1 THE ILLINOIS DEPARTMENT ON AGING PRESENTS THE: 4th Annual CCP Supervisors Conference Crowne Plaza: Springfield, IL May 5-6, 2015 Bringing together Community Care Program supervisors to discuss program issues.
2 4th Annual CCP Supervisors Conference May 5-6, 2015 The Illinois Department on Aging is pleased to offer the 4th Annual CCP Supervisors Conference. The goal of this conference is to bring together supervisors from within the Community Care Program, including supervisors from in home agencies, adult day service, care coordination units, managed care organizations and emergency home response to address program issues affecting every agency. The conference is designed to help professionals meet the ever-changing needs of participants and to increase knowledge and understanding of the daily tasks demanding attention. As you look at the various sessions being offered at this conference, you will see that again many changes are on the horizon that directly influence our service delivery system. This conference will provide first hand updates on program changes and introduce best practice sessions to assist you in serving the older adults of Illinois. CONFERENCE REGISTRATION: Registration for this conference is online only, regardless of method of payment. To register, click on the following link: CCP Supervisors Conference. If you have problems with the registration form, send to UIS IDOA Conferences and Training. FEE: The conference registration fee for the 2015 CCP Supervisors Conference is $50.00 per person. PAYMENT: We encourage everyone, especially groups attending the conference, to prepay, either by check or credit card. Prepaying will ensure a more expedient on-site check-in process. 4 Certification Procedures Continuing Education Credits and CCP Care Coordinator Recertification Certification Request Submission Procedures: One form, the Certificate Request Form will be used to request Continuing Education credit and CCP Care Coordinator Recertification. The Certificate Request Form and a Certificate of Attendance will be located in the conference program. A total of 11 continuing education credits will be awarded for this conference. (Tuesday May 5, 2015: 5, Wednesday May 6, 2015: 6) How to submit the Certificate Request Form: You have two (2) options to submit the form: 1) Complete the form at the conclusion of the conference and give the form to an IDoA staff person at the conference registration desk. 5 Day 1: Tuesday May 5, 2015 Conference Agenda 9:30 10:00 Conference Registration 10:00 10:15 Welcome remarks from the Illinois Department on Aging and representatives from homecare, care coordination, adult day service and MCO (Diamond/Sapphire) 10:15 12:00 Jim Vanden Bosch, Founder and Executive Director of Terra Nova Films Tweaking the Meaning of Caregiving 12:00 12:45 Using clips from several videos, this presentation will explore the benefits of redefining what we have typically called caregiving especially when it applies to supporting persons who are living with dementia. The typical approaches of caregiving may not, we are beginning to discover, be the best way to engage the emotional sensitivities and many remaining abilities of persons who are living with dementia. With a basic shift in our perception, we can find ways to help persons living with dementia fulfill the deep human need for connection, self-expression, and well-being. Video clips will demonstrate how a truly person-centered approach with, for example, the use of music, art, and storytelling can open up new avenues of expression that allow persons with dementia to be vibrantly connected with others, their environment and themselves. 1:00-2:00 Family Dynamics in Ethnic Populations (Diamond/Sapphire) 2:15-3:15 Presented by: The Coalition of Limited English Speaking Elderly (Marta Pereyra and members of CLESE). This session will explore the various dynamics in senior ethnic populations and will also examine the issues of the ethnic elderly being at risk of self-neglect and elder mistreatment. 1) Collaboration between Adult Day Services and Community Agencies (Diamond Room) Presented by: Tom Shoemaker, MSW, LCSW OSF Home Care, Peoria, IL; Jackie Bowers, Director of Adult Day Service IPMR Senior Worlds, Peoria, IL; IADSA Board Member. This session will focus on how Adult Day Services work with other Community Care Providers and Community agencies to help maintain persons in their homes. This session will explore the benefits of collaboration for the physical, safety and well-being of each participant. Hospice, home health and emergency response systems are focused. 2) Working with CCP, ORS, and MCOs (Sapphire Room) Presented by: Jennifer Moore RN, Home Care Director Chicago Commons (Chicago IL) and Diane Drew, Executive Director for CHELP since Diane has served on CCPAC since 2003 and is a Certified Information & Referral Specialist for Aging (CIRS-A). This session will address how best to work with CCP, ORS and MCOs in today s environment and how to work together as a team for seniors.
