2 A New Day in Health Care Health care is changing. Gone are the days when we focus our care solely on patients who are already sick. To help patients get healthy and stay healthy, we know you need a team focused on helping you manage your health. To better express our focus on patient and family-centered care, Iowa Health System changed its name to UnityPoint Health. The name reflects the way the organization s hospitals, physicians and clinics and home care are reforming health care delivery through coordinated care. St. Luke s Regional Medical Center is now UnityPoint Health St. Luke s, St. Luke s Clinic Network is now UnityPoint Clinic and St. Luke s Home Care is now UnityPoint at Home. While our names have changed, our vision at UnityPoint Health - Sioux City is still focused on putting you at the center of everything we do from our clinics to the hospital and home, achieving the best outcome for every patient every time. UnityPoint Clinic, the employed physician group within UnityPoint Health, and UnityPoint at Home, the home care services of UnityPoint Health, work together with the hospitals of UnityPoint Health to ensure our patients are treated in the most effective, most efficient environment appropriate to their health care need. Our mission to improve the health of the community is still our top priority. UnityPoint Health St. Luke s Foundation continues to impact health care through Siouxlanders generosity. St. Luke s College UnityPoint Health remains on the campus of St. Luke s to shape future health care professionals. We are humbled by your support. And we are privileged to care for you and your loved ones. The past year has brought many exciting enhancements to UnityPoint Health, and we look forward to accomplishing additional milestones in the year ahead. Most importantly, we look forward to providing you with quality care tailored to your individual needs. UnityPoint Health. The point of unity is you. Peter W. 3 Service Braving the Storm Little Khloe Ryan spent ten days in the neonatal intensive care unit at UnityPoint Health St. Luke s. Thankfully, those days were relatively uneventful getting to the Neonatal Intensive Care Unit (NICU) however, was more of a journey. On a snowy February night, Hannah Dirks and Brendan Ryan of Rock Rapids, headed to the hospital in Sioux Center, Iowa, where daughter Khloe Emersyn was born. The umbilical cord was wrapped around her neck and she suffered a collapsed lung. The delivering doctor resuscitated her, but it was evident she needed the specialized care of a neonatal intensive care unit. The storm blowing outside though was getting worse. Our doctor called Sioux Falls, but nobody would come. St. Luke s in Sioux City was the only hospital that would come get her, says Hannah. We got the call and even though the weather conditions were poor, we knew this baby needed help, remembers Andrea Orr, neonatal nurse practitioner at St. Luke s. The transport team prepared for the trip. Orr, along with Jill Jeffords, a NICU transport nurse, and Michael Donovan a respiratory therapist, and two Siouxland Paramedics braved the storm. When they arrived in Sioux Center, Orr, with the help of the NICU transport team and paramedics, placed a chest tube in little Khloe to help stabilize her before heading back into the snow. During the drive to Sioux City, the paramedics contacted the Highway Patrol for an escort because it was so difficult to see the road, all while Khloe rested peacefully. It was a very long ride. I think we were only going 35 mph in the ambulance, says Brendan who rode with Khloe to Sioux City, while Hannah recovered. I was anxious to get there, but I wasn t worried at all. I knew they would get us there safely. Once at St. Luke s, Hannah and Brendan stayed by Khloe s side. All of the nurses were extremely nice, said Hannah. They were great at communicating what was happening. We are just very thankful for all they did for us. Khloe is now healthy, growing and happy. ANDREA ORR, Neonatal Nurse Practitioner holds one of the many newborns cared for each year in our NICU. NICU BY THE NUMBERS: Siouxland s ONLY 330 BABIES were cared for in 2012 NICU 30 to give little ones 24/7 In-hospital NICU TRANSPORTS in NEONATOLOGISTS AND 3NURSE PRACTITIONERS Provider Coverage

4 Collaboration Presenting Siouxland s Newest Cardiology Labs UnityPoint Health St. Luke s is excited to announce the near opening of a dedicated electrophysiology (EP) lab and a replacement cardiac catheterization lab. These two newly constructed labs are located conveniently next to one another in a new space on the main floor of the hospital, which will also house new pre-procedure and post-procedure cardiology rooms. This type of layout improves patient flow and in turn leads to a better patient experience. St. Luke s partners with the trusted physicians of Cardiovascular Associates (CVA) to provide care for Siouxland s heart patients. If it s a planned procedure, patients can be referred to St. Luke s Cardiology Services department for non-invasive tests such as EKGs, echocardiograms, stress tests, and holter monitors; invasive cath lab procedures such as heart catheterization, and stent placement; and EP procedures including defibrillators, ablations, and pacemaker implants. But sometimes, heart attacks happen fast. In an emergency, patients who