Services include emergency relief, housing (crisis and transitional accommodation), meals, a day centre, welfare and essential health provision. St Patrick s also delivers specialist programs including education and training, art and music, recreation, youth at risk, rough sleepers and proudly partners with other agencies across the community to achieve these goals. The agency s primary aim is to promote social inclusion and self-worth and help people to return to independent living, a safe environment and a better quality of life. PROGRAM PROFILE St Patrick s Community Support Centre s (St Patrick s) Housing and Support Services provides a number of specialist services working with individuals who are rough sleeping, including those who are chronically homeless. These include Street to Home (S2H), and more recently, the 20 Lives 20 homes project developed in partnership with Ruah Community Services S2H is an interagency, integrated response to the accommodation, general health and other support needs of people sleeping rough consisting of a team of specialist staff who engage with clients who are sleeping rough, within a case management model. The S2H team assess clients immediate and long-term needs along with assisting clients to connect with services and support agencies and secure long term, suitable and sustainable housing. 10. 2 St Patrick s including the S2H team - form a key part of a broader collaboration in Perth known as 50 Lives 50 Homes, a partnership of human services agencies with backbone leadership provided by Ruah. The 20 Lives 20 Homes project will be the first place-based trial of the 50 Homes 50 Lives housing first model in the metropolitan area, based in Fremantle. The St Patrick s 20 Lives 20 Homes team will work along with Ruah and a range of collaborating agencies, to provide intensive wrap around support to individuals facing complex barriers to exiting homelessness. Ruah will provide backbone support for the project, along with after-hours support for individuals engaged with the program. 20 Lives 20 Homes aims to remove barriers, through a person-centred and trauma informed approach, working with the individuals to put in place the wrap around services required, brokering of housing, and provision of support to sustain that housing in the long term. The service activity will take place exclusively in Fremantle region. POSITION PROFILE The 20 Lives 20 Homes Support Worker role works with team members in the S2H program to support chronic rough sleepers in to rapidly secure sustainable and stable housing and provides intensive case management through a client-centred and trauma informed approach. The role will focus on establishing relationships with chronic rough sleepers in the Fremantle area, lifting their engagement with our client services to improve access to intensive support services, adapting to meet the needs of people from diverse backgrounds and experiences, accommodate for cultural safety when engaging Aboriginal and Torres Strait Islander people in the service, and demonstrating cultural responsiveness in your supportive approach and service delivery. KEY ACCOUNTABILITIES Delivery of quality service provision Contribute to meeting reporting and compliance requirements including key performance indicators, outputs and outcomes set by funding bodies within area of responsibility Meeting KPI s set by St Patrick s Community Support Centre Contribute to service development and planning RESPONSIBILITIES AND DUTIES Case management /Outreach In liaison with St Patrick s front line staff and external stakeholders identify and establish relationships with chronic rough sleepers in the Fremantle area and prioritise their needs through intensive case management under the guidance and supervision of the Senior Assertive Outreach Worker. This work will include: Undertaking regular weekly case management meetings with clients as required Developing personal and accommodation support plans with clients. Intensively support clients to carry out support plans and achieve their goals, including actively monitoring their progress with tools such as Outcome Star. Actively supporting individuals to access and maintain treatment in drug and alcohol programs as required. Provide supported referral and links to mainstream services including local health services, mental health services and drug and alcohol specialist services. 10. SUPPORT WORKER 20 LIVES 20 HOMES 2
