1 Position Description: HACC Practitioner Position Details Position Title: Location: Classification: Time Fraction: Hours of Work: Contract Detail: HACC (Home and Community Care) Practitioner PSS Mornington (or other locations as directed) SCHADS Level 3 - Dependent upon qualifications and experience Part-time classification Monday to Friday 8.30am to 5.00 pm Subject to Funding Organisation Overview Peninsula Support Services (PSS) is a not-for-profit, Mental Health Community Support Service (MHCSS) supporting people (16-64 years) with severe and enduring mental illness and psychiatric disabilities, along with supporting HACC eligible clients (no age limit) and carers of people with a mental illness. PSS assists people to recognise their values, strengths and goals, with an emphasis on improving health and wellbeing, independence, social participation and enhancing meaningful relationships with significant others. PSS was established in 1987 and has evolved significantly to become a leader in the areas of delivering flexible support packages tailored to the individual using strengths-based, recovery-orientated practice, quality and evidenced based service provision. PSS strives to provide opportunity for people living with mental illness to enjoy a meaningful life by strengthening self-identity, personal responsibility and hope. Our vision is for optimum personal recovery and wellbeing. PSS believes that recovery-orientated practice is a partnership where the person with the lived experience is the expert in their recovery and our staff bring their understanding and expertise to support this journey. Our service model is based on Coleman s Recovery approach and incorporates the (DoH) Victorian Framework for Recovery-Orientated Practice (2011) and the National Mental Health Standards (2010). This model believes that recovery practice should be person-centred, accepting of diversity, strengths-based, provide respect and dignity, community connection and participation. PSS places high importance on maintaining a skilled, supported and informed workforce, providing opportunities for reflective practice, flexibility and innovation, developing and maintaining collaborative partnerships, evaluation, planning and continuous improvement. 2 Conditions of Employment In accordance with Peninsula Support Services Enterprise Agreement 2011, incorporating the terms and conditions specified in the Social, Community, Home Care and Disability Services Award 2010 Salary Range from Level 3.1 $46, The current Enterprise Agreement 2011 allows for flexible working hours. Full time employees will be entitled to one Rostered Day Off every month Includes salary packaging up to $16,000 per annum provided by a selected salary packaging provider. This can lead to tax savings up to $5,500 per annum Clear result of Criminal Record and Working with Children Checks are mandatory prior to commencement of position, with the costs covered by PSS Peninsula Support Services encourages people with a disability to apply and encourages full disclosure of illness or disability. Qualifications and Requirements Qualification in mental health, aged and disability care or related discipline minimum Cert. IV in Aged Care related studies, diploma level preferred Ability to drive 12 seater bus, note: training will be made available Previous work experience in Mental Health and/or community sector an advantage Current Victorian Driver s License and ability to undertake travel as required Satisfactory Police Record Check Consent to a Working with Children s Check Right to work in Australia. 3 Position Overview In the PSS structure, the HACC (Home and Community Care) Practitioner, works closely with the HACC Coordinator to provide service delivery that is age appropriate, client directed, maintains current levels of physical activity, strengths based and family sensitive in line with the HACC Service Delivery, ASM (Active Service Model) and Diversity guidelines. The HACC Practitioner will use their skills, knowledge, case work and group work strategies to support their clients and their family to develop the Individual Support Plan (ISPs), Wellness and Discharge (where appropriate) Plans that achieves the identified goal within set timeframes. Under general direction of the HACC Coordinator, the HACC Practitioner ensures a high standard of service delivery that is consistent with the organisation s values and service models. The HACC Practitioner is charged with providing opportunities that assist with maintaining physical and daily living skills and personal development, community and family education, mental health and wellbeing coping strategies and community engagement in line with the client s Individual Support Plan. HACC service delivery is complimented by drawing on external (informal and formal) support networks (Support Team) working collaboratively to ensure that when the client is ready for discharged, they are able to self-manage with these supports in place. Individual Support Plans and case/group work is regularly reviewed with the support of the HACC Coordinator. The objectives of HACC service provision is to holistically assist clients to: Develop the knowledge, skills and confidence clients need to make decisions and choices about their support needs Learn or relearn skills and develop confidence required for activities of daily living (e.g. budgeting, buying and cooking food, healthy living skills, personal care, care for one s home environment) Improve the individual s social relationship skills and confidence (e.g. making and keeping social and recreational contacts, and relationships with friends, family, neighbours and significant others) Access and engage with (public or private) mental health treatment services, including general practitioners as required to manage their mental illness