6 3) Bed Bugs are Here to Stay What you Can Do (Plaza G) Presented by: Linn David Haramis, PhD, Program Manager Vector Control, Illinois Department of Public Health, Division of Environmental Health. Local agencies throughout U.S. have been overwhelmed by bed bug complaints during the last 7 years. Bed bugs can cause a variety of negative physical health, mental health, and economic consequences. The national problem is due to convergence of 3 human behaviors: lack of awareness of historical and biological link humans have with bed bugs; increased international travel; past over-reliance on pesticides. Learn how you can help your client and protect yourself when the client's home is infested with bed bugs. 3:15-3:30 3:30 4:30 1) Prevention and Preparation for Site emergencies in Adult Day Services (Diamond Room) Presented by: Keith Cunningham, Lieutenant, Champaign County Sheriff Office; Cindy Cunningham, ADS and In Home Services Consultant; IADSA Board Members. This session will focus on how ADS staff responds to on site-related and participant related emergencies. Discussion will include the concealed carry law and how centers can keep people from entering with their guns if they choose. OSHA regulations with be presented along with how to respond to hazardous spills. 2) Problem Solving 101 (Sapphire Room) Presented by: Cheryl Aguirre, the Director of the Home Services Program at Home Life Healthcare, Schaumburg, IL. Join us to learn the process of problem solving and gain experience running through the various challenges that a Home Care Supervisor may encounter from the common to the extraordinary. We will work through the stages of problem solving, come up with a plan of action, implement, and monitor! A Home Care Supervisor is not only a scheduler but also a matchmaker, disciplinarian, counselor, trainer, and customer service representative. Working as a Home Care Supervisor is one of the most stressful and emotionally challenging jobs in the field of Home Services if you do not have the tools for success. Problem Solving 101 is designed to give you one of the tools needed to succeed as a Home Care Supervisor. Through this program you will learn to go through the various steps and stages of problem solving in order to overcome barriers you may face in order to achieve set goals 3) Safety Tips for Home Visits--What you can do to protect yourself (Plaza G) Presented by: Dyle Stokes, Deputy Chief Administrative Services, City of Springfield Police Department. Care coordinators are all too familiar with the dog "that won't bite anybody" and with the occasional client who becomes angry and difficult to deal with. This session will provide tips on how to deal with these and other safety topics. When do you take a second person with you? Do you have an adequate set of procedures in your agency to cover such situations? Do you have communications equipment to call for help? How can your local police department help you? (Note: Deputy Chief Stokes will designate an officer for this presentation)
7 Day 2: Wednesday May 6, :00 10:00 a.m. Working Breakfast Each breakfast table will have a preset topic. You are welcome to sit at any table. Your breakfast assignment is, as a group, is to identify what is working and what issues need to be discussed regarding your topic. For every problem identified please also suggest a solution. At the end of breakfast, each table will submit their reports. Department staff will compile the reports and send out a document which will list the issues and provide clarification and or directions. (Ruby/Emerald) 10:00-3:00 p.m. Illinois Department on Aging Updates and Forum Moderated by Mary Gilman and Robin Morgan, Illinois Department on Aging (Diamond/Sapphire) Despite recent changes in state government affecting everything from administration to budget, the Illinois Department on Aging s mission remains the same. We are charged with serving and advocating for older Illinoisans and their caregivers by administering quality and culturally appropriate programs that promote and encourage independence, dignity and quality of life. As care supervisors, you serve an indispensable function of fulfilling this mission. Communication consistency and shared goals are the keys to successful quality inhome, case management, adult day and emergency home response services within every community in our state. Today is designed to share with the Aging network supervisors the Department s stance on initiatives such as Person Centered Planning, the Medicaid Waiver, DON scoring, Quality Assurance, Managed Care, Electronic Visit Verification, Money Follows the Person, Nursing Home Diversion, and more. Time allows for interaction and questions from supervisors to include your input and concerns in advancing best practice solutions and guidelines for each initiative. Basic tenets of case management, clarification of policies, and a look into what is to come will be included. Your participation is essential to a successful discussion. Come prepared to join in and be a part of this forum.
