4 Goal: NYSPFP Health Care Disparities Initiative Goal and Approach To assist hospitals in identifying potential disparities in care delivery through standardized capture of data and information. To better address these disparities and reduce harm. Approach: NYSPFP will provide hospitals with tools, education and training opportunities on: Best practices to improve the capture of standard Race, Ethnicity and Language (REaL) data, Use of this data to inform patient-centered care, and Targeted interventions to reduce disparities. 4

5 Hospital Survey Overview In order to better understand existing practices for collecting REaL data, NYSPFP sent out a hospital Health Care Disparities Survey. The survey covered the following topics: Processes and systems for collecting data, Uses of existing data, Education and training programs for staff, and How NYSPFP can best support hospitals in improving their ability to collect REaL data. The survey also assessed hospitals collection of sexual orientation and gender identity (SOGI) data and disability status. Hospitals completed the survey between April and June NYSPFP received 46 survey responses. 5

16 NYSPFP Race, Ethnicity and Language (REaL) Data Report The REaL Data Report: o o o o Provides information on race and ethnicity reporting at your hospital. Compares your reported data with other NYSPFP participating hospitals. Compares race and ethnicity data that your hospital has reported via claims with census data from within the geographical area where patients treated at your hospital live. Provides potential areas for improvement in data quality, and information on the communities your hospital draws from.
