1 Beaumont Health Foundation Descriptions of Charitable Funds For information on other funds, please contact Don Cohen at the Beaumont Health Foundation office, CLINICAL PROGRAMS AND SERVICES Beaumont Children s Funds will provide for the unmet needs of Beaumont Children s, supporting its mission of excellence in patient care, service, education, and the advancement of the discipline of pediatrics. Funds will be used throughout Beaumont Children s at Beaumont, Dearborn, Beaumont, Grosse Pointe, Beaumont, Royal Oak, and Beaumont, Troy Center for Exceptional Families The Center for Exceptional Families was created in an effort to meet the distinct medical and therapeutic needs of children with special needs or life threatening injuries as well as to provide educations, recreational and social support. With a family centered approach, the CEF works with moms, dads, siblings, teachers and other individuals who play a key role in a child s life to ensure that they have the resources they need to help the children reach their potential. Unique in scope and approach to care, the CEF is one of only a few places in the country where children and families have access to the best in class programs, educational, social and recreational support, a multidisciplinary medial team and comprehensive, compassionate care all under one roof Center for Human Development Your gift may provide special needs such as tutoring for dyslexic children and other learning disabilities help defray the cost of evaluations, purchase educational supplies, and provide patient resources for children with development disorders such as autism, emotional problems, and learning disorders Children s Miracle Network Beaumont Children s is the only Children s Miracle Network Hospital in Southeast Michigan. One-hundred percent of your contribution stays at Beaumont to fund leading-edge technology, life-saving pediatric programs, and special services for our youngest patients. Twice per year, a committee of Beaumont Children s leaders review grant requests from across the system and award funding based on need and available funds Children s Outreach Programs Supports the unmet needs of children s/pediatric programs at Beaumont Hospitals in Dearborn, Wayne and Trenton. Children s outreach programs include back to school programs for backpacks and school supplies at some of our clinics Children s Speech Treatment Scholarships Gifts support treatment costs for children whose families are unable to afford outpatient speech pathology services Digestive Health Contributions support vital patient care programs for children and adults with inflammatory bowel diseases Heart (Cardiology) Gifts support the unmet needs of the Heart and Vascular Center of Excellence, such as patient and staff education programs, equipment needs, and research to advance cardiac care and services. 2 Heart &Vascular Programs This initiative supports Heart and Vascular programs and services offered at Beaumont Hospitals in Dearborn, Trenton and Wayne. It includes education opportunities for nurses, physicians and clinical staff, cardiac rehabilitation programs for faster healing and recovery, prevention education focusing on lifestyle changes including diet and exercise and lifesaving medications for those unable to pay Heart & Vascular Fund Supports innovations, treatments and technologies needed for our heart patients at Beaumont, Farmington Hills Hospice The important services that Beaumont Hospice provides to patients and their families cannot be overstated. Offering comfort and peace during the final stages of an illness is truly a noble and worthy endeavor. Your gifts support the delivery of the highest quality end-of-life care for patients and bereavement support for families following the loss of their loved one, including individual counseling, telephone support calls, group support, and art therapy for children and teens prior to and following the death of a parent or sibling Langnas Memorial Laboratory Enrichment Fund Supports the professional development of laboratory staff at Beaumont, Farmington Hills Neonatal Intensive Care Unit, Beaumont, Troy To support the expansion of the Neonatal Intensive Care Unit (NICU) at Beaumont, Troy. Additional funding may be used for the unmet needs of the Beaumont, Troy NICU Neuroscience Gifts support the unmet needs of the Neuroscience Center of Excellence, such as patient and staff education programs and equipment to advance neuroscience care and services. Also may be used for Beaumont s stroke program, which includes leading-edge medical treatment, outstanding stroke rehabilitation services and stroke-prevention education programs for patients Oncology Services Program Gifts will help our outstanding team of physicians, oncologists and scientists continue to develop leading-edge treatment therapies and conduct important clinical trials supported by the National Cancer Institute. Funds will also support patient care programs such as the Cancer Survivorship Program, support groups, education activities and other patient needs Orthopedics Your gifts will support research, education, and new technology as well as other unmet needs of the Orthopedic department Pediatric Rehabilitation Scholarship Your gifts may support individual or group therapy costs for selected outpatient pediatric rehab (OT/PT) patients whose families are unable to afford needed services Physical Medicine & Rehabilitation Your support will allow for growth and expansion of a specialized inpatient rehabilitation facility and outpatient services, new and expanded programs and services for patients with traumatic brain injuries, orthopedic conditions and musculoskeletal disorders and those recovering from stroke and injury, cutting edge equipment, specialized training of medical residents, nurses, clinicians, and therapists, and community support, education, outreach, wellness and fitness programs Troy Cancer Program Your gifts offer those who have been diagnosed with cancer individualized care focusing on exercise, nutrition and support groups through Beaumont Hospital, Troy s Cancer Survivorship Program. 