Thank you all for the warm memories new and old we shared during the Melanie DuPont, Administrator Holiday Season. Our community and home is the better for each of you! Our mantra for 2016 is Care. Commitment. Service. We each choose to do work that allows us to care for others. We serve others. I challenge each of us to commit to the best service and care we can give. I look forward to all we will accomplish together as care for one another with commitment to service. And we have fun while doing it! 2 resident. We attempt to identify individual social and emotional needs. We inform residents and/or their representatives of the residents' rights. We promote the preservation of resident's rights through staff education and training. We provide social services information to the interdisciplinary team as part of the total plan of care. We facilitate family meetings, care plan meetings and discharge planning as well as documentation of these and other events pertaining to the resident. As the Social Services Department of Lake City Nursing and Rehabilitation, Our goal for 2016 is as it always is, to strive to be a vital part of each resident's quality of life. Your Social Services Department goal is to assist and assure that from the time of admission to discharge the resident receives the best quality of care and maintain as much of their normal lifestyle as possible. In order to achieve this goal, family involvement is a must. There is constant contact with the resident and /or their family for information, encouragement and to offer support when needed during this adjustment time for both. Social Workers are required to have a four year college degree in Social Work or related filed in a facility with more than 120 beds. There are specific State Regulations related directly to Social Services such as dignity, privacy, grievances, mail (all residents rights) and it is the Social Workers responsibility to make sure that all are adhered to. Social Workers are honored for the month of March annually. Special thanks to our Administrative officials for providing the educational opportunities to the Social Work Team through training that helps increase our knowledge base as we continue to strive to better serve our residents and/or their families. Nursing News Cathy LaPrade, Director of Nursing Services and Liz Perez, Assistant Director of Nursing Services, would like to welcome all of her new staff to the team. This is an exciting time for Lake City, we are so honored that you chose us to be your home away from home. Our most valuable asset is our residents and staff who are due our highest degree of respect, support and cooperation. So I welcome you all to assist me to pave the way to greatness. Activity Professionals Creating Smiles for Miles. I want to live while I'm still living Those are the words of Mr. Levie Kelley, a 3 year resident at Lake City Nursing and Rehab. Center. He is the reason why I took on the role as Activities Director in March of I knew that it would be a challenge but it was worth it. This past year I have not only seen the activity department grow but I have seen the facility as a whole grow as well. From years past most activities were resident centered. We visualize our facility as family oriented. As a family we laugh together, cry together and play together. We are excited about what the New Year has to offer. From our New Year, New You

3 Masquerade Party in January 2016 to our Fall Festival planned for October We are already in motion. By having the right people in place, Fatiha Benrbia 700 Hall Activity Assistant, Marquita James and Jacquelyn Carter West Wing Activity Assistants, and Zikeia Shannon East Wing Activity Assistant, 2016 will be our best year ever. Here is a small peak at our 2016 Calendar. Inside activities to include Scavenger Hunts, Red Hat Ladies, Happy Hours, Cooking Classes, and Quilting to name a few things that we have in store. I am so honored to be a part of this winning team. They are considered late after the tenth of the month. Any other business office questions can be directed to Annette Carter, BOM at ext.254. Medicaid questions should be directed to Erica Bethley at ext.256. Any questions regarding trust or insurances should be directed to Steven Driver at ext. 263 Payments can be made at the front desk seven days a week between the hours of 8am-8pm. We are here for you. Feel free to contact us with your questions. January Birthdays We would like to say a very Happy Birthday to our Staff and Residents celebrating their Birthdays in the Month of January. Rehab: The Heart of The City The Rehab Department at Lake City Nursing And Rehabilitation likes to think of themselves as the Heart of the Facility. The team is easily identified by their red scrubs donned everyday. The department consist of therapists who come in to participate in a variety of facility functions. The primary role is to evaluate and treat residents who present with a multitude of dysfunctions. Diagnoses range from cardiac, cognitive, musculo-skeletal, orthopedic and neurological in nature to simply ensuring proper footwear is in place to minimize fall risk. Once a resident comes onto the facility, they are screened to determine what their rehab needs are. Once this is identified, an evaluation is initiated. From there, we determine the appropriate plan of care to optimize return to prior level of

4 function. We currently have 3 full-time physical therapists, 2 part-time physical therapists, 3 full-time physical therapy assistants, 3 full-time occupational therapist,3 part-time occupational therapy assistants, 3 full-time speech therapists, and 2 rehabilitation technicians. We currently have a gym that competes with many out-patient facilities. We offer most modalities currently on the`` market. All of our therapists are modality certified and ready to assist with maximizing all levels of modality. Rehab works very closely with all other disciplines in the facility to ensure that your loved ones needs are met full-circle. Lake City's Dining Services 2016!!!!! Happy New Year from the dining services department. We are very excited about the 2016 dining program. Our focus will be creating a enchanting dining experience every meal, with Action Station cooking and improved customer service. Also with the start of the new year we will re-start the resident choice meal selection for both meals at the begenning of each month which will give residents enough time to come up with menu iteams selections.
