With healthcare providers in more locations than ever before, you have quick and convenient access to care, right when you need it. To get you started, this guide will help you find the right care, schedule an appointment, look up a location or find a specialist. 1

4 The Leading Edge of Convenience Easy Appointment Scheduling Request an appointment online at renown.org/appointments or call to schedule with our Customer Care Center. Medical Group We have more than 300 providers in 17 different specialties, including family medicine, internal medicine, pediatrics, OB-GYN and geriatrics. We also offer early morning and evening appointments at some of our primary care locations. Search for a provider at renown.org/findadoc or find more information at renown.org/medicalgroup. Urgent Care There are 10 locations throughout the region. Many offer evening and weekend hours, as well as on-site imaging and lab services. Virtual Check-In is a convenient way to get in line at a Renown Urgent Care location from the comfort of your home, office or even on the go. Find a location and check-in online at renown.org/virtualcheckin. Labs Renown Lab Services offers 14 locations throughout the region. Find locations and schedule an appointment at renown.org/lab. You may also call X-Ray & Imaging We offer the latest technology for screening and diagnostic imaging, including mammography, MRI, CT scan and X-Rays. 5 Virtual Visits Healthcare Comes to You Virtual Visits offer an alternative to Urgent Care. You can now see a medical provider by using your cell phone, tablet or computer. Virtual Visits available for ages 5 and up. Two Steps to Get Started with Virtual Visits 1. Request a Virtual Visit appointment: Fill out the online form at renown.org/virtualvisits or call Connect to your appointment: Use your computer or mobile device to automatically connect. 7 Virtual Check-In for Renown Urgent Care Virtual Check-In is a convenient way to get in line at a Renown Urgent Care location. Text notifications to your cell phone keep you informed about estimated times and when your turn is coming up. Virtual Check-In gives you the freedom to continue with your day until a medical provider is available to see you. Virtual Check-In FAQs Q: How do I use Virtual Check-In? A: Just follow these four easy steps: 1. Visit renown.org/virtualcheckin 2. Answer one quick question about your symptoms 3. Select an Urgent Care location 4. Enter your cell phone number and first name, then click Join Now Once you re in line, just sit back and relax, run errands or continue to work until it s your turn to be seen. Text notifications to your cell phone will keep you updated on your estimated time. You also have the option to add more time, get notified at a specific time, cancel your visit and more by sending text commands from your cell phone. Once you re at Urgent Care, a monitor will communicate your status by the last four digits or your phone number. Q: What text commands can I send? A: Commands you can send from your cell phone include: S L M Status Update Leave the Line need More time N# Notify me (#) of minutes before my turn (i.e. N30) H Help menu Q: What if I don t have a cell phone? A: Virtual Check-In is a service specifically designed to send updates to cell phones accepting text messages. If you don t have a cell phone, you can go to an Urgent Care location and check in at the front desk. 5

8 MyChart A Free, Secure Online Tool to Manage Your Health Information, 24/7 With MyChart, you have a one-stop place to: Schedule an appointment anytime, day or night Receive test results as soon as they re in Send a message to your healthcare team, privately and securely Request prescription refills Keep track of your family s health Review immunization records View or download your Summary of Care document How to Register for MyChart 1. Go to renown.org/mychart. 2. Click on Sign Up Now and complete the new member form. You will be asked to enter information and answer questions to verify your identity. 3. Create a MyChart username. 4. Create a MyChart password. 5. Enter your password reset question and answer. 6. Turn on notifications by confirming your address. 7. Accept Terms & Conditions. You can now view your health information.

9 Contact Us Knowing when to go to the nearest Emergency Room versus Urgent Care can help ensure you receive the care you need quickly. Do you need to go to the emergency room? Should you go to an urgent care center, or can you wait to see your doctor? The Nurse Health Line is staffed seven days a week, 24 hours a day by nurses who can help provide medical guidance.