choose St. Luke s are choosing lifesaving care. St. Luke s and CVA serve patients acute care heart needs 24 hours a day, 7 days a week. Located just steps from our emergency department (ED), the new interventional cardiology suite will result in enhanced door-to-balloon times for patients. This means less time is needed between the patient s arrival in the ED to when he/she is moved to the cardiac cath lab to receive heart care. In the end, this means less heart muscle will be lost and the patient s outcome will be better. Patients can rest assured when they put their hearts in our hands. Our experienced staff and the trusted cardiologists of CVA are not only working in the newest labs in town, they are also using the newest equipment too. State-of-the-art technology such as the Siemens Biplane, which is used in the EP lab, creates improved visualization, because multiple images of the heart can be seen at one time. These enhancements are part of St. Luke s and CVA s GENEROSITY HEALS HEARTS Enhancements like the new Cardiac Cath Lab and EP Lab could not be possible without kind donations and support from our community. We d like to thank Knife River for being a significant donor in this project. If you would like to make a contribution to support this project or other advancements in local health care, contact UnityPoint Health St. Luke s Foundation at (712) promise to work together to enhance cardiology services for Siouxland patients. 5 Medical Scare Has Siouxlander Feeling Lucky As Siouxlander and railroad conductor Lee Reed would say, luck was on his side. Unexpected blood clots after knee surgery led Reed to a visit with family physician Dr. DJ Todt of UnityPoint Clinic Family Medicine at Sergeant Bluff who diagnosed blood clots in Reed s lungs and thighs as Deep Vein Thrombosis (DVT). A serious condition, DVT can cause blood clots to break off and travel through the bloodstream damaging organs and even causing death if not treated promptly. Fortunately said Todt, a new endovascular surgeon, Dr. Wade Kang, had recently joined UnityPoint Clinic General and Vascular Surgery. Reed was referred to Dr. Kang for treatment of his DVT. While I was in a lot of pain at that point, I was also ready to get back to my life, so I was somewhat reluctant about having to see another doctor, recalls Reed. Reed s resistance changed to reassurance when he met Kang for the first time. When Dr. Kang walked into the patient room, he made sure I understood what was going on and why it was important to have it treated. I appreciated that he seemed to genuinely care about helping me get better, says Reed. Reed was admitted to UnityPoint Health St. Luke s where Kang performed percutaneous thrombectomy and thrombolysis, both non-invasive treatments for DVT. After the procedure, Dr. Kang came by to see how I was doing, even checking in with my nurse in the early morning hours to make sure my recovery was going smoothly, adds Reed. The care I received was outstanding. Everyone from staff in St. Luke s cath lab to the hospital s intensive care unit and fourth floor were great. Today, after a short hospital stay, Reed is grateful to return to work at the railroad. There were so many people committed to helping me get better. When I first met Dr. Kang, he told me how lucky I was that my DVT hadn t caused more damage, adds Reed. I am really lucky. My good fortune is because of the outstanding care I received at UnityPoint Health. 10 Quality PACE Program Offers New Option in Elderly Care A book could be written about Allen Martin s life. Filled with twists and turns, the Sioux City man has experienced his share of challenges. Today, with the assistance of Siouxland PACE, Martin is optimistic about life s next chapter. Only three years ago, Martin was home bound. A bout of polio as a young child had taken its toll on the former airplane mechanic. His deteriorating health not only confined him within the four walls of his home, it also impacted his life socially and emotionally. With his wife s encouragement, Martin took the first steps toward turning his life around and began attending PACE s therapeutic and recreational center. Continuum of Care Enhances Living Operated by UnityPoint Health St. Luke s, Siouxland PACE was the first PACE program approved in the State of Iowa. Siouxland PACE currently serves approximately 130 participants in four Iowa counties. The mission of PACE (Program of All-Inclusive Care for the Elderly) is to help individuals like Martin live as independently as possible at home and within the community through a wide range of services. To be eligible to enroll in PACE, participants must be 55 years or older, be determined by the State of Iowa to meet nursing facility level of care criteria and reside in the county service area of the PACE organization. In cases when an individual requires facility level of care, Siouxland PACE supports the participant with the transition and continues to provide care. Our program provides peace of mind to our participants and their loved ones. We provide a unique opportunity for our participants to work with a group of professionals all committed to their health and well-being, says Randy Ehlers, Director of Siouxland PACE. As part of St. Luke s, we can offer participants a wide continuum of services to meet their individual needs. A team of