Primary focus will be the emerging role of the care manager within the interdisciplinary team and shift from acute inpatient care transitions to pre-operative discharge planning. LEARNING OBJECTIVES: 1. Describe the shift from acute inpatient care transitions to preoperative care planning for elective surgeries. 2. Discuss the development and application of discharge planning tools. 3. Review patient and family education approaches within a shared decision model. Integrate palliative care concepts within case management practice.

Review best practice rounding processes including scripting and time management and discuss how to hardwire your rounding process so that it remains timely and efficient. We ll also review the roles of each team member and how to involve the patient and family in the bedside rounding process. Get tips for effective documentation and a documentation template. Receive strategies to take back to your facility to evaluate your rounding process through outcomes metrics. LEARNING OBJECTIVES: 1. Define best practice interdisciplinary care rounds. 2. Review the roles of each team member on rounds. 3. Discuss the outcomes that can be achieved using interdisciplinary rounds. Discuss efforts underway to ensure individuals in need of services are able to receive the right care, at the right time in the right setting. Collaborative models for innovative change will be featured. LEARNING OBJECTIVES: 1. List at least one policy that impedes access to care. 2. Describe efforts at the Federal level to improve the service delivery system. 3. Identify at least one innovative initiative that may have transferability to the participant s local provider/community.

Strategies can be applied to achieve excellence combined with fun and humor to improve productivity, create better working relationships, enhance creativity, change the workplace culture and lead to the delivery of better services. LEARNING OBJECTIVES: 1. Recognize that the time in our lives is limited; thus, we must make the most of every day. 2. Manage stress by changing your perspective and seeing the humor that s all around you. 3. Change the processes in life and work by improving the steps.
