An independent, nonprofit Community Health Center providing primary & preventative care The organization must Be located in, or serve, a Medically Underserved Area Be designated by HRSA in a competitive process Meet 19 Program Expectations Be governed by a Board with a patient majority (51%) Slide fees for uninsured patients (under 200% FPL); charge a nominal fee for those who qualify for a 100% discount

7 Anyone Can Be Our Patient We accept Most all commercial insurances Medicare and Medicaid (Traditional and MCPs) Uninsured patients may apply for discounted fees based on Federal Poverty Guidelines Everyone pays something Payment is collected at time of service Exception: fees are waived for homeless persons in homeless shelters

CHC care is ranked by GAO as among the most costeffective Average cost of care is $1.67 per CHC patient/day vs. $2.64 US average at all physician offices (2011) Chronic disease management, reduced use of ER, fewer hospital days produces $24B in annual health system savings In OH, almost $1B is wasted annually on avoidable ER visits; if avoidable ER visits were redirected to a CHC, the US would save $18B in annual health care costs
