2 About Area 4 Pharmacotherapy Network The Area 4 Pharmacotherapy Network is funded by the Victorian Department of Health and Human Services. The SEMPHN is funded as the lead agency for the Area 4 Pharmacotherapy Network to implement the Network across Southern and Eastern Melbourne. The Area 4 Pharmacotherapy Network is dedicated to implementing a more integrated and cohesive Pharmacotherapy service in the Southern and Eastern Metropolitan Melbourne. SEMPHN is in partnership with the Eastern Melbourne PHN to support the objectives of the Pharmacotherapy Network. The Network provides advice and support to GPs, pharmacists and the AOD sector regarding best practice and emerging issues; connecting GPs and Addiction Medicine Specialists, facilitating access to training and increasing prescribing and dispensing of pharmacotherapy. Job Summary The Pharmacotherapy Liaison Coordinator role is to build capacity for community based pharmacotherapy services for opioid dependence across the A4PN catchment. The role has a focus on supporting front line practitioners in their various roles that promote improved access and quality of pharmacotherapy services. The role works closely with PHN staff and external primary health services to develop systems, frameworks, targeted support and intervention to support the pharmacotherapy service system. The role brings together primary health professionals involved in the area of addiction through the local Communities of Practice education and training model. The Pharmacotherapy Liaison Coordinator will coordinate mentoring opportunities and develop and document referral pathways between community providers and specialist service providers. There will be a strong focus on local Addiction Medicine Specialists and relevant alcohol and drug services within the catchment. Key Responsibilities Pharmacotherapy Network Development Building capacity of GPs and Pharmacies to respond effectively to clients requiring pharmacotherapy treatment; Undertaking mapping and monitoring of the needs and assets of each local catchment area to identify opportunities for increased access to pharmacotherapy services; Working with key stakeholders, including consumers, to clarify and document client pathways to better support those with complex health issues that are being prescribed pharmacotherapy; Providing a range of primary health care initiatives to support client pathways for complex health issues and stabilisation of clients on pharmacotherapy; Working with the alcohol and drug sector to provide training and identify integrated programs and pathways to specialist services; Develop and maintain networks with relevant primary health providers, government and community agencies, through projects, consultation, and participation; Coordinating individual and community requests for information; Developing and maintenance of data portal for resources, guidelines support material etc.; Participating in research / evaluation projects where applicable and relevant; Establishing annual service objectives, develop performance indicators and evaluate outcomes. 3 Quality and Clinical Governance Practices within the scope of professional discipline and range of activities. Promote and embed a culture of clinical best practice, quality improvement and measureable outcomes. Work collaboratively with the Coordinator Pharmacotherapy Network to establish and manage the clinical governance, risk management and continuous quality improvement initiatives in relation to Area 4 Pharmacotherapy Network. Work collaboratively with the Coordinator Pharmacotherapy Network to coordinate accreditation activities within the Area 4 Pharmacotherapy Network. Project Work Under the guidance of the Network Coordinator, design, develop and implement locally based projects that promote access to quality pharmacotherapy services. Implement key projects as part of the Pharmacotherapy Network Action Plan. People Promote, and maintain a positive and collaborative work environment. Identify and act on opportunities to integrate and work collaboratively across Primary Health Networks, primary care, AOD and mental health programs. Contribute to optimal team efficiency and output. Participate in probation and annual professional reviews. Maintain effective relationships with internal and external stakeholders. Finance Work within the approved Area 4 Pharmacotherapy Network budgets Research, Planning and Development Ensure collection and analysis of appropriate data for planning, reporting, evaluation and quality purposes. Contribute to the design, development and implementation of research opportunities. Liaise with population health and planning roles to ensure robust action plans are developed, implemented and reported. Participate in pharmacotherapy and AOD activities as identified by the Coordinator Pharmacotherapy Netwok, local and State wide networks and connections. 5 Key Selection Criteria Qualifications Tertiary qualifications in pharmacy, nursing or other appropriate health qualifications, or equivalent. Skills, Knowledge and Experience Demonstrated strong understanding of the issues and challenges surrounding the provision of pharmacotherapy services. Relevant experience in pharmacotherapy service provision including stakeholder engagement and training. Experience in developing and implementing successful health care initiatives and programs. A good understanding of substance misuse and the principles of harm reduction. Demonstrated experience in quality and clinical governance practices and best practice standards. Demonstrated experience in effectively engaging and liaising with GPs, practice nurses, pharmacists and practice managers. A high degree of initiative and flexibility to adapt to changing circumstances as the project develops. Proven ability to multi-task, set priorities and meet strict deadlines. Good written and oral communication skills, including proven skills in negotiating, developing and managing cooperative relationships with multiple and diverse stakeholders. Ability to work independently and as part of a team. Well-developed computer skills, including , Microsoft Office suite (Word, Excel, Powerpoint) and Customer Relationship Management (CRM) systems. A good knowledge of relevant legislative and regulatory requirements. Other A demonstrable commitment to SEMPHN organisational values.
