1 POSITION DESCRIPTION Position Details: Title: Department: Reports to: Location: Paediatric Anaesthesia and IC Fellow Paediatric Anaesthesia and PICU Service Clinical Director Starship Children s Health Authorised by : Director of Anaesthesia Date: February 2018 Purpose of Position: To provide clinical care of patients under the care of the Department of Paediatric Anaesthesia & The Paediatric Intensive Care Unit. To develop skills in the management of critically ill neonates, infants and children in preparation for independent professional practice as a Paediatric Anaesthetist. To develop a consultant approach Key Accountabilities: In accordance with appropriate Australian and New Zealand College of Anaesthetists Guidelines including: Policy document TE13 Guidelines for the Provisional Fellowship Program. ANZCA training program curriculum 2013 General: A Clinical Duties: 1. Carry out preoperative assessment and ongoing clinical management of patients, under the responsibility of the Department of Paediatric Anaesthesia. This will include delivering anaesthesia services in the operating rooms and at remote locations (such as Greenlane Surgical Unit, CT,MRI scanner, angiography suite, cardiac investigation rooms and radiotherapy). 2. Participate in on-call anaesthetic and perioperative services, as outlined below. 3. Participate in supervision and teaching of other trainees when clinically appropriate. 4. Participate in acute and post-operative pain management of paediatric patients. B Administration Duties: To use the Anaesthesia Fellowship year to allow development of an interest in continuing education, teaching & teaching skills, understanding of research methods and techniques and to understand and develop a responsibility and commitment to the training of other staff. Provide, and participate in, appropriate educational activities for trainee specialists, RMO s, medical students, postgraduate nurses, undergraduate nurses, anaesthetic technicians, other para-medical staff, and interested community groups in subjects such as basic life support. 2. Supervise the preparation of material to be used for teaching. 3. Ensure and review quality of patient care by participating in Quality Assurance programs. 4. Maintain personal knowledge and skills and participating in Continuing Medical Education. Participate in ANZCA Continuing Professional Development program as per ANZCA training requirement. 5. Contribute to the activities of appropriate professional associations. 6. Carry out reviews and investigations on drugs, equipment, methods of clinical management, physiological, pharmacological and other matters relevant to anaesthesia, acute pain management and resuscitation. 7. Maintain a high standard of professional care in accordance with the NZMA Code of Ethics, Australian and New Zealand College of Anaesthetists guidelines, statutory and regulatory requirements, company policies and subsequent amendments thereto. 8. Deliver care in a sensitive fashion, taking into account cultural differences and the recognition of patients' rights. 9. Ensure informed consent for all patients is obtained in accordance with the Company s policy for any operation, test or procedure. 10. Attend operating sessions and other Departmental activities using the allocated time effectively and efficiently to achieve the goals of the annual business plan 11. Compulsory Presentation of Audit or Research work done during the year at the Annual Auckland Trainee/ Registrars Meeting. C Specific Paediatric Anaesthesia Fellow Duties (6 months): 1. Day time commitment 4½ days per week, (i.e. 9 sessions). 2. This will include 8 sessions per week of clinical work plus 1 session for personal continuing medical education (CME/MOPS). 3. The clinical work will include duties undertaken by both trainees and specialists as deemed appropriate by the Clinical Director and other specialist staff. 4. "On call" weekday night and weekend duties will be required. On call duties with the Department of Anaesthesia will involve supervising registrars with immediate consultant backup, covering registrar colleagues during periods of examinations and unexpected leave, and being available for interesting cases, as deemed necessary. 5. Time for clinical research will be awarded upon submission of an appropriate project to the Clinical Director or his designee. This will be substituted from the clinical sessions, or may be negotiated as additional work depending on departmental resources. D Details and duties during Paediatric Intensive Care Unit attachment (6 months): 1. During this period the fellow will be primarily responsible to the Clinical Director of the PICU and his staff (Refer Appendix A for specific role description). Page 2 of 5

3 2. Arrangement can be made for fellow to have extra experience in the Paediatric Cardiac Anaesthesia Service in the operating room but only if PICU rostering can accommodate this. APPENDIX A: Key Accountabilities: A. Clinical Duties: ROLE DESCRIPTION: PICU ATTACHMENT (6 months) 1. The Fellow will work co-operatively with all other teams and members of the PICU team to optimise patient care. 2. The Fellow s first responsibility is clinical care of patients in the PICU. 3. The Fellow will be expected to participate in the on-call roster for PICU as per the roster general description and guidelines. 4. The Fellow is responsible for concise, accurate and problem oriented handover and to write notes documenting progress at the end of every shift. 5. Fellows are expected to keep the on-call Consultant informed of patient admissions and progress. They are expected to call for assistance early rather than face a situation in which they are clinically inexperienced. 6. Fellows are an important part of the Paediatric Emergency Team. Management of the airway and breathing are first priorities. 7. Consultations on the ward regarding potential admissions will be reviewed as expeditiously as possible and discussed with the on-call Consultant. Requests from the wards for IV placements will be dealt with once PICU work is complete. 8. Guidelines both clinical and procedural - will be read and adhered to. 9. Transportation of patients both incoming and in transfer to other institutions is an expected part of the PICU Fellow role. Transports should be discussed with the on-call Consultant and undertaken only when clinical experience is appropriate for the patient being transferred. 10. To be clinically useful transport teams should be able to depart PICU within 30 minutes of a decision to move. While call is not in-house, it is therefore important to be available within this timeframe. 11. Transportation medicine is tiring and long transport duties (greater than 10 hours) will be accompanied by an 8 hour break. B. Administration: 1. The Fellow will ensure accurate and up to date documentation on every patient. 2. Discharge notes and documentation are to be completed prior to any patient (dead or alive) leaving the Unit. 3. Notes must be legible, dated, timed and signed. 4. The Fellow will read and adhere to the Service Description in the Location Manual. Page 3 of 5

4 C. Education and Research: 1. The Fellow will attend the PICU Orientation Program. 2. Fellows are expected to contribute to all aspects of the teaching program both within PICU and the rest of the hospital e.g. Grand Round presentations. 3. Fellows are expected to read around patients to maximise their learning. An education file is provided in the area and text books are available from Consultants and in the locked cupboard in PICU. 4. There will be at two formal teaching sessions per week, including Journal Club, Tutorial and Grand Round, in addition to fortnightly clinical meetings such as QA/ transport and mortality review. 5. Bedside teaching/questioning and discussion of relevant literature is encouraged and expected both during patient handovers and where appropriate in relation to direct patient care. 6. Teaching in practical procedures including airway management is part of clinical care. The Fellows are expected to make the most of opportunities in this regard, including visits to the Operating Room for extra experience when rosters and patient load allow. 7. A research project may be undertaken during the attachment and Fellows will be supported in doing so. This is essential for advanced trainees for meeting College requirements. D.
