3 County: Mahoning (Bid Spec #1) Supportive Services Fiscal Year 2019 AAA funding: $13,842 Service definition: Providing services or assistance in obtaining Senior Center Services for older (60+) persons, enabling them to maintain independence in the community. Services provided under supportive services include: A) Assistance and advocacy in obtaining benefits or services through completion of necessary forms; e.g., Medicare, Medicaid, insurance, food stamps. Assistance with referrals and linkage of clients to appropriate community resources that could include scheduling initial appointments, assisting/participating with clients in scheduling appointments and follow-up (contacting clients and/or applying for public benefits). This does not include transporting clients to appointments. B) Assistance with or teaching skills in handling personal finances and budgeting. C) Language translation and/or interpretation for the purpose of assessing other services; e.g., medical appointments. D) Coordination with adult protective services. Unit of service: One hour of direct service to, or on behalf of, a client. The unit rate shall reflect only direct service to, or on behalf of clients. Calls lasting less than 8 minutes are not billable. Service Description: Provide supportive services for senior citizens, 60 years of age and older who live in Mahoning County. Unit Rate: Includes cost of facility, staffing, administration, travel and all other costs associated with service delivery. AAA Current Reimbursement Rate: $
4 County: Mahoning (Bid Spec #2) Personal Care Fiscal Year 2019 AAA funding: $ 25,535 Personal Care Service Definition: Service comprised of tasks that help a consumer achieve optimal functioning with ADL s and IADL s. Unit of service: One unit of service is one hour of personal care service. Service Description: Provide homemaker services or personal care services for frail and/or homebound elderly in Mahoning County. The activities include personal hygiene, housekeeping, laundry, essential shopping errands, and meal preparation. *Note: Each eligible consumer is only allowed 24 hours per month maximum. Unit Rate: Includes all facility, staffing, equipment, administrative, travel, and training costs including record documentation time. *Note: Travel time to and from job location is not billable as service time. AAA Current Reimbursement Rate: $
5 County: Mahoning (Bid Spec #3) Homemaker Fiscal Year 2019 AAA funding: $35,331 Service definition Service that provides routine tasks to help a consumer to achieve and maintain a clean, safe, and healthy environment. The activities include housekeeping, laundry, essential shopping errands, and meal preparation. Homemaker services do not include the provision of chore or personal care services. Unit of service: One hour of homemaker service. All time spent in performing the service. Service description: Provide homemaker services for frail and homebound seniors in Mahoning County. *Note: Each eligible consumer is only allowed 12 hours per month maximum. Unit Rate: Includes all facility, staffing, equipment, administrative, travel, and training costs including record documentation time. *Note: Travel time to and from job location is not billable as service time. 6 County: Mahoning (Bid Spec #4) Medication Management Fiscal Year 2019 AAA funding: $11,553 Service definition: A program organized to provide an individual with a basic medication evaluation and an evaluation of his/her present state of health in their home environment. The purpose of such program is to identify problems needing more indepth medical diagnosis, treatment and education services. Program functions include medication screening and management. The program also may include blood pressure checks, routine laboratory tests, vision and hearing tests, or other basic medical examinations. No treatment is to be included in reporting for this service category. Unit of service: One hour of assessment and education/training. Each individual person evaluated by a qualified medical professional equals one unit. Service description: Provide in-home health screening and education for health promotion. Must be designed to serve the medically at risk elderly in Mahoning County. AAA Current Reimbursement Rate: $