3 Women s Health Natural Childbirth and Breastfeeding Support Your gifts support natural childbirth and provide breastfeeding help to new mothers at your site. Women who have come to trust Beaumont for outstanding obstetrical, neonatal and pediatric care can now give birth naturally in a safe, welcoming and supportive environment surrounded by excellent physicians and specially-trained caregivers Women s Services This initiative supports expansion of Women s programs and services offered at Beaumont Hospitals in Dearborn, Trenton and Wayne. It includes enhancements to our comprehensive regional breast care program with cutting edge equipment, facilities and technology, personalized service through Nurse Navigation to support women on the journey from diagnosis to treatment to recovery. PATIENT SUPPORT PROGRAMS Alzheimer s Patient Support Supports programs to assist Alzheimer s patients and their families. This fund also supports the general needs of the program Bridge to Hospice Program The Bridge to Hospice program is a palliative consultation program for patients that are hospice criteria appropriate, but wish to continue with their curative care. These patients have advanced illnesses and are being transitioned to hospice. The focus of the program has been to offer free, community based services staffed by home health aides that support the philosophy of maintaining the patients in the home Cancer Center Patient Needs Fund Assists with health-related expenses not covered by insurance or other programs at Beaumont, Farmington Hills Cancer Patient Education & Support Assists with health-related expenses not covered by insurance or other programs at Beaumont, Dearborn Chapel Fund Grosse Pointe Gifts have helped refresh the 2 nd floor Hospice rooms for a nicer patient environment and continue to support the unmet needs of the pastoral care program Chapel Fund Royal Oak Gifts provide for the unmet needs of the spiritual care program at Beaumont, Royal Oak Clinical Ethics My Voice, My Choice is an advance directive package designed to help in naming a person you trust to be your medical advocate, in case of a medical emergency. Funds provide for materials and education Emergency/Trauma Center Patient Care Fund Supports the unique needs of our Emergency/Trauma Center patients at Beaumont, Farmington Hills Geriatrics Gifts will support the unmet needs of the Geriatrics program, such as patient and staff continuing education programs, equipment needs, and activities to advance geriatric care and services Imaging Services Gifts support the unmet needs of the Imaging program, including continuing educational initiatives for department employees. Many of the initiatives provided through this fund have no other source of funding charitable gifts are vital. Spirit of Giving, Description of Funds Page 3

4 Integrative Medicine Gifts provide general program support and financial assistance for many vital services not covered by insurance, including clinical massage therapy, energy balancing with Reiki, guided imagery, medical acupuncture, scar therapy, gentle yoga, meditation, and naturopathy for management and stress relief. Cancer patients have described the services as a little piece of paradise during an unpleasant time Moceri Learning Center Fund Equipment, supplies, education and any other needs of the Center Palliative Care To support the unmet needs of the Palliative Care department at Beaumont, Dearborn Patient Care Fund Offers services and items to keep our patients healthier and more comfortable at Beaumont, Farmington Hills Patient Resource Fund Your gifts are targeted to assist qualified patients in obtaining healthcare needs such as to have prescriptions filled for medications to aid in their recovery from an inpatient hospitalization or a medical condition or surgery. Staff screens patients for eligibility, which can be lack of insurance coverage, prohibitive co-pays or limited financial resources. These assistance funds are also available at the discretion of staff to cover other needs such as durable medical equipment, home health care services, IV antibiotics to be administered in a home setting, transportation, medical supplies and other types of care that the patient may not be able to access on their own Parenting Program Contributions help provide support and education for new parents, which helps families begin on a positive note with a strong parenting base, ultimately helping contribute to a healthier community. The Parenting program focuses on prevention strategies that are designed to strengthen the capacity of parents to care for their children s health and wellbeing. Since inception, this program has made a difference for more than 100,000 families. Specialty groups such as single moms, young moms, and multiple births receive dedicated support for their situation as well Petrovich SRNA Scholarships/Education Fund Scholarship awards and educational initiatives of the program supporting annual scholarships for qualified candidates who meet certain criteria Spiritual Support Services Spiritual Support Services ensures the emotional and spiritual needs of our patients are met. They offer support and comfort aligned with the patient s personal faith experience. The program supports the spiritual needs of