1 Case study of Chelladurai ( ) OP No.2757 Patient personal details CASE STUDIES Name Age : Thiru. Chelladurai : 60 yrs Name of village : Erendellaiparai OP No : 2757 Date of visit : Complaints reported The patient was taken to RHC with the complaints of chest pain, palpitation & sweating over the body by his wife. He was checked by the doctor immediately. The blood pressure and pulse were checked up. He was not a diabetic patient. He said that he felt pain over the chest and left hand. The patient was taken to RHC As he had the symptoms for heart attack, first aid medicine was given to get relief from the pain. After some time, the pain was reduced. His wife and his relatives were asked to take them to hospital at Dindigul city which was 6 kms away from the center. The patient was taken immediately to the hospital. ECG was taken at the hospital and it was diagnosed as mild heart attack. He was admitted in the hospital for 3 days. He got discharged from the hospital. He came back to RHC with all the medical reports. Doctor advised him to take care of his health and motivated him follow the diet and exercise. Follow up The patient came to RHC twice after the treatment. Now, he is following the advice given by the RHC doctor and he feels better. He is under regular medication. He was also advised to go for follow up at govt. hospital. As the ECG facility was available at RHC, it would have taken at the RHC itself. He is keeping well now and advised for regular medication and follow up.
2 Case study of Karthik, 24/07/2017 (OP No. 3628) Karthik, 21 yrs old male was taken by his friends who worked together in a company. He got a cut injury over his forehead and started bleeding. The patient was asked to lie down and the staff nurse of rural health center attended the patient. The cut injury was cleaned with antibiotic. The cut injury was sutured and dressed after giving local anesthesia. The cut injury was sutured and stopped bleeding. Doctor prescribed medicines and advised him to come for follow up. He was under regular medication. He came of follow up and dressed the injury for one week continuously. The suture was removed after one week and the injury was cured. He was on regular medication and doctors advice. He is doing his regular work without any trouble. He and his friends thanked the whole team of RHC. 3 Case study of Mr. Ammachiammal (Name changed) (July 2017) My Name is Ammachiammal and I am 70 yrs old now, married. I am residing at Rettiyapatty Village. I had a severe cold, cough and palpitation on my chest for the past one year. Initially treatment at Govt. Head quarters hospital for the said complaints. But my illness was not treated. As I had an increased pulse rate, I went and took treatment at private hospital. Later, I came to RHC at Erendellaiparai for treatment. As my blood pressure was gone up, I got fainted and fell down. I got an injury over my head. The wound was dressed up properly at RHC. I went for follow up regularly to the hospital. I was asked to come for a follow-up. I went again for a checkup. When I was checked up by doctor, my heart beat was high. Doctor asked me to go for ECHO at GH. I met the doctor again. Doctor prescribed me some mild drugs and asked me for taking medicines regularly. I felt that I did not have any pain over my chest and I am good now. I am taking medicines properly. I am so happy about my health condition and my sincere thanks to the doctor, Nurse and all the staff at the health centre of COST Trust. Case Study of Mrs. Mary (Name changed) (July 2017) (OP No. 3292) My name is Mary (name changed), 55 yrs old and I am residing at Kallathupatti village. I had a severe headache, body pain and piles complaint. I took treatment at nearby hospital at kallaathupatti. I had this problem now and then. I got fainting sensation for some time. When I went to Rendellaiparai villages to see my relatives, I got to know about the hospital. I met the doctor and explained my health complaints. Doctor checked my health thoroughly and prescribed medicines. He also advised me to take the medicine regularly and for proper diet. She also asked me to come for a regular check up. I came to the hospital for follow up on I got relief for my head ache. My blood pressure also came down to normal. I was given injection for my pain. Later, doctor advised me to take lab investigations to confirm diabetes. As it was diagnosed as diabetes, she advised me the diet pattern along with some exercise. Now, my glucose levels have come down and feel good now. I am taking the medicines regularly with walking in the morning every day. By god s grace, I was treated at COST Trust. They showed me care and support which was helpful to treat my illness. Thanks to the trust hospital run by COST Trust.
4 Case study of Mrs. Nandhini (Name changed) (May 2017) OP No My Name is Nandhini and I am 27 yrs old, married and residing at srilankan refugee camp at Adiyanuthu village. I underwent caesarean delivery recently and have male baby. Pus had been formed over the operated/sutured area. Later, I heard through my relative about health centre run by COST Trust at Erendellaiparai. I stepped into the centre for my pain and the doctor at the centre checked me thoroughly. Doctor had injected the antibiotics for my pain and dressed my wound in the sutured area. Doctor also advised to take medication regularly and to take rest at home. I again visited the centre on as follow-up. The pus over the area was totally cured and there was no pain in the abdominal area. The suture was removed at the centre. I was given regular drugs and medicines and I followed the same. Now, I had no pain in the area and I am happy now. I thank the hospital. Case study of Mr. Arumugam (Name changed) (May 2017) OP No. 119 My name is Arumugam (name changed), 63 yrs old, s/o of subbiah and I am residing at SP Thottam village. I had a severe cough, cold and body pain for the past two years. I was taking treatment from at Govt. Hospital, Dindigul. I had breathing trouble now and then. My cough was not yet treated at the hospital. Then, I was taking treatment at the private hospital. While on treatment, I feel that I am normal. But my cold and cough were not treated fully. I came to know the health centre running at Erendellaiparai village. I went to the hospital and took treatment for my illness. Doctor at the centre checked me thoroughly. He prescribed the following medicine. There, I was diagnosed as Asthma and the following medicine were given. I was asked to come for a follow up after one month. I went for follow up to the centre. Then the medicine was changed and to take the medicine regularly. The medicines were prescribed as below. Then, doctor asked me to take test such as chest x -ray, blood test, BC, TC, Sputum test. The chest x ray was normal and the sputum test was negative. I took treatment every day along with antibiotics. I was also advised to follow proper diet, to take hot water etc. As I took treatment regularly and followed the advice by doctor at the centre, my cough and cold were treated now. I am so happy now and I can do my work without any trouble. Now. I feel better and I would like to thank for the doctor and the COST hospital. Case study of Mrs. Nandan (Name changed) (June 2017) OP No My Name is Nandan and I am 46 yrs old, married and residing at Konapatty village and I am working in a private company. While I was working in the unit, he found a green whip snake and he feared that the snake was moved over his hands. But I felt that I was bitten by snake and I was feared so much. I had palpitation and got fainted. The fellow employees took me immediately to the private hospital. Though I got treated at the hospital, but I did not relieve form my complaints of fear and palpitation. He was afraid of his health and a job. One of my relatives referred me about the health centre at Erendellaiparai village. I rushed to the health centre on the second day and met the doctor at the centre. Dr. Zakkariah, our Medical officer checked me thoroughly and gave me medicines and injections. Doctor also counseled me for my fear. I visited the health centre twice in a week and followed the medicine given by doctor. The nurses also advised me to take vitamin tablets regularly. My blood pressure was also high. After coming to the hospital, I felt relaxed and normal
5 mood. Now, my palpitation and fearness has come down and feel normal. I am happy now. I thank the hospital, doctor and nurses. Case study of Mrs. Koyal (Name changed) (June 2017) OP No My name is Koyal (name changed), 27 yrs old, and I am residing at Kosavapatty village. I got injured over my head due to tin hit while doing my work and there was over bleeding on my head. I came to primary health centre which nearer to my village. Doctor at the PHC gave me some tablets but my bleeding was not stopped. My friend asked me to go the health centre at Erendellaiparai run by COST Trust. I could not even open my mouth due to severe bleeding. My cut area was sutured by nurse at the health centre and dressed immediately. Doctor asked me the details about the incidence. I explained him and checked my health and asked me to lay bed for some time to relieve my pain. I was given injection and medicines for my pain. The bleeding was totally stopped and pain was slowly reduced. I was asked to come for a follow up every week for a month. On 15 th June 17, I came back to health centre and took away the suture. The injection and ointment were given at the centre. I followed the doctor s advice and my injury was cured. I am so happy now and I can do my work without any trouble. Now. I feel better and I would like to thank for the doctor and the COST hospital.
