1 Community s Project definitions 2018 benchmarking cycle 12 Community/District Nursing definition NHSBN supplementary definition 16 Health Visiting 11 Community Diabetes - Adult 14 Community End of Life Care Direct treatment of patients with diabetes including type 1 and 2. Includes interventions such as glucose monitoring and training on administering insulin. Care of a patient during the last stage of their life (patients in a progressive state of decline). This includes Palliative Care and Terminal Care. Community/District Nursing services provide nursing care to housebound patients in a particular geographic area. The service is delivered in the patients' own home or in residential care homes. District Nursing services may also provide clinic based services. Core Community/District Nursing functions include initial, holistic assessment of patient s needs, referrals to social care, referrals for equipment, prescription of medication or referrals for medication, continence care, follow up care for patients recently discharged from hospital, long term care for chronically ill patients, end of life care, wound care and support for families and carers. Out of hours and rapid response district nursing services should be included within Community/District Nursing. Specialist nursing services (i.e. condition specific services) should not be included within Community/District Nursing. Community Matrons/Case Management s should not be included within Community/District Nursing. s for children from birth to school age and their families/carers delivered by Health Visitors. s delivered by the Community Specialist Diabetes Team (for adults 18+) only. If this is a multi-disciplinary team, (MDT), ensure that the finance, activity and workforce data are included for all members of the team. s delivered in the community by a Specialist End of Life Care Team. If this service is provided by a multi-disciplinary team (MDT), ensure that the finance, activity and workforce data are included for all members of the team.
2 07 Community Continence definition NHSBN supplementary definition 04 Community Cardiac 30 Community Respiratory 32 School Nursing Treatment of patients with continence problems including physiotherapy, pelvic floor treatments and pants and pads service. Directly treated for conditions related to heart disease. This includes heart failure. Including myocardial infarction, angina and cardiac rehabilitation. Respiratory disease including Chronic Obstructive Pulmonary Disease and asthma. s delivered in the community by a Specialist Continence Team. If this service is provided by a multi-disciplinary team (MDT), ensure that the finance, activity and workforce data are included for all members of the team. s delivered in the community by a Specialist Cardiac Team. If this service is provided by a multi-disciplinary team (MDT), ensure that the finance, activity and workforce data are included for all members of the team. s delivered in the community by a Specialist Respiratory Team. If this service is provided by a multi-disciplinary team (MDT), ensure that the finance, activity and workforce data are included for all members of the team. s for children of school age delivered by School Nurses in maintained schools and academies. Include services provided to special state schools. Exclude services provided under contracts with the independent education sector. 08 Integrated Sexual Health s associated with prevention and treatment of sexually transmitted diseases and contraceptive services. Include Level 1, 2 and Level 3 services. This includes specialist HIV treatment and care.
3 19 Community Matrons/Case Management definition NHSBN supplementary definition 20 Musculoskeletal 26 Physiotherapy - Adult 33 Speech and Language Therapy - Adult 23 Occupational Therapy - Adult Musculoskeletal system including bones, joints and supporting muscles. All patients receiving treatment for MSK conditions such as hip replacement, knee replacement and other joint disorders. Includes MSK rehabilitation. Includes voice rehabilitation e.g. after a stroke, language development and swallowing difficulties. Community Matrons/Case Management services pro-actively identify very high intensity users of services, typically with long term conditions. The services work in partnership with the individual, their carers and other relevant health and social care professionals to co-ordinate and proactively manage their care, utilising holistic physical assessment and non-medical prescribing skills in order to reduce avoidable admissions to hospital and improve quality of life. Specialist nursing services (i.e. condition specific services) should not be included within Community Matrons/Case Management s. Core Community/District Nursing s should not be included within Community Matrons/Case Management s. MSK services are typically multi-disciplinary teams. The whole service should be included. Therapy services (not MDT) should not be included. Physiotherapy services for adults (18+) delivered in community (i.e. non-acute) settings. Speech and language therapy services for adults (18+) delivered in community (i.e. non-acute) settings. Traded S< services are to be excluded. Occupational therapy services for adults (18+) delivered in community (i.e. non-acute) settings.

N/A Dietetics - Dietetics services for children (up to 18 years) delivered in community (i.e. non-acute) settings. Children's 27 Podiatry Podiatry services (including biomechanics) delivered in a community setting. Podiatric surgery and orthotics should be excluded. N/A Wheelchair services Clinical assessment function only for supply of wheelchairs and associated equipment to support to clients with a long-term mobility need. 06 Community Paediatrics General services for children (up to 18 years) delivered in community (i.e. non-acute) settings. Patients can include children with long-term disability (e.g. cerebral palsy, learning disability), children with mental health issues (e.g. autism and ADHD), children who it is feared are being abused, or children who are being fostered and adopted. Community Paediatricians also take responsibility for advising on the health of communities. Some community paediatricians work in the highly specialised area of audiology, looking after children with severe hearing loss. If this service is provided by a multi-disciplinary team (MDT), ensure that the finance, activity and workforce data are included for all members of the team.
5 definition NHSBN supplementary definition Community Integrated Care Teams (CICT) A CICT is a multi-disciplinary and frequently, multi-agency team, providing complex clinical care to service users in their own homes (including residential care homes) and in local clinics and health centres. The core function of the CICT is long-term condition management, providing patients with support for self-management and keeping people as independent as possible in their own homes. CICTs will also support complex discharges from hospital. Typically, CICTs will provide care over a longer time period; as a rule, continuing beyond 6 weeks. The team should be professionally and functionally integrated, but staff do not necessarily have to work for the same organisation. The CICT may offer both physical and mental health care. Most CICTs will be organised around GP practices in neighbourhoods / localities. 05 Community Dental N/A Children s Community Nursing Team (CCNT) All community dental services (for all ages) excluding personal dental services. Children s Community Nursing Teams provide out of hospital care to children and families in their own homes, meeting the needs of ill and disabled children and young people. This would include children with acute and short-term conditions, children requiring care for long-term conditions, children with disabilities and complex conditions, including those requiring continuing health care, and neonates. It would also include children with life-limiting and life-threatening illness, including those requiring palliative and end of life care. Most CCNT offer services up to the age of 16, but some services will extend to age 19. (CCNT)