our patients, families and staff, as well as communication education and outreach on the issues of grief, loss and end of life decision making at Beaumont Hospitals in Dearborn, Taylor, Trenton and Wayne Therapeutic Music Program To provide a therapeutic music program to all eight hospitals to include live therapeutic music for patients, families and employees that integrates with the BH strategic imperatives including patient and family-centered care and best-in-class quality, safety, service, and financial performance Weight Control Center Studies show that people who are most successful at losing weight and keeping it off rely on a team of professionals who offer medically supervised weight loss solutions and help. The multidisciplinary experts continue to provide patients with personalized weight loss solutions, helping them feel better and gain positive attitudes about eating and exercise. 5 EMPLOYEE SUPPORT PROGRAMS Beaumont Health Employee Assistance This initiative is established to lend a helping hand to employees in their time of greatest need as the result of a personal crisis or catastrophic event. Since its inception, the fund has helped numerous employees through both personal and financial crisis situations. Your gift can be used to help employees at your site Beaumont Health Employee Education This fund is established to provide continuing education assistance to Beaumont Health employees. While our community donors have created numerous scholarship endowments, this annual fund is truly employees helping employees. These funds help Beaumont to enhance recruitment, provide retention and create career advancement opportunities for employees Bwell Your Employee Wellness Program Designate a gift that creates conditions for employee well-being and a healthy culture at work. Bwell Implements innovative employee wellness initiatives and programs throughout the year. HEALTHY COMMUNITIES Adolescent/Teen Health Centers Services provided at the Teen Centers located in Taylor, Inkster, Romulus and River Rouge focus on health promotion and risk reduction, along with general medical care, counseling services, immunizations, and management of chronic illnesses for adolescents. Programs that are cornerstones to the type of outreach and care given by staff are its peer education, after school and summer programs for youth, and counseling and testing programs Beaumont Healthy Communities Healthy Communities is a place-based initiative that encourages residents to lead healthier lives - and provides resources to help. Its integrated framework coordinates primary healthcare, public health and community interventions; and emphasizes a shared responsibility for community health, while encouraging residents to take active roles in their own health Community Benefit Programs Community outreach initiatives that align with the community health needs assessment. These vital programs and services benefit patients, employees and community members throughout our entire service area. Programs include healthy communities, heart health screenings, diabetes education and obesity prevention. The fund also support other community benefit programs including needy patient programs Community Health Education & Screenings Fund Provides programs and services to address the needs identified in the Community Health Needs Assessment which include obesity, cardiovascular disease and diabetes Ingber Africa/International Trip Your gifts will help to change and improve the lives of numerous people in Africa or other impoverished destinations. Each year, Drs. Peters and Sirls and their team have taken life-changing medical treatment to the men, women and children of Africa, while facilitating an invaluable learning experience for Beaumont urology residents Minority Outreach Program Gifts help Beaumont s Oncology Services host educational forums in targeted minority and underserved communities that include information about screening, prevention and treatment of breast, prostate, colorectal, and lung cancers. In addition, mammograms and other cancer screening tests are provided, raising prevention awareness to save lives. This past year, the program also included prescreening, education and prevention events on diabetes, weight loss, smoking cessation, and behavioral health. Participants are given resources and assistance to seek further medical care NoBLE Anti-bullying Program Your gifts to NoBLE (No Bullying Live Empowered) provide community outreach programs and support to children, Families and schools surrounding bullying related incidences. NoBLE is also focused on engaging and unifying healthcare providers in bullying prevention/intervention efforts on a national level. Spirit of Giving, Description of Funds Page 6

7 Older Adult Services/Helping Hands Gifts help subsidize transportation to medical appointments and treatment, and also companion services for financially disadvantaged patients in the communities we serve. The program helps seniors (most are over the age of 80) who are trying to maintain independence at home and can t afford these much-needed services Student Heart Check Every year high school students die from sudden cardiac arrest. These deaths can be due to abnormal heart structure or abnormal rhythms that can lead to cardiac arrest. To help young people detect their risk for sudden cardiac arrest, Beaumont holds free community screenings on an ongoing basis. Your gifts will save lives Summer Speech/Patient Assistance, Wayne To support needy patients and families at Beaumont Hospital, Wayne -- including the Summer Speech Program which allows students to continue receiving needed speech assistance when school programs are not operating.
