Primary focus will be the emerging role of the care manager within the interdisciplinary team and shift from acute inpatient care transitions to pre-operative discharge planning. Describe efforts at the Federal level to improve the service delivery system. 3. Identify at least one innovative initiative that may have transferability to the participant s local provider/community.
