3 Overview An interactive conference for healthcare professionals to explore ways to improve their patients care transitions. Nationally recognized speakers will address demonstration projects, innovative initiatives and practical solutions to decrease readmissions to acute care and improve adherence to medications and lifestyle modification. Vendor sessions will allow participants to learn about resources available for their patients that can facilitate care transitions and improve health outcomes. Tuition $75 (Includes Continental Breakfast and Lunch) Intended Audience Case Mangers, Social Workers, Discharge Planners, Nurses, Administrators and other Healthcare Professionals Objectives Upon completion of the program, participants will be able to: 1. Discuss how emerging healthcare delivery and payment models such as medical homes, accountable care organizations, and value based purchasing may impact delivery of services and outcomes for their patients. 2. Describe demonstration projects to improve care transitions, with specific attention to systems and processes that translate to practice within their hospitals and communities. 3. Review basic principles of prevention, rehabilitation, and chronic disease management for patients and diabetes, cardiovascular and pulmonary disease and risk factors for these diseases. 4. Examine how diabetic self-management training, cardiovascular and pulmonary rehabilitation fit into the care continuum and how they can improve patient outcomes. 5. Explore opportunities to utilize new and existing services in the Hudson Valley and New York City to improve the health of their patients and to decrease hospital readmissions. 6. Analyze barriers to care transitions, including those related to participation in prevention, rehabilitation, and chronic disease management. 7. Determine how to help their patients overcome these barriers to care transitions. Continuing Education Credits Attendees will be eligible to earn up to 5.5 contact hours (0.55 CEUs).
