VLBW neonates have more health and growth problems than normal, full-term neonates. These problems are more common in male Black VLBW infants. They also experience less positive mother-infant interactions, especially when mothers are emotionally distressed. Unmarried Black mothers with less education have reported more mental health problems. This is a significant problem because maternal mental health functions as an important protective factor against the negative health and growth outcomes associated with prematurity. The source of the vulnerability of male Black VLBW infants and Black mothers must go beyond simple gender and ethnic socialization differences. We firmly believe that there are biological factors. Our ultimate research goal is to identify this biomarker to be testosterone. The state of Nevada enjoys the highest level of diversity in the nation, showing the rates of low birthweight birth among Black (13.9%) and Asian (11.7%) significantly higher than White (9.2%) and Hispanic (9.6 %). We thus believe that Nevada is an ideal place to recruit relevant study population. We will use a comparative and longitudinal research design. Forty mother-vlbw neonate pairs will be recruited from a Neonatal Intensive Care Unit at Summerline and Sunrise hospitals in Las Vegas. With maternal consent, data will be collected by the research nurse and nursing students from medical records, interview with mothers for the data that are not available in the medical records, standardized questionnaires for maternal mental health status, and biochemical measurement of testosterone and cortisol levels using either EIA or LC/MS at birth and 40 weeks postmenstrual age (PMA).

4 CHO CV_4 Project Number: R01HD Status on Grant: Principal Investigator Approved Project Period: 06/01/ /30/2018 % Effort: 25% PI: June Cho Source: NICHD Annual Direct Costs: $257,515 Title of Project: Testosterone and Cortisol Levels in Infant Health and Development Project Goal: This longitudinal study was designed to assess the significance of testosterone and cortisol levels between genders in: (1) infant health and growth, (2) mother-infant interactions, and (3) infant cognitive/motor/language development among very low birthweight (VLBW, birth weight less than 1,500g) infants through the first two years of life corrected for prematurity. This collaborative study involves a multidisciplinary team comprised of an academic research nurse investigator, neonatologist, pediatric endocrinologist, developmental-behavioral pediatrician, developmental psychologist, and pediatric nurse. The team members will integrate biochemical (laboratory research), behavioral (observational method), and developmental science. A long-term goal of the study is to identify the role of testosterone and cortisol in health and development of VLBW infants. Project Number: R21HD Status on Grant: Principal Investigator Approved Project Period: 07/01/ /30/2012 % Effort: 25% PI: June Cho Source: NICHD Annual Direct Costs: $137,500 Title of Project: Perinatal Testosterone in Infant Health, Mother-Infant Interactions and Development Project Goal: In this exploratory study we investigated the levels of testosterone and cortisol as biological and environmental factors affecting gender differences in VLBW infants. The saliva samples at postnatal period (before 40 weeks of postmenstrual age) were collected from 62 mother-vlbw infant pairs to examine the associations of the salivary testosterone and cortisol levels with: (1) neonatal health and growth outcomes, (2) mother-vlbw infant interactions, and (3) infant cognitive/motor/language development up to 6-month corrected age for prematurity. Maternal depressive symptoms and infant socioemotional problems, characteristics of mothers and infants, and days of saliva collection after birth were statistically adjusted. The latter is

We hypothesized that many neonatal health and growth problems are associated with increased levels of perinatal testosterone in both male and female genders. The sample size was over 10,000. It included all different birthweights (normal birthweight, low birthweight, and very low birthweight) of infants.
