1 Child Health Service Access and Entry Criteria The Child Health Service has 60 resourced inpatient cots or beds for infants, children and young people up to 16 years of age. The Child Health Service has a Neonatal Intensive Care Unit for infants requiring care immediately after birth and there are two inpatient wards. Ward One is for children aged 0-6 years of age Ward Two is for children aged 6 to 16 years. The Children s Ambulatory Service includes an Acute Assessment Unit, a Day Stay ward, a Paediatric Outpatient Service and a Community Paediatric Nursing Team based in Wellington and extends to the Kapiti Coast. There is an Acute Assessment and Outpatient facility at Kenepuru, and Outpatient clinics at Kapiti. The Child Health Service also includes the Child Development Team and the Regional Genetics Service. The attached appendix outlines the specific Child Health Service areas, age of the child that the service is for, specific access and eligibility criteria details, the referral process and when the service is available. Neonatal Home Care Team NICU Transport Team Discharge from NICU Birth and on occasion sick children Neonates are accepted with both medical and/or surgical conditions. Wellington Region only Monday to Saturday pm Phone triage occurs afterhours and the phone is held by the NICU ACNM from 4.30pm and Sundays. Lower North Island for tertiary level Other tertiary regions on request. Complex haematological infants Discharges from NICU requiring transition to home. Complex infants requiring ongoing support for feeding, oxygen therapy and / or palliative Tertiary retrievals and secondary / primary returns to other NICUs within the region Referrals are accepted from LMCs and Delivery Suite. Regional referrals go via the neonatal consultant. Bridging the Gap Discharge form is completed by NICU staff and forwarded to Neonatal Homecare Team. Referrals are verbal and occur at a consultant to constant level. 24/7 service Page 2 of 6

Ward One 0 6 years old Lower North Island for tertiary level Wellington region - primary, secondary and tertiary level care Paediatric Medicine - acute and children with respiratory, nephrology, cardiology, neurology, gastrointestinal, and endocrine disorders. Paediatric Surgery acute and children requiring: Ear, nose and throat General surgical Neurosurgical and /or Orthopaedic surgery Children are admitted acutely via the Emergency Department, or the Wellington / Kenepuru acute assessment unit. Surgical elective admissions are chosen by the consultant and arranged via booking clerks. Medical elective admissions are arranged by the consultant or RMO. 24/7 Ward One Isolation 0-15 years Lower North Island for tertiary level Wellington region - primary, secondary and tertiary level care All children needing admitting to a paediatric inpatient ward requiring isolation. Priority patients for the isolation unit are those requiring airborne isolation precautions, such as for TB and chicken pox. If there are more patients than the 5 bed unit can take, contact isolation can be managed on either ward in single rooms Ward Two 6 16 years Paediatric Medicine acute and children with respiratory, Children are admitted acutely via the Emergency Department, or the Wellington / Kenepuru acute assessment unit. Elective admissions are arranged by the consultant or RMO. 24/7 Page 3 of 6

4 Ward Two Paediatric Oncology Ward Two Oncology Day Case Unit Child Development Team nephrology, cardiology, neurology, gastrointestinal, endocrine and eating disorders. Paediatric Surgery - acute and children requiring: Ear, nose and throat General surgical Neurosurgical and /or Orthopaedic surgery. Tertiary services are provided to children from the Hawkes Bay, Whanganui and Mid Central DHB regions 0 16 years All suspected and new oncology diagnoses are admitted and treated in collaboration with CHOC years Monday Friday pm 0-16yrs Puketiro Centre, Kenepuru and Ewart. Monday to Friday pm Oncology children who require chemotherapy and supportive Provide a wide range of assessment, diagnostic and therapeutic services for children with both physical and intellectual disabilities living in the CC DHB catchment area. Information cards are given to the parents of all oncology children stating what to do and who to contact after hours if they are concerned about their child. Referrals must be from a health care professional. Where complex children are referred, from another DHB, for Oncology children who become unwell after hours are able to contact the paediatric registrar directly. If the registrar thinks that the child needs to be admitted the registrar will contact Ward Two and let the staff know. The child can come straight to the ward. Page 4 of 6

Children s Outpatient Services (Medical) Children s Outpatient Services (Nurse Led) Children s Acute Assessment Unit (Wellington) 0 16 years Delivered in Wellington and Kenepuru Hospitals; and the Kapiti Health Centre years Wellington and Kenepuru Hospital Clinics are also held at Oro Toa and Strathmore 0 16 years Monday to Friday hours Visiting paediatric specialists provide oncology, cardiology, neurology, and respiratory gastroenterology and nephrology clinics. Booked Nurse procedures Nurse led eczema and continence clinics GP / inpatient referral / other service providers i.e. CYFS / Public health inpatient referral GP referral / ED accepted by RMO Children s Acute Assessment Unit (Kenepuru) Community Nursing Team 0 16 years Monday to Friday hours 0 16 years Kenepuru and Wellington bases Entry criteria, referral and response timeframes will be as per the overarching Specialist Community Nursing Service specification which is based on health status risk. GP referral / ED accepted by RMO The children and young people considered to be of a higher Page 5 of 6
