The event itself is spectacular. Guests will be treated to Clay County s premier food and wine event with the top chefs from the best and most exclusive restaurants in Northeast Florida. Over the past 2 years, we have grown our programs for children and families in Clay County because of support from generous Corks and Forks sponsors. The annual Corks & Forks event benefits St. Vincent s Community Outreach Programs in Clay County. To date, Corks & Forks has raised more than $300,000 in net proceeds to help fund the vital programs St. Vincent s provides in the Clay County community. Some of these programs include St. Vincent s Mobile Health Outreach Ministry, ancillary testing for The Way Free Clinic, Momentum which addresses childhood obesity, and Faith Community Nursing. 2 St. Vincent s Medical Center Clay County ST. VINCENT S MEDICAL CENTER CLAY COUNTY To meet the needs of this population, the St. Vincent s Medical Center Clay County opened in 2013 to provide a number of outpatient services, community outreach programs and emergency care. Patients living in Clay County now have access to a full range of hospital based services; services that expand their reach each year. St. Vincent s Medical Center Clay County Expansion Since the new St. Vincent s Medical Center Clay County opened in October 2013, it has surpassed all expectations in terms of use, quality of care and patient satisfaction. The response from the community has been unbelievable. For example, the Emergency Room was built to see approximately 50 patients per day, but on average provides quality care to more than 100. The hospital opened with 64 beds, but consistently needed anywhere from 75 to 95 beds. So it was decided that expansion of the hospital and its services were needed in order to continue to provide quality care for Clay County residents. In 2016 the expansion added maternity and women s services, 30 inpatient beds, 13 treatment rooms in the Emergency Department and space to add 30 additional beds in the future. J. Wayne & Delores Barr Weaver Women & Infants Pavilion The new J. Wayne and Delores Barr Weaver Women & Infants Pavilion opened in August of The Family Birth Place offers critically needed healthcare services for expectant mothers in Clay County, in a healing and compassionate environment. The new three story tower includes 8 Labor & Delivery suites along with Anti-Partum suites. It is also a resource that offers comprehensive education and support to the entire family, such as childbirth education, sibling classes, safe-sitters classes that teach preteens and teens how to safely care for infants, lactation consults, and follow-up appointments for weight and jaundice checks. In its first year, 490 babies have been born at this hospital, and the number of babies born each month is only expected to grow. Registration Clay County patients can register using our Patient Self Check- in Kiosk. This technology allows patients to initiate their registration as opposed to waiting for someone else to begin the process. Our volunteers assist patients who need further instruction. Emergency Services The Emergency Room volume has grown dramatically since the opening of St. Vincent s Clay County in October of In fact, there were 43,000 emergency room visits at Clay in Fiscal Year Outpatient & Inpatient Surgical Services for Pediatrics and Adults In Fiscal Year 2017, St. Vincent s Clay County had a total of 3,121 inpatient and outpatient surgical procedures. That is an average of 60 per week! Additional services provided at St. Vincent s Clay County include Orthopedics, General Surgery, a Surgery Center with a kid-friendly design, Laboratory, Pharmacy, and Wellness and Community Education.
3 Medical Office Building The Medical Office Building adjacent to St. Vincent s Clay County Medical Center is fully occupied, bringing additional specialties to Clay County like the Wound Center, Sleep Center, ENT, OBGYN, General Surgery and Gastro Intestinal. Volunteer Services The hospital has attracted some of the very best volunteers in Northeast Florida. Last year 251 adult volunteers contributed 35, hours of service, while 58 teenage volunteers contributed 3, hours of service during the summer program. These remarkable people volunteered as greeters, way finders, worked the information desk, provided comfort to loved ones in the Operating Room s waiting rooms, helped vulnerable patients in the Emergency Department, and calmed the fears of the women in Mammography who were facing serious diagnoses. ST. VINCENT S CLAY COUNTY COMMUNITY OUTREACH St. Vincent s mission is to serve those who are most poor and vulnerable in our community. This is best demonstrated by meeting people where they are in order to provide free hospital based outreach where it is needed most. Below are some of our special community outreach programs that improve the lives of thousands. Mobile Health Outreach Ministry Since 1991, St. Vincent's award winning Mobile Health Outreach Ministry has brought healthcare to the doorsteps of those who are most in need of medical care in underserved areas of Clay County once a week. This program offers a complete doctor s office on wheels, staffed with a full-time physician, nurse, driver/emt and medical assistant. It provides primary health care, immunizations, flu season coverage, vision, & hearing screenings, blood pressure checks, hemoglobin screenings, referrals for specialty care, access to prescriptions, diagnoses and treatment of medical conditions to all men, women and children at no cost. Mobile Health also partners with the Clay County Health Department and We Care to increase medical access. Pediatric Mobile Health Outreach Ministry Since 2004, the Pediatric Mobile Health Outreach Ministry has provided children with free immunizations, school and sports physicals, and new backpacks filled with school supplies to arm them with tools for success as they head back to school. Last year 1,733 school physicals and 444 immunizations were provided. Sports physicals give underserved children the chance to play team sports and participate in after-school activities. Research shows participating in school sports and activities helps children become successful adults and more successful community members. Homeless Initiative In October 2015, St. Vincent s Mobile Health Outreach Ministry expanded its services when it partnered with the Clay County Health Department and Mercy Support Services, to help address the dire healthcare needs of the homeless. St. Vincent s targets the homeless twice a month in Clay County at locations they frequent. Thanks to these new partnerships and a generous grant from the Jessie Ball dupont Fund for this program expansion, St. Vincent s is reaching out to the homeless and providing them with compassionate, quality medical care, and any needed follow-up patient referrals to the Clay County Health Department.
