1 A proud tradition of excellence. VNA of Hudson Valley is the area s largest provider of home healthcare services, headquartered in Westchester County. The Visiting Nurse Association of Hudson Valley s inspiration came from Ellen Wood, a young Mount Kisco resident who saw women in her family caring for sick neighbors and became determined to get the proper training to carry on this noble tradition. In 1896, she graduated first in her class at Johns Hopkins Training School for Nurses and quickly applied her skills during the Spanish-American War. Back in Mount Kisco in 1898, Mrs. Henry Marquand organized local women into an auxiliary of the American Red Cross Relief Committee. Their purpose was to gather surgical and hospital supplies for wounded soldiers care. When peace came that Fall, Ellen Wood joined the Red Cross Relief Committee as their first nurse, saying Now we have done so much to provide nursing care for the soldiers, why cannot we do the same for our people at home? On November 15, 1898, the Red Cross Relief Committee became the District Nursing Association of Northern Westchester the ancestor of the VNA of Hudson Valley. Quality, compassionate home health care since Things have obviously changed enormously at the VNA of Hudson Valley from the days of nurses on foot to today s thoroughly modern practitioners. But the unifying theme has always remained the same care, compassion and commitment. Miss Wood exemplified our approach to providing the finest care at home more than a century ago, and we are proud to carry on that tradition today.
2 Delivering a broad array of nursing, rehabilitation, therapy and social work services. We assist individuals of every age with compassionate care in the comfort and familiarity of their own homes. Offering comprehensive, compassionate services to patients with advanced or life-threatening illnesses and their families. We enable individuals to remain at home with quality of life and physical comfort. Our Mission VNA of Hudson Valley is a family of organizations sharing a mission: To provide quality healthcare to all people in their communities, regardless of their ability to pay, in a manner that recognizes the whole person and their environment. A primary focus is to maximize the resources of the organization for the benefit of the patient. Providing care to individuals who need help to maintain their independence and remain in their homes. We coordinate and deliver a wide variety of services that assist with the activities of daily life. Encouraging community awareness and philanthropic support for the mission of the VNA of Hudson Valley family of organizations. The family of organizations that make up the VNA of Hudson Valley strives to foster independence and choice for all individuals with the overall goal of improving the quality of life by assuming a proactive advocate role.

We are a multi-faceted organization that offers and coordinates a broad array of home health services which are available to all patients who can be treated at home instead of a hospital, or who need follow-up care after a hospital stay. We care for people of all ages from infants to seniors 24 hours a day, 7 days a week. Our registered nurses provide skilled intermittent nursing services and coordinate the plan of care ordered by the attending physician. This plan of care may incorporate certified home health aides, speech language pathologists, physical or occupational therapists, medical social workers, mental health home care from psychiatric nurse clinicians, and maternal child health programs. The highly skilled people who make up our staff are dedicated to the individuals they serve. The outstanding quality of our services has been validated with accreditation from the Joint Commission on Accreditation of Healthcare Organizations. VNA of Hudson Valley is a free-standing, not-for-profit, certified home healthcare agency that has earned the trust of the community through over a century of service. Our mission was then as it is today to provide compassionate care to all those who need it, regardless of their ability to pay. We care for people of all ages, from infants to seniors, delivering services 24 hours a day, 7 days a week.

Hospice care is a way of living with dignity, hope and support during advanced or life-threatening illness. Hospice Care in Westchester & Putnam focuses on the medical, emotional, and spiritual needs of the individual and their family through the assistance and care of nurses, social workers, other professionals and trained volunteers. Hospice enables patients to remain at home with quality of life and physical comfort. We coordinate all services including hospital care, if needed, so that continuity of care is maintained 24 hours a day, 7 days a week. Your staff helped me give my father the best gift I had to offer, a loving and peaceful end of life in his home. Thank you for your support, allowing him to live his last days with peace and dignity. M.R. We provide palliative care, the expert treatment of pain and other symptoms. We also enhance our core hospice services with a broad-based Healing Arts Program that promotes pain management, relaxation and improved quality of life with holistic therapies including therapeutic massage, shiatsu massage, art, music and pet therapies, among others. Hospice is for the family, too, with special services for adults, teens and children to support them in managing stress and coping with their loved one s illness. Hospice Care in Westchester & Putnam is JCAHO accredited, fully licensed and certified. We are the only hospice provider in Putnam County, and the largest not-for-profit organization providing hospice services in the area. We enhance our services by promoting pain management, relaxation and improved quality of life therapies.
5 VNA Home Health Services Programs: Care for individuals who need help in their homes Needs are assessed by a registered nurse Companions/ homemakers Rehabilitation services and respite care Personal emergency response system partner Care Plus Program to meet all non-medical needs Personalized services. VNA Home Health Services helps individuals remain in their homes, with the aim of promoting self-care and independence. The professional staff at VNA Home Health Services will develop a plan of care specifically designed to assist patients with health-related tasks and therapy, as well as the activities of daily living. Our Home Health Aide was everything we could have dreamed of she was well trained, compassionate and caring. My wife and I came to trust her within a very short time and she became a valued partner in our plan of care. S.A. Registered nurses provide skilled nursing services and coordinate the plan of care ordered by the attending physician. Certified Home Health Aides can provide personal care, perform health-related tasks and assist with activities of daily living. Companions provide homemaking and other non-medical services. Physical Therapists may also visit the home to facilitate exercise programs or to train the patient in the use of special equipment to restore muscle and joint function. All providers of care are highly skilled and screened through a series of reference and background checks, validation of credentials and our own interview process which establishes the providers reliability and genuine concern for our patients well-being. As a not-for-profit organization, VNA Home Health Services bases its fee on the cost of services provided. Direct payment, Medicaid and third party reimbursement is accepted for services. VNA Home Health Services is fully insured and licensed by the State of New York Department of Health, and JCAHO accredited. Our providers are highly skilled, credentialed, and genuinely concerned for their patients well-being.