Contribute to achieving client directed outcomes in regard to physical health, community connections and other relevant needs. Client-facing support including: Facilitating HACC PAG and SRS groups Ability to apply the principles of the ASM Participate as directed, in providing needs assessment Support or develop the client-directed Individual Support Plan Monitoring and review of this plan at regular intervals, in partnership with the client, and HACC Coordinator Achieve client directed outcomes in regard to physical health, community connections and other relevant needs Engage in Planned Activity Groups (PAG) which support their ability to remain living in the community by providing peer support and a range of enjoyable and meaningful activities Provision of support (based on the client s support plan), with a focus on: o Building the individual s daily living skills and their capacity for self-care and self-management of their mental illness o Helping the individual to improve their relationship skills and social connectedness o Provide supported referral to assist the client to access and engage with health services (e.g. clinical mental health treatment services and primary healthcare, including drug and alcohol treatment services), human services and other social support services they need o Activity related to planned discharge o Building the individuals daily living skills and their capacity for self-care and self- management of their physical and mental health wellbeing o Follow-up phone and in person support including assisted onward referrals. Non-facing client support including: Access and engage with mental health treatment services (public or private) including general practitioners and other health services as required Travel time Time spent documenting case notes or other client related information Time spent organising activities or providing other support functions on behalf of clients (e.g. organising appointments) Develop quarterly group programs and budget accordingly. 5 Finance and Performance Ensure all service expenditure has approval in line with PSS Delegation Schedule policy and guidelines Ensure all accounting and documentation processes are accurate and within budget Working closely with the HACC Coordinator, reflectively review HACC practices in line with service model, performance targets and related policies, and practice guidelines Support the HACC Coordinator by ensuring Minimum Set Data (MDS) targets are met Support the Service Manager by preparing draft ASM, Diversity Plan as directed by the HACC Coordinator Support the Service Manager by assisting with funding and reporting requirements and compliance obligation, as directed by the HACC Coordinator. Practice Knowledge General understanding of mental health and behavioural diagnoses and symptoms Good understanding of the causes of stigma and barriers faced by those with disability and mental ill health Sound understanding of age and disability services Experience in providing; strengths-based, person-centred group work practices. Professional Development Participate in fortnightly supervision with the HACC Coordinator Undertake annual performance appraisals with the HACC Coordinator and the Service Manager and develop and implement own professional development plans Actively take part in training and skill building opportunities. Administration Promotion Ensure the completion of Minimum Data Set (MDS) and other reporting requirements within set timeframes in readiness for reporting by the Service Manager, and completed to the highest standard Record case notes as required in line with policies and guidelines Ensure HR policies and procedures are adhered to Ensure adherence to Critical Incident Reporting Ensure HR responsibilities are met within set timeframes i.e. timesheets, personal and annual leave forms, 50/52 requests Possess good written and verbal communication and IT skills. Actively promote the HACC program within the community and with local service providers Ensure consistent number of clients are actively engage to meet funding targets Enact strategies as directed, to ramp up client numbers as directed. 6 Key Selection Criteria 1. Demonstrated practice knowledge of working with HACC eligible, older people with mental ill health and/or those who present with behaviours that makes access to general services and community programs difficult. 2. Demonstrated skills and knowledge in the area of planning, implementing, reviewing and evaluating effective recovery-orientated case work and group work. 3. Proven ability to consider and develop and deliver effective annual program plans in-line with funding direction i.e. HACC Active Service Model, Diversity and Diversity Plan. 4. Demonstrated skills in promoting programs and achieving referrals from clinical and general service providers and general and cultural aged community groups. 5. Good understanding of the HACC Active Service Model and Diversity planning. 6. Proven ability to have courageous conversations with others including problem solving and conflict resolution. 7. Demonstrated ability to work as an effective and collaborative team member, including demonstration of a high level of professional and ethical behaviour within an organisational setting. Demonstrated ability to work autonomously and flexibility; including managing competing priorities, using initiative to problem solve and manage own workload. 8. Demonstrated knowledge of and ability to undertake risk assessments and/or strategies that ameliorate risks when working with individuals who can at times display difficult and aggressive behaviours. 9. Competent administration abilities including working with IT systems such as client databases, computer literacy and level of ability with Microsoft packages (Word, Outlook and Excel) and search engines. 10.