4 Faith Community Nursing Since 1991, the program has connected St. Vincent s HealthCare, community resources and local church congregations to promote holistic health and wellness in our community. The program is available to all faith traditions. Faith Community Nursing provides free health screenings, health counseling, education literature, health workshops, annual four-day Foundation Course Training for RN s, and quarterly Morning of Renewal seminars. Presently, in Clay County, we have over a dozen active faith partners consisting of several denominations. Momentum Since 2015, St. Vincent s has offered this afterschool program in Clay County which addresses adolescent obesity through clinical, nutritional, behavioral, and exercise components. The program began at Wilkinson and Orange Park Junior High, helping change the lives of 128 adolescents in FY16. Momentum even won One Spark s juried award in the Health & Sciences category last year. The program was so successful in its first year, that the schools have incorporated Momentum s nutrition component into their physical education curriculum, expanding the program s reach to nearly 1,000 adolescents for Fiscal Year 2017! The Way Free Medical Clinic Since 2005, St. Vincent's has partnered with the Way Free Medical Clinic (WFMC) in Green Cove Springs to provide healthcare access to the underserved and uninsured in Clay County. St. Vincent's donates free laboratory work for these patients, including complete blood counts, comprehensive metabolic panels, and pap smears. Without St. Vincent s donations, the costs of these tests would have to be absorbed by WFMC, thus leading to an unsustainable financial burden for the clinic. In addition to donating laboratory services, St. Vincent s donates up to ten free surgeries a year for patients of the Way Free Medical Clinic. Diabetes Management Program St. Vincent s comprehensive, evidence-based disease management program helps patients embrace their diabetes in order to achieve long term health. These educational classes are offered in Clay County three times per month. The program provides an individualized care plan; medication adherence training; lessons on tracking daily blood glucose levels and how that relates to medications, diet and exercise; preventing long term complications; personal goals; and identifying signs of hypoglycemia and hyperglycemia. As a result of the program, participants manage their diabetes, lead a healthy lifestyle, and prevent unnecessary hospitalizations. Student Health Expo St. Vincent s Mobile Health Outreach Ministry s Pediatric Dietitian annually participates in Clay County s High School Student Health Expo. The annual health expo was held at Orange Park High School, with 1,600 students in attendance. The dietitian conducted a scavenger hunt on healthy eating patterns, taught about food label facts, and explained what to increase and decrease for a healthy eating pattern. Prizes and give-a-ways included a food journal, water bottle, bracelet and a fast food nutrition guide. This nutritional knowledge is at a perfect time in their lives to learn about a healthy lifestyle and obtain information that will prepare them for a healthier future.
5 Club 55+ This club was developed exclusively for residents 55 years and older. This hospital based program is designed to keep our community healthy and active by offering a unique blend of activities, programs and inpatient welcome services. A wide variety of health-related seminars, wellness screenings, fitness programs, social and leisure services are scheduled monthly for members. Other Community Partnerships St. Vincent s is a generous supporter of non-profits and community organizations in Clay County, providing cash donations, free health and wellness education at events, and associate volunteer hours. Organizations and events we supported in FY16 include: Concert on the Green, Clay County Annual Senior Expo, Clay County Senior Christmas Party, Clay County Foster Children Easter Baskets, Kids First of Florida, and JP Hall Children s Charities.
