2 SeSSion descriptions KEyNoTE ADDRESS Understanding Case Management from a Physician s Perspective Ronald Hirsch, MD, FACP Medical Director, Case Management Sherman Hospital Elgin, IL SESSIoN ABSTRACT: A physician advisor from a community hospital will discuss his approach to case management and offer insights into key measures to educate and improve relations with physicians, reduce length of stay and lessen the risk of denials. The session will provide a practicing physician s perspective on the current environment of decreasing reimbursement among increased scrutiny, and share views on what practicing physicians and physician advisors expect from their case managers. SESSIoN 2A The Importance of Partnership: Palliative Medicine & Case Management Danielle J. Doberman, MD, MPH Medical Director, Palliative Medicine Program Greater Baltimore Medical Center Baltimore, MD SESSIoN ABSTRACT: Palliative medicine is a sub-specialty of internal medicine which focuses on relief of distressing physical, emotional and spiritual symptoms in patients with chronic and life-limiting illnesses. Interdisciplinary palliative medicine teams are increasingly common in small community hospitals and large academic medical centers, and growing as an out-patient clinic offering. It is vital for members of the palliative medicine team and the case management team to work as partners. This session will discuss patient populations who might benefit from palliative medicine and define an optimal relationship between case managers and the palliative medicine team. SESSIoN 2B Committing to Best Practice: A Health System s Journey to Reduce Preventable Readmissions Through an Implementation Bundle Amy Deutschendorf, MS, RN, ACNS-BC Sr. Director, Utilization/Clinical Resource Management Johns Hopkins Health System Baltimore, MD SESSIoN ABSTRACT: This session will focus on the current evidence around practices to reduce preventable readmissions and the processes required to implement a care coordination bundle of interventions across a healthcare system. The speaker will discuss the selection of strategies, implementation plans and outcomes measures.

3 SESSIoN 3 How Strong Utilization Review Committees Can Prevent Inappropriate Hospital Admissions Steven Meyerson, MD VP of Corporate Communication & Compliance Accretive Health Chicago, IL SESSIoN ABSTRACT: By building a utilization review committee that meets regulation requirements, hospitals can reduce the risk of regulatory scrutiny and ultimately denials. According to 42 CFR , the hospital must have in effect a utilization review plan that provides for review of services furnished by the institution and by members of the medical staff to patients entitled to benefits under the Medicare and Medicaid programs. This session will explore how Medicare s Conditions of Participation dictate the requirements for utilization review committees. The speaker will provide recommendations for members of the utilization review committee and discuss the role of physician advisors as peer reviewers. SESSIoN 4 Making the Business Case for Quality Improvement William J. Ward, Jr., MBA Associate Professor of Public Health and Nursing Director, Master of Health Administration Degree Health Policy and Management Johns Hopkins Bloomberg School of Public Health Baltimore, MD SESSIoN ABSTRACT: It is almost intuitive to think that improvements in quality can result in improved financial performance; every other industry has embraced this principle for many years. In health care, however, the need to make the business case for any quality initiative still exists. Those involved in such initiatives must be able to articulate the positive financial performance outcomes that accrue to such quality improvements. This session will help clinicians and others involved in quality improvement initiatives understand the connections between quality improvement and financial outcomes, the four principal effects of quality improvement (cost efficiency, cost reduction, throughput improvement and revenue enhancement) and how to make the business case. Discharge challenges and resource demands far exceed current capacity. The speaker will discuss co-occurring disorders and the challenges associated with treatment and recidivism.
