1 Schedule 1 KINGSTON AT HOME [Draft contents based on the 2014/15 section 75 while negotiations are carried out for 2015/16. Details will be refreshed once these are complete] Unless the context otherwise requires, the defined terms used in this Scheme Specification shall have the meanings set out in the Agreement. 1 OVERVIEW OF INDIVIDUAL SERVICE In January 2012, Kingston and The Royal Borough of Kingston agreed to work in partnership to develop and deliver an integrated approach to the commissioning and provision of health and social care services delivering rehabilitation, reablement, residential/nursing home care and home based services under the umbrella project Kingston at Home (K@H). The intention of the K@H project is to deliver an integrated home based service model of rehabilitation and reablement (as shown below) led by Your Healthcare CIC () as the preferred provider, who is leading the redesign and co-production work with RBK and the. This direction of travel is underpinned by The Case for Change - March 12 revised March 13 Joint RBK and K Commissioning Intentions - March 12 revised March 13 Management Agreement and Collaborative Agreement between and RBK transferring management responsibilities for RBK services covered under the BTA to ( effective from 1 April 2013) Your Healthcare CIC intermediate care and rehabilitation services community nursing and therapy teams and Cedars Unit inpatient beds specified and commissioned by Kingston PCT in August 2010 and its successor body Kingston from 1 April 2013 Business Transfer Agreement (BTA) of RBK Services to (effective from 1 June 2013) covering the following services o Amy Woodgate Resource Centre including day care, both health and social care; o Day Care at Newent Resource Centre; o Adult Placement Scheme; o Shared Lives o Home Care Service, including the Reablement Team; 1

2 o Community Outreach Team Section 31 between RBK and joint funding arrangement for Intermediate Care Services to support discharge implemented in Section 75 between RBK and Kingston discharging Lead Commissioner responsibilities to Kingston for those services under the BTA (effective from 1 May 2013). The RBK Transforming Community Services Programme The RBK transforming Community Services Programme has identified a range of service transformation and redesign opportunities to improve the cost effectiveness and deliver efficiencies across a range of adult social care services which are part of and /or interface with the Kingston at Home project. The Kingston s QIPP programme As part of s 13/14 QIPP programme, the joint Kingston at Home project is required to deliver a contribution to the s QIPP programme, this statement of intent outlines the details of the KAH QIPP scheme. Following discussions between and K Commissioners, it has been agreed to focus the K@H QIPP on enhancing and transforming the community infrastructure to provide an increase in home based rehabilitation, treatment and care through a reduction in 12 community health beds in the Cedars Unit Tolworth Hospital to release funding for reinvestment into the community workforce and pathway redesign to deliver the new service model and care pathways. A joint commissioner and provider transition/project plan is under development to be completed by 31 May 2013 and financial modelling will be undertaken to effect the commissioning and service transformation of the new Kingston at Home service model from September AIMS AND OUTCOMES Facilitate a better use of health & social care resources for local people Improve access and outcomes for service users through seamless care provision Support greater flexibility for service delivery and redesign Facilitate integrated health and social care commissioning and integrated service provision Provide a framework for integrated commissioning Ensure a fit with each organisation s statutory duties Ensure value for money for health and social care monies through integrated commissioning 2

5 4 FUNCTIONS and RBK have identified project managers to deliver the Kingston at Home Programme. The services as listed in this Schedule are commissioned and contracted via Kingston s NHS Contract with. The K Commissioner for services will act as lead commissioner. Both organisations will ensure access to other professionals e.g. finance, information, communications, procurement to support delivery of the programme. 1 SERVICES The services covered by Kingston at Home statement of intent from the NHS standard contract 13/14 STATEMENT OF INTENT (extract from NHS Standard Contract 2013/14 - Particulars Between Kingston and Your Healthcare CIC) Service Descriptions Refer to the NHS Standard Contract Documentation 13/14 between Kingston and CIC. Service Users Health Services NHS healthcare is free at the point of contact. There is no eligibility criteria associated with the health services under Kingston at Home. Council Adult Care Services Access to Council Services for individuals is based on the eligibility criteria underpinning Fair Access to Community Services (FACS) which for RBK is critical and substantial For the reablement service FACS will not apply and will be free of charge up to six weeks 5

Set out what arrangements will apply upon termination of the Individual Service, including without limitation the following matters addressed in the main body of the Agreement (1) maintaining continuity of Services; (2) allocation and/or disposal of any equipment relating to the Individual Scheme; (3) responsibility for debts and on-going contracts; (4) responsibility for the continuance of contract arrangements with Service Providers (subject to the agreement of any Partner to continue contributing to the costs of the contract arrangements); (5) where appropriate, the responsibility for the sharing of the liabilities incurred by the Partners with the responsibility for commissioning the Services and/or the Host Partners.
