2 Purpose of the role : To ensure the safe, effective and cost-effective (i.e. quality) use of medicines for inpatients in Waitemata District Health Board (DHB) hospitals. To support the efficient supply, safe storage and distribution of medication to all services and clients of Waitemata DHB hospitals. To provide a clinical pharmacy service to identified services and inpatients of Waitemata DHB hospitals in accordance with relevant organisational and professional standards. To ensure training and support for pharmacists within the first three years of registration, and for pharmacists entering into hospital pharmacy who have not had previous experience in hospital pharmacy. Scope of Practice Pharmacist as defined by the Pharmacy Council of New Zealand Communication and Key Relationships 1. To communicate clearly and effectively with all other pharmacy staff members 2. Attend regular staff meetings 3. Promote team work practices in the pharmacy, participating in consultative processes within the department 4. To communicate clearly and effectively and to build good relationships with other members of the multidisciplinary team within the hospital Good communication between staff member and other members of the team Attendance at staff meetings Pharmacy staff promote the core values of Waitemata DHB Good communication between pharmacist and multidisciplinary team members Quality and Service 1. To provide a clinical pharmacy service that adheres to the relevant organisational and professional Standards. To participate in the collections of clinical pharmacy interventions and workload statistics as requested by Clinical Pharmacist Team Leader 3 To participate in Quality programmes as requested by the Clinical Pharmacist Team Leader 5 To participate in peer review and competence assessment programmes Clinical Pharmacy interventions and statistics are provided Participation in Quality programmes Clinical competency maintained Clinical Pharmacy Activities 1. Patient Medication Histories and Medicine Reconciliation Participation as per Waitemata DHB Clinical Pharmacy Guidelines 2. Monitoring of medicines Medication Order Review Adverse Drug Reaction Monitoring and reporting Clinical review Therapeutic Drug Monitoring 3. Participation in ward rounds and meetings under supervision. 4. To support the multidisciplinary team in the use of electronic medicines management systems (emedicines Reconciliation and eprescribing & Administration). 5. Provision of Medicine Information 6.Patient counseling 7.Education on medicines to patient groups and health professionals 8.Participation in Drug Utilisation Evaluation activities as directed by Clinical Pharmacist Team Leader 9.Participation in Continuing Education and maintenance of CE Logbook Waitemata District Health Board -JOB DESCRIPTION - Page 3

4 10. Participation in NZHPA activities where appropriate 11. Participation in post-graduate clinical pharmacy studies where appropriate 12. To ensure that any problems in the supply of medicines to the ward are resolved, and to act as the liaison person between the ward and the dispensary team General Duties 1.To maintain dispensing skills by participating in the dispensary checking roster at a level as agreed with the Clinical Pharmacy Team Leader and the Clinical Coordinator 2.To participate in the weekend service 3. To carry out any other duties as requested by the Clinical Pharmacy Team Leader or the Clinical Coordinator Participation in dispensary roster Participation in weekend service Other duties as requested Statutory & Treaty of Waitangi obligations Ensures the professional and political integrity of WDHB by carrying out all functions in compliance of the Treaty of Waitangi and by demonstrating a serious commitment to keeping the treaty alive.