PACE professionals meets with each participant to determine which services can help maintain their overall health. While some participants may require only one or two Located at 309 Cook Street in Sioux City, UnityPoint Health St. Luke s Siouxland PACE serves residents of Woodbury, Plymouth, Cherokee and Monona counties. services, other participants take advantage of additional resources. Siouxland PACE offers a therapeutic and recreational center, transportation, rehabilitation and restorative therapies, durable medical equipment, dietary services, in-home support, medications and medical care and care coordination by the PACE clinic. Dr. Amy Callaghan serves as Siouxland PACE s medical director and primary care physician. Callaghan brings a wealth of expertise in internal medicine to PACE including experience in the hospital and clinic settings. Dr. Callaghan provides participants with a single health care resource. Her goal is to help participants meet their individual goals, adds Ehlers. If adjustments need to be made to a treatment plan, Dr. Callaghan will work with the participant and their interdisciplinary team to make those changes. For participants eligible for Medicaid only or dual eligible for both Medicare and Medicaid, there is no cost for the program. If an individual has Medicare but is not Medicaid eligible, there is a monthly fee for the program and a Part D premium. In cases when a participant does not receive either Medicaid or Medicare, the participant pays three fees to the program: Medicaid, Medicare and Part D premiums. Siouxland PACE s enrollment coordinator is available to assist those interested with additional information on program costs. The Best Place for Me Siouxland PACE continues to open new opportunities for Allen Martin who enjoys seeing his new PACE friends during his weekly visits to the therapeutic and recreational center. For someone who was initially reluctant to join the program, Martin now says there isn t a place he d rather be. Since joining Siouxland PACE, I m able to take better care of myself. I really enjoy the social aspect the program offers something that even surprises my wife, says Martin. Siouxland PACE has been an excellent resource for me. For more information, call (712) or

11 Electronic Medical Records Improve Your Patient Experience Thanks to EPIC, an electronic medical record system recently adopted by UnityPoint Health St. Luke s, patient information can securely be shared across the continuum of care. What does this mean for patients? Well the days of paper charts being floated from one doctor to the next are over. Now, a patient s records are conveniently and securely stored electronically. This means a physician with the proper security codes and passwords can log in from home, the clinic or the hospital to address questions and manage care. In addition, if a patient is transferred to another hospital department or another UnityPoint Health facility, there is no delay in waiting for a paper chart to be transferred too. Now the patient s chart is easily accessible in each department or care facility he or she is transferred to. EPIC also has advantages to how patient information is entered. There are sections of the software that can only be updated by physicians and other sections that are meant for hospital staff. This ensures each provider is updating all the necessary information in a timely fashion. And a side benefit is the electronic records are always legible, unlike some handwritten notes. Now that there is a computer in every patient room at St. Luke s, some may wonder if there is concern that anyone can log in and see patient information. Rest assured only health care professionals with proper passwords, codes and swipe badges will have access to the computers with EPIC records. The process for giving patients medication has also improved due to EPIC and the placement of computers in each patient room. Once a patient is admitted to St. Luke s he or she is given a wristband with a bar code. Then, when it is time for the patient to be given medicine, a nurse scans this wristband and then scans the medication so that the patient information is cross referenced with the prescriptions to prevent medical errors. Another perk to the EPIC system is that we can now give patients more access to be an active partner in their health. UnityPoint Health is excited to introduce a patient portal in our clinics and hospital. My UnityPoint TM has recently rolled out to all UnityPoint Clinics and UnityPoint hospitals. Why is EPIC better for Patients? Registered patients can securely complete the following tasks: Review hospital and clinic records View lab results Request an appointment Request prescription renewals Communicate with their primary health care team To learn more about MyUnityPoint patient portal or to register, visit unitypoint.org. Current medical information will be at your provider s fingertips no matter if you are at our hospital, one of our clinics, or even another UnityPoint Health hospital. Health information can be more efficiently shared between areas of the hospital that are involved in your care. We will now be using bar-code scanning technology for medications in order to better monitor safety in reducing and preventing medical errors. Improved privacy and security of your medical record.
