3 President and Chief Executive Officer Our commitment to the community extends far and wide as we care for and improve people s lives where they work, live, worship, and play. For more than 155 years, Providence Health System has been privileged to serve the residents of the District of Columbia striving to provide the care needed, when and where it is needed. As a community hospital and catholic organization, we continue to live our mission of providing care to those in need, particularly to those who are struggling. At Providence, we take a comprehensive view of caring for our patients and the community that we serve. We understand that good health is not created solely within the confine walls of the hospital. That is why we have initiatives such as the Medical Mission at Home that place special attention on those who are most vulnerable. This collaboration, which brings public and private partners together, not only shows how community partnerships can create better opportunities for health, but is also a good reminder of how much can be accomplished if we work together. Important programs such as Breast Cancer Patient Navigation and the Model Cities Senior Wellness Center are also highlighted in this report. Providence looks forward to a vibrant and innovative future, serving the District of Columbia where we are most needed. We hope you enjoy reading about our recent work in the pages that follow. We fulfill this promise to our communities as we continue to embrace Healthcare That Leaves No One Behind by initiating and expanding community-based programs to improve the health of those we serve. Through partnerships and collaboration with other community stakeholders, we continue to show our commitment to the community in the development of programs that improve the health of all and especially those who are struggling the most. In fiscal year 2016, Providence provided more than $13 million in overall community benefit, including over $3 million in financial assistance and $7 million in the community benefit categories outlined below C o m m u n i t y B e n e f i t R e p o r t Page 2
5 Model Cities Senior Wellness Centers Promoting healthy lifestyles is the primary focus of the Senior Wellness Center - through an optimum state of health and well-being, achieved through disease prevention and health promotion. The Model Cities Senior Wellness Center provides services to DC seniors through a partnership with the District of Columbia Office on Aging which Providence operates. The fitness and education programs are designed to assist members in improving and maintaining health. In fiscal year 2016, Model Cities membership increased by 122 new members with a total of 1,013 active members. Activities are designed with the emphasis on nutrition, cardiovascular, muscular strength, muscular endurance, flexibility, and body composition. In FY16, 70 health promotion classes were conducted with an average of 6 fitness classes per day; more than 1,564 seniors participated in fitness classes. Seniors are also provided programming that focus on their social needs to improve the quality of life. As such, trips and social activities are scheduled daily. There were 60 trips scheduled and 450 social activities completed for FY16. In addition, three health fairs were conducted through collaboration with 10 organizations. Providence Medical Mission at Home As part of our mission to serve all persons with special attention to those who are most vulnerable, in May 2016, Providence hosted its second Medical Mission at Home (MMAH), a day of Hope, Health and Healing at the Our Lady of Perpetual Help Church to benefit the east end community and others around the Washington, DC area. Providence and its partners delivered free health, social and support services to approximately 75 people in the DC area. In partnership with local community organizations, more than 200 volunteers came together to provide comprehensive community health services. We provided health screenings and testing (e.g., blood pressure, cholesterol, etc.), medical care, dental care, foot care and foot washing, behavioral health support, and spiritual counseling and prayer. Out of the 75 people who received medical care, more than half also received dental care. Fifteen percent (15%) received foot care, had their feet washed, and received new socks. Thirty percent (30%) sought spiritual counseling and ten percent (10%) received bahavioral healthcare services. Providence Breast Cancer Patient Navigation Program The initial reaction to a breast cancer diagnosis can leave women in shock and disbelief and too often overwhelmed to search for and access different available care options. At the same time, with the assistance of a community partner and a navigator, these patients can better understand their treatment and care options. The Providence Breast Cancer Patient Navigation Program provides this critical support along the continuum of breast health care, focusing specifically on navigation of patients with abnormal screenings, to ensure they receive appropriate diagnostic follow-up and treatment. Patient Navigation Services In fiscal year 2016, navigators tracked and facilitated communication amongst patients, hospital-based specialist and primary care providers supporting many patients to receive the care they needed. Out of 7,964 patients that were screened, 639 patients received additional imaging and 221 of those patients had follow-up biopsy. The navigators also provided referral assistance to various clinical and social services such as employment related issues, social support, interpreter services, home health services due to disability issues, financial assistance, and psychiatry due to fear or negative perceptions. Patients were accompanied to doctor s visits. Ninety-three (93) patients were assisted with referrals from their primary care providers; 135 patients received assistance with scheduling appointments; and 30 patients were assisted with obtaining breast prosthesis and bras C o m m u n i t y B e n e f i t R e p o r t Page 3
6 Education Patients received education through an inter-disciplinary team approach. The objective was to provide education to patients with abnormal findings and their families as they move through the breast cancer continuum of care. Patients were provided with community resources for support group, housing, employment, disability and mental health concerns. Translation services were also provided onsite. In 2016, Providence Health System, through the DC Healthy Communities Collaborative, conducted its second community health needs assessment (CHNA). This assessment represents a shift from a focus on individual clinical conditions to larger social determinants of health. Four priority areas mental health, care coordination, place-based care (bringing care to the community), and health literacy were identified.
