2 What is Disciple Now? An in-town weekend retreat for 7 th -12 th graders designed to take students to a deeper level in their relationship with Christ, connect them with local mission ministries, and help them enjoy the fellowship of other believers. Disciple Now begins on Friday evening and continues until the end of Sunday morning worship services. Students stay in host homes hosted by our church members and they travel throughout the weekend via church-provided vehicles. When is Disciple Now? January 16-18, 2015 How much is Disciple Now? Regular Registration: $65. Regular Registration starts on November 1, 2014 and ends on December 31, We ask that when you register you pay a $20 deposit. This deposit is non-refundable. The deadline to sign up for Regular Registration is Wednesday, December 31 st. Please make all checks payable to First Baptist Hurst. FULL PAYMENT IS DUE BY SUNDAY JANUARY 11th. If you have any questions regarding payment or balances you can contact Ronda Allison, Ronda@firsthurst.com. Late Registration: $85. Late Registration starts on January 1, 2015 and ends January 11, Please note that if you register late, we cannot guarantee that a t-shirt will be available. We order the t-shirts prior to Late Registration based on the number of students that signed up during Early Registration and Regular Registration. You can register by filling out the envelope attached and dropping it in our black mail box located at the Student Ministry Welcome Desk. This will also serve as your payment envelope. Please include payments in these. 2. You can register online at: firsthurst.com/dnow Medical & Surgical Waiver You must have a Medical & Surgical Waiver on file to attend Disciple Now weekend. It MUST be notarized and it MUST have a parent signature. If you are over the age of 18 you do not need a parent signature, but it still must be notarized. You can find these at the Student Ministry Welcome Desk & Cart. They can also be downloaded from the website (listed above). Time Away Card If for any reason you need to be away during Disciple Now weekend you will need to fill out a Time Away Card. If MUST have a parent signature. You can turn this in with your registration or turn it in to your host home or group leader on Friday January 16. Disciple Now Covenant Please read over the Disciple Now covenant and turn in with a parent & teen signature. This will serve as a means of accountability for the weekend. You must have one on file to attend. Early Check-in Check-in will be on Wednesday. You will be given important material for the weekend including a map to the host home you will be staying at for the weekend. You may also turn in final payments, medical waivers and covenants. Students who complete early check in will receive a fast pass for registration on Friday evening, speeding up the process for everyone! Questions or Concerns? 4 WHAT TO BRING TO DISCIPLE NOW o Medical Release Signed and notarized You MUST have one on file to attend. o Disciple Now Covenant Signed by your parent You MUST have one on file to attend. o Time Away Card If you need to be away at any time during the weekend. o Bible & pen o Sleeping bag or bed roll & pillow o Towel & washcloth (your host home will not provide these) o Toiletries o School type clothing o Jacket, sweatshirt or fleece o Tennis shoes o Pajamas o 2 liter drink & one snack to share o Extra spending money ($10-$15 for possible extra activities during free time) PLEASE DO NOT BRING o Alcohol, tobacco, firearms, fireworks or knives o Supplies to play pranks o A bad attitude o Video game systems o Music Players (ipods, MP3 s, etc )

5 DISCIPLE NOW COVENANT Pranks and practical jokes detract from the purpose of the weekend and WILL NOT be tolerated. Fireworks, firearms, knives, any form of tobacco, alcohol, drugs, game systems, MP3 players and radios have no place at Disciple Now and need to be left at home. Be aware that you are responsible for any damages that you may cause. You may not leave Disciple Now at any time for any reason without a time-away card signed and submitted by your parent or guardian. At no time do guys or girls need to be in each other s areas, or spending time alone together. You are expected to attend and participate in all scheduled services and activities. If you are too sick to participate in Disciple Now services and activities, you are too sick to remain at Disciple Now. ALL CLOTHING SHOULD REFLECT MODESTY AND DISCRETION! Extremely short shorts/skirts/dresses, sheer clothing, and shirts with inappropriate messages are not permitted at Disciple Now. Always wear your name tag. Your signature indicates that you and your parents know that if you fail to use good judgment and common sense in following the rules listed above, you will be dismissed from Disciple Now and returned home with no reimbursement of your money. 6 Medical and Surgical Waiver First Baptist Church of Hurst 1801 Norwood Dr Hurst, TX INSTRUCTIONS: The 2015 Medical and Surgical Waiver will apply to all student events, trips and projects from January 1, 2015 through December 31, In the event that a participant needs medical attention, this waiver will provide an adequate and current record of the student s medical information, and parental permission for First Baptist Church of Hurst as well as the hospital. Please be thorough with each answer. It is the responsibility of the parent or guardian to keep this information current. After completing the form, have it notarized in the space provided, as this is a requirement at many hospitals. 7 WAIVER: " TO BE FILLED OUT BY PARENTS OR LEGAL GUARDIANS OF PARTICIPANTS UNDER 18 YEARS OF AGE I, the parent and/or legal guardian of, a minor, hereby acknowledge that said minor is presently under my care, custody and control. I hereby give said minor permission to participate in any and all activities at First Baptist Church of Hurst, Texas, for which he/she, with my approval, registers to participate. I further expressly grant permission for said minor to participate in all activities while an active participant on trips and church events. If in the event there arises an emergency necessitating medical or surgical attention, I hereby consent and give my permission to the First Baptist Church of Hurst, Texas staff, its representatives, the sponsors, or any attending physicians to make such decisions and to perform such medical treatments and/or surgery upon said minor which may, in their sole discretion, be necessary and proper under the circumstances. I, the undersigned parent and/or legal guardian of said minor, do hereby release, acquit, discharge and covenant to indemnify and hold harmless First Baptist Church of Hurst, Texas, its staff and/or representatives, the sponsors, and/or any attending physician from any and all claims, damages, actions, and causes of action of any nature, including but not limited to negligence, damages, and liabilities, arising out of the treatment of any sickness and/or accident and any related risks and dangers thereto, from financial responsibility for all medical treatment provided during the attendance of any student ministry events. I also assume responsibility for providing and paying for return transportation of said minor from the event location should it be necessary for disciplinary reasons.