3 Linking clients with social security support, as well as accessing employment, education and training where possible. Establish positive, meaningful, supportive and lasting social networks and support reconnection with family and personal networks where possible Accessing brokerage funds to ensure services are responsive to client needs and to facilitate an integrated approach between specialist homelessness services and mainstream agencies. Supporting clients to be fast tracked into secure, safe and suitable housing that is appropriate to their needs, whilst enabling them to retain and thrive in long-term housing. Actively support access and maintenance, where required, of treatment and/or support in specialist services / programs. Providing referral and links to mainstream services including local health services. Liaising with key project partners to ensure a wraparound approach to supporting clients in the program, particularly through Ruah and the after-hours support team Providing clients with advocacy support when required. Stakeholder engagement /networks Attend and participate in network meetings including team / program meetings, with project partners including Ruah and others, as directed or required Liaison with specialist services that can provide additional support for client groups with identified complex and diverse needs i.e. ageing clients, clients living with disabilities, drug and alcohol issue, mental health, Aboriginal and Torres Strait Islander people or from cultural and linguistically diverse (CALD). Develop and maintain a comprehensive knowledge of services available to the client groups. Administration Undertake administrative tasks as appropriate including data entry, contributing to reports and minutes. Record client data on agency and or funding CRM s as required. Ensure appropriate records are kept in line with privacy act and organisation policies. Contribute to program reports as directed. Organisation requirements Work in a team based environment, contributing to and supporting teams at St Patrick s as appropriate. Ensure that the rights, interests and needs of the people we support are addressed effectively, while understanding and respecting the linguistic and cultural diversity of the people we support and our work teams. Ensure that all St Patrick s Policies and Procedures are implemented and complied with, including occupational health and safety regulations. 10. SUPPORT WORKER 20 LIVES 20 HOMES 3
4 Undertake appropriate training and maintain qualifications, licences, certificates, checks and clearances. Contribute to the continuous improvement of activities, including safety and quality. Undertake other duties as reasonably requested. CHARACTERISTICS OF THE POSITION The employee is required to set priorities, plan and organise their own work as well as being team work orientated. The employee is responsible for project and/or functions that will be required to establish outcomes to achieve the organisation s goals. The employee adheres to established work practices; however they may be required to exercise initiative and judgement where practices and direction are not clearly defined. High level interpersonal skills are required to develop and maintain positive working relationships with clients and staff and other service providers. SELECTION CRITERIA 1. High-level communication skills, including written and verbal skills. 2. The ability to effectively network within the sector. 3. Good understanding of existing public, private and community housing pathways for people in housing crisis 4. The ability to form working relationships with high-needs clients. 5. Ability to work effectively as a team member and ability to work independently when required. 6. Demonstrated understanding of issues affecting the Aboriginal and Torres Strait Islander community particularly in regards to transience and homelessness, cultural awareness and sensitivities. 7. Demonstrated ability to establish effective working relationships with people from a wide range of socio-economic, multicultural and linguistically diverse backgrounds. 8. Ability to work within the values of St Patrick s Community Support Centre. 9. An understanding of health and safety issues relevant to the position. QUALIFICATIONS AND EXPERIENCE Essential 1. Under the provision of Section 50D of the Equal Opportunity Act 1984 (WA), and considering the duties involved and client demographics, Aboriginal and or Torres Strait Islander Applicants are required for at least one of the two 20 Lives 20 Homes Support Worker. 2. Diploma or Degree Level qualification / or equivalent experience in the field. 3. Demonstrated relevant experience in case management. 4. Demonstrate understanding factors contributing to homeless as well as identified gaps and limitations while working with people experiencing homelessness. 10. SUPPORT WORKER 20 LIVES 20 HOMES 4
5 5. Demonstrate understanding and relevant experience working with people at risk 6. Appropriate First Aid certificate or willingness to undertake first aid training. 7. Working with Children Check as required per agency policy and procedure. 8. National Police Clearance. 9. Possession of an appropriate class motor vehicle driver s license. Desirable 1. Demonstrated experience in working with clients with high needs. 2. Evidence of training and experience in undertaking a trauma informed and client-centred approach to working with clients 3. Evidence of training and continuous professional development in working with individuals from a diverse or Aboriginal and Torres Strait Islander background in a culturally safe manner 4. Previous knowledge of data collection programs (or similar), such as SHIP, DSS Data exchange 5. Demonstrated understanding of existing of existing public, private and community housing pathways for people in housing crisis 6. Relevant computer skills in MS Office software and applications.
