement Amendment (CLIA) program. She has been federally trained in nursing home, intermediate care facilities for the intellectually disabled, hospital, and transplant hospital surveys.

13 Jeanne Hoffman, MBA has since 1988 served as Licensure Unit Supervisor for the Minnesota Board of Medical Practice, the state agency that licenses physicians, physician assistants, acupuncturists, athletic trainers, respiratory therapists, naturopathic physicians and traditional midwives. Currently, there are nearly 27,000 practitioners within 7 different professions licensed by this Board. She was an early adopter of the FSMB s Uniform Application and also initiated a re-entry program for physicians and allied health professionals in Minnesota. She has participated in the National License Portability Project, Wisconsin Portability Project, Uniform Application Task Force, and the North Carolina Board Evaluation Task Force. Her academic background includes a Bachelor of Business Administration from Mankato State College and Master of Business Administration from St. Thomas University. Prior to coming to the Minnesota Board, Ms. Hoffman worked in the private sector in marketing research and analysis. Shirley Brekken serves as the chief executive officer for the Minnesota Board of Nursing. Her responsibilities are focused on activities involving the enforcement of state laws and rules related to the practice of nursing and include organizational management, facilitation of Board functioning and public relations. Shirley has been actively involved with the National Council of State Boards of Nursing (NCSBN) having served on several national committees and had felt most rewarded for sharing in the work to move nurse licensure examination from paper and pencil to computer adaptive, the development of a national nurse licensure data base, and a performance measurement program for nursing regulation. Most recently her NCSBN work has been focused on leadership development and governance, and she serves as vice-president of the Board of Directors. Shirley is a member of Sigma Theta Tau, national nursing honor society. She has received numerous state and national awards; most recently the Minnesota Organization of Leaders in Nursing Star and Moline awards and the R Louise McManus award, the most prestigious award given for sustained and significant contributions through the highest commitment and dedication to the purposes of the National Council and nursing regulation.
