1 1 Job Description Minor Illness Practitioner Job Summary The King Street Walk in Centre (WIC) is a nurse led and patient focussed service based within the centre of Wakefield. The WIC provides healthcare access and provision to the surrounding community for those attending with a primary care, minor illness or minor injury related condition. The minor illness practitioner will work as an autonomous practitioner in the assessment, diagnosis and treatment of patients presenting to the service. The Minor Illness Practitioner will work within agreed guidelines and protocols to deliver a safe, effective and efficient service. The post holder will also be required to support the development of the WIC in order that the its purpose and function is appropriate to the demands placed upon the service. Principle Responsibilities Work within the King Street Walk in Centre as an autonomous practitioner caring for patients, their families and significant others Utilise advanced clinical skills to provide education to service users and empower them to make informed choices about their treatment Work as an autonomous practitioner in accordance with their professional body Work within the policies and procedures laid out by Local Care Direct Clinical Responsibilities To assess, diagnose, treat and refer or signpost patients who attend with undifferentiated or undiagnosed condition relating to primary care, minor illness or minor injury Liaise and network closely with other colleagues within primary and secondary care and other agencies, referring patients as necessary Ensure evidence based care is delivered at a high standard to patients providing expert clinical advice when appropriate To formally and informally disseminate knowledge and skills to other colleagues, promoting peer review and reflective practice within the King Street WIC Identify own professional development needs through the Personal Development Review Process To participate in the audit process, evaluating the service, implementing action plans and change of practice as necessary Be actively involved in the development of new policies and procedures Prescribe medications as appropriate following national and local guidance

2 2 Be computer literate and have experience of using SystmOne Establish patient capacity and obtain informed consent prior to care delivery. Where a patient lacks capacity, act in the patients best interests seeking appropriate advice and support where necessary Be fully compliant with the companies Infection Prevention and Control (IP&C) policies and attend/complete yearly IP&C training Safeguard children and vulnerable adults promoting their welfare and to work to prevent, recognise, respond, appropriately refer and record potential neglect or abuse, adhering to the company s Safeguarding Children and Safeguarding Vulnerable Adults and PREVENT policies and procedures Effective Communication To communicate at all expected levels to ensure an effective service is provided To ensure that the service user is involved with any discussions about their care management and that this is documented To maintain accurate, contemporaneous healthcare records appropriate to the consultation Managing People / Self Ensure that professional practice is evidence based and current Ensure that Continual Professional Development is maintained and facilitates the ability to provide care to a high standard in a way that is both safe and cost effective Ensure that mandatory training is up to date and participate in training as appropriate Provide and effective and efficient service including utilising time management, organisational, co-ordination and management skills To support the clinical lead undertaking specific areas of work as requested Participate in an annual Professional Development Review (PDR) Demonstrate Team Working Skills Participate in audit to improve service delivery and design Seek and participate in clinical supervision Actively promote the service and signpost to other services as appropriate Share achievements and challenge tradition Managing Information Adhere to the requirements of the Data Protection Act 1998 and Caldicott recommendations Continually evaluate and audit the quality and effectiveness of the practice of self and others Critically evaluate and interpret evidence based research findings from diverse sources, making informed decisions about their implications for changing and / or developing services and clinical practice

3 3 Clinical Governance Promote and maintain high standards relating to health, safety and welfare to self and others Ensure that comments or complaints are dealt with in accordance with Local Care Direct Policies and Guidelines Work within the policies and guidelines approved by Local Care Direct Report incidents as outlined in the Incident Reporting Policy Financial Responsibilities Demonstrate a basic understanding of budget management and have the ability to be creative in the utilisation of resources Support the clinical lead and organisation with cost improvement programmes Understand good resource management for effective day to day running of the WIC Service Improvement Contribute to the service planning of the WIC Assist in the recruitment and selection process Promote and maintain good working relationships within and outside the urgent care setting utilising the opportunity to promote the work of Local Care Direct Establish links within both primary and secondary care to ensure a corporate and strategic achievement of goals The main duties and responsibilities shown above are not exhaustive but should merely be regarded as a guide.